FUNDO:
O programa Lipid Research Clinics foi criado em 1971. Os objetivos foram avaliar as técnicas então vigentes para o diagnóstico da hiperlipoproteinemia; adquirir dados em todas as faixas etárias sobre a prevalência de diferentes tipos de hiperlipoproteinemia, particularmente formas determinadas geneticamente; coletar dados de alta qualidade sobre a prevalência e incidência de aterosclerose em diferentes padrões de hiperlipoproteinemia; e para melhorar os métodos de detecção, diagnóstico e cuidados médicos de doença cardíaca coronária. O programa consistia nos Estudos Populacionais e no Estudo de Prevenção Primária Coronária. O estudo de prevenção primária coronariana é descrito em Ensaios Clínicos no banco de dados.
Como resultado do estabelecimento do Programa Conjunto EUA-URSS em Doenças Cardiovasculares em 1972, clínicas de pesquisa lipídica foram estabelecidas em Moscou e Leningrado em 1974. O componente U.R.S.S. desta pesquisa colaborativa consistiu em dois Estudos de Prevalência e um Estudo de Acompanhamento, todos os quais seguiram de perto o protocolo de Estudo de Prevalência das Clínicas de Pesquisa Lipídica dos EUA.

Estudo de Prevalência:
O Prevalence Study foi iniciado em 1972 para determinar a prevalência de diferentes tipos de dislipoproteinemias em dez North American Lipid Research Clinics. O Estudo de Prevalência consistiu em duas triagens designadas como Visita 1 e Visita 2, e foi conduzido de acordo com um protocolo padronizado em populações-alvo bem definidas. A coleta de dados começou em 1972 e terminou em 1976. O tempo médio entre as duas telas foi de 96 dias. Os objetivos da Visita 1 foram: fornecer estimativas da prevalência de dislipidemia em populações específicas; investigar a distribuição de colesterol e triglicerídeos; e para selecionar participantes para a visita 2. Um total de 60.502 participantes elegíveis foram selecionados na visita 1. Uma amostra aleatória de 15 por cento (N = 9.107) de todos os participantes da Visita 1 que tinham lipídios elevados (N = 6.882) ou estavam tomando medicação para alteração de lipídios (N = 346) foi solicitada a retornar para a triagem da Visita 2 de Estudos Populacionais da Lipid Research Clinics . Os três grupos de 16.335 participantes representaram aproximadamente 25 por cento de todos os sujeitos triados na Visita 1. Os objetivos da Visita 2 foram: identificar participantes com dislipoproteinemia primária ou secundária; determinar a prevalência dos padrões lipídicos e lipoproteicos e suas associações com doença coronariana, outras doenças vasculares e outros fatores de risco para doença coronariana; e determinar as relações entre os padrões de lipídios e lipoproteínas e variáveis nutricionais, fisiológicas e sociodemográficas selecionadas, como educação e ocupação do chefe da família, tabagismo, pressão arterial, altura, peso, dobra cutânea tricipital, índice de Quetelet e uso de hormônios sexuais por mulheres . Os procedimentos da visita 2 incluíram uma entrevista, um exame físico, um ECG, química clínica e recordatório alimentar de um dia.
Estudo familiar:
O Family Study forneceu dados sobre a relação dos atributos familiares e genéticos com os lipídios e lipoproteínas plasmáticos. Uma amostra de todos os participantes da Visita 2 do Estudo de Prevalência foi escolhida para participar como probandos. Informações demográficas básicas foram coletadas e lipídios plasmáticos e lipoproteínas foram medidos em todos os parentes de primeiro grau e probandos. O Estudo da Família foi iniciado em 1975 e a coleta de dados foi concluída em 1978.
Estudo de Acompanhamento:
O Estudo de Acompanhamento, iniciado em 1977, foi projetado para relacionar a observação inicial feita em 1972-1976 com a mortalidade total e por causa específica. O estudo avaliou o status do fator de risco de triglicerídeos plasmáticos, colesterol HDL e eletrocardiografia de exercício para mortalidade por doença cardiovascular em geral e doença cardíaca coronária em particular. Medições basais de acompanhamento incluíram lipídios e outras químicas clínicas, recordatórios alimentares de um dia, histórico de medicamentos e exames físicos. Também foram feitas avaliações das relações do uso de estrogênio em mulheres com subsequente doença cardiovascular, doença cardíaca coronária e mortalidade por câncer e das relações dos níveis de lipídios e lipoproteínas com o câncer específico do local, bem como a relação dos níveis de retinol com a mortalidade por câncer .
