BAKGRUNN:
Lipid Research Clinics-programmet ble opprettet i 1971. Målene var å evaluere de nåværende teknikkene for diagnostisering av hyperlipoproteinemi; å skaffe data på tvers av alle aldersgrupper om utbredelsen av forskjellige typer hyperlipoproteinemi, spesielt genetisk bestemte former; å samle høykvalitetsdata om prevalens og forekomst av aterosklerose i forskjellige mønstre av hyperlipoproteinemi; og å forbedre metoder for påvisning, diagnose og medisinsk behandling av koronar hjertesykdom. Programmet besto av befolkningsstudier og Coronary Primary Prevention Trial. Den koronar primærforebyggende studien er beskrevet under Kliniske studier i databasen.
Som et resultat av etableringen av U.S.S.S.R. Joint Program in Cardiovascular Diseases i 1972, ble lipidforskningsklinikker etablert i Moskva og Leningrad i 1974. U.S.S.R.-komponenten av denne samarbeidsforskningen besto av to prevalensstudier og en oppfølgingsstudie, som alle fulgte tett protokollen for U.S.S.S.R. Clinics Prevalence Study.
DESIGN NARRATIV:
Prevalensstudie:
Prevalensstudien ble igangsatt i 1972 for å bestemme forekomsten av forskjellige typer dyslipoproteinemier ved ti nordamerikanske lipidforskningsklinikker. Prevalensstudien besto av to skjermbilder utpekt som besøk 1 og besøk 2, og ble utført i henhold til en standardisert protokoll i veldefinerte målpopulasjoner. Datainnsamlingen begynte i 1972 og ble avsluttet i 1976. Mediantiden mellom de to skjermene var 96 dager. Målene med besøk 1 var: å gi estimater av prevalensen av dyslipidemi i spesifiserte populasjoner; å undersøke fordelingen av kolesterol og triglyserid; og for å velge deltakere for besøk 2. Totalt 60 502 kvalifiserte deltakere ble screenet ved besøk 1. En 15 prosent tilfeldig prøve (N=9 107) av alle besøk 1-deltakere som hadde forhøyede lipider (N=6882) eller tok lipid-endrende medisiner (N=346) ble bedt om å returnere til skjermbildet Lipid Research Clinics Population Studies Visit 2 . De tre gruppene på 16 335 deltakere representerte omtrent 25 prosent av alle forsøkspersoner som ble screenet ved besøk 1. Besøk 2 prosedyrer inkluderte et intervju, en fysisk undersøkelse, et EKG, klinisk kjemi, og en-dags diett tilbakekalling.
Familiestudie:
Familiestudien ga data om forholdet mellom familiære og genetiske attributter til plasmalipider og lipoproteiner. Et utvalg av alle deltakerne i besøk 2 av prevalensstudien ble valgt til å delta som probander. Grunnleggende demografisk informasjon ble samlet inn og plasmalipider og lipoproteiner ble målt på alle førstegradsslektninger og probander. Familiestudien ble igangsatt i 1975 og datainnsamlingen ble fullført i 1978.
Oppfølgingsstudie:
Oppfølgingsstudien, initiert i 1977, ble designet for å relatere baseline-observasjonen som ble gjort i 1972-1976 til total og årsaksspesifikk dødelighet. Studien vurderte risikofaktorstatusen til plasmatriglyserider, HDL-kolesterol og treningselektrokardiografi for dødelighet av kardiovaskulær sykdom generelt, og koronar hjertesykdom spesielt. Oppfølging av baseline-målinger inkluderte lipid- og andre kliniske kjemier, en-dags dietttilbakekallinger, medisinhistorier og fysiske undersøkelser.
