SFONDO:
Il programma Lipid Research Clinics è stato creato nel 1971. Gli obiettivi erano valutare le tecniche allora correnti per la diagnosi dell'iperlipoproteinemia; acquisire dati in tutte le fasce di età sulla prevalenza dei diversi tipi di iperlipoproteinemia, in particolare delle forme geneticamente determinate; raccogliere dati di alta qualità sulla prevalenza e l'incidenza dell'aterosclerosi in diversi modelli di iperlipoproteinemia; e per migliorare i metodi per il rilevamento, la diagnosi e l'assistenza medica della malattia coronarica. Il programma consisteva negli studi sulla popolazione e nella sperimentazione sulla prevenzione primaria coronarica. A seguito dell'istituzione del programma congiunto USA-URSS per le malattie cardiovascolari nel 1972, le cliniche di ricerca sui lipidi sono state istituite a Mosca e Leningrado nel 1974. La componente dell'URSS di questa ricerca collaborativa consisteva in due studi sulla prevalenza e uno studio di follow-up, che seguivano tutti da vicino il protocollo dello studio sulla prevalenza delle cliniche di ricerca sui lipidi degli Stati Uniti.
PROGETTAZIONE NARRATIVA:
Studio sulla prevalenza:
Lo studio sulla prevalenza è stato avviato nel 1972 per determinare la prevalenza di diversi tipi di dislipoproteinemia presso dieci cliniche di ricerca sui lipidi nordamericane. Lo studio sulla prevalenza consisteva in due schermate designate come Visita 1 e Visita 2 ed è stato condotto secondo un protocollo standardizzato in popolazioni target ben definite. La raccolta dei dati è iniziata nel 1972 ed è terminata nel 1976. Il tempo mediano tra i due schermi è stato di 96 giorni. Gli obiettivi della Visita 1 erano: fornire stime della prevalenza della dislipidemia in popolazioni specificate; studiare la distribuzione del colesterolo e dei trigliceridi; e per selezionare i partecipanti per la visita 2. Un totale di 60.502 partecipanti idonei sono stati sottoposti a screening alla visita 1. A un campione casuale del 15% (N=9.107) di tutti i partecipanti alla Visita 1 che presentavano livelli elevati di lipidi (N=6.882) o stavano assumendo farmaci che alterano i lipidi (N=346) è stato chiesto di tornare per lo schermo della Visita 2 degli studi sulla popolazione delle cliniche di ricerca sui lipidi . I tre gruppi di 16.335 partecipanti rappresentavano circa il 25% di tutti i soggetti esaminati alla Visita 1. Gli obiettivi della Visita 2 erano: identificare i partecipanti con dislipoproteinemia primaria o secondaria; determinare la prevalenza dei pattern lipidici e lipoproteici e le loro associazioni con malattia coronarica, altre malattie vascolari e altri fattori di rischio per malattia coronarica; e per determinare le relazioni tra i modelli di lipidi e lipoproteine e variabili nutrizionali, fisiologiche e sociodemografiche selezionate come l'istruzione e l'occupazione del capofamiglia, il fumo, la pressione sanguigna, l'altezza, il peso, la plica del tricipite, l'indice di Quetelet e l'uso di ormoni sessuali da parte delle donne . Le procedure della visita 2 includevano un colloquio, un esame fisico, un ECG, analisi chimiche cliniche e richiamo dietetico di un giorno.
Studio familiare:
Il Family Study ha fornito dati sulla relazione degli attributi familiari e genetici con i lipidi e le lipoproteine plasmatiche. Un campione di tutti i partecipanti alla Visita 2 dello studio sulla prevalenza è stato scelto per partecipare come probandi. Sono state raccolte informazioni demografiche di base e sono stati misurati i lipidi plasmatici e le lipoproteine su tutti i parenti di primo grado e probandi. Il Family Study è stato avviato nel 1975 e la raccolta dei dati è stata completata nel 1978.
Studio di follow-up:
Lo studio di follow-up, avviato nel 1977, è stato progettato per mettere in relazione l'osservazione di riferimento effettuata nel 1972-1976 con la mortalità totale e causa-specifica. Lo studio ha valutato lo stato dei fattori di rischio dei trigliceridi plasmatici, del colesterolo HDL e dell'elettrocardiografia da sforzo per la mortalità per malattie cardiovascolari in generale e per le malattie coronariche in particolare. Le misurazioni basali di follow-up includevano i lipidi e altre sostanze chimiche cliniche, richiami dietetici di un giorno, anamnesi di farmaci ed esami fisici. Sono state inoltre effettuate valutazioni delle relazioni dell'uso di estrogeni nelle donne con successive malattie cardiovascolari, malattie coronariche e mortalità per cancro e delle relazioni dei livelli di lipidi e lipoproteine con cancro sito-specifico, nonché della relazione dei livelli di retinolo con la mortalità per cancro .
