ACHTERGROND:
Het Lipid Research Clinics-programma werd opgericht in 1971. De doelstellingen waren het evalueren van de toen geldende technieken voor de diagnose van hyperlipoproteïnemie; gegevens verzamelen over alle leeftijdsgroepen over de prevalentie van verschillende soorten hyperlipoproteïnemie, met name genetisch bepaalde vormen; gegevens van hoge kwaliteit verzamelen over de prevalentie en incidentie van atherosclerose in verschillende patronen van hyperlipoproteïnemie; en om methoden voor detectie, diagnose en medische zorg van coronaire hartziekten te verbeteren. Het programma bestond uit de Populatiestudies en de Coronaire Primaire Preventie Trial. De coronaire primaire preventiestudie wordt beschreven onder Klinische onderzoeken in de database.
Als resultaat van de oprichting van het U.S.-U.S.S.R. Joint Program in Cardiovascular Diseases in 1972, werden in 1974 lipidenonderzoeksklinieken opgericht in Moskou en Leningrad. De U.S.S.R.-component van dit gezamenlijke onderzoek bestond uit twee prevalentiestudies en een follow-upstudie, die alle het protocol van de U.S. Lipid Research Clinics Prevalentiestudie nauwlettend volgden.
ONTWERP VERHAAL:
Prevalentieonderzoek:
De prevalentiestudie werd in 1972 gestart om de prevalentie van verschillende soorten dyslipoproteïnemieën in tien Noord-Amerikaanse Lipid Research Clinics te bepalen. Het prevalentieonderzoek bestond uit twee schermen, aangeduid als Bezoek 1 en Bezoek 2, en werd uitgevoerd volgens een gestandaardiseerd protocol bij goed gedefinieerde doelpopulaties. Het verzamelen van gegevens begon in 1972 en eindigde in 1976. De mediane tijd tussen de twee schermen was 96 dagen. De doelstellingen van Bezoek 1 waren: schattingen geven van de prevalentie van dyslipidemie in gespecificeerde populaties; de verdeling van cholesterol en triglyceriden onderzoeken; en om deelnemers te selecteren voor bezoek 2. In totaal werden 60.502 in aanmerking komende deelnemers gescreend bij bezoek 1. Een willekeurige steekproef van 15 procent (N=9.107) van alle Visit 1-deelnemers die verhoogde lipiden hadden (N=6.882) of lipidenveranderende medicatie gebruikten (N=346) werd gevraagd om terug te komen voor het Lipid Research Clinics Population Studies Visit 2-scherm . De drie groepen van 16.335 deelnemers vertegenwoordigden ongeveer 25 procent van alle proefpersonen die bij Bezoek 1 werden gescreend. De doelstellingen van Bezoek 2 waren: deelnemers met primaire of secundaire dyslipoproteïnemie identificeren; om de prevalentie van lipiden- en lipoproteïnepatronen en hun associaties met coronaire hartziekten, andere vaatziekten en andere risicofactoren voor coronaire hartziekten te bepalen; en om de relaties tussen lipiden- en lipoproteïnepatronen en geselecteerde nutritionele, fysiologische en sociodemografische variabelen te bepalen, zoals opleiding en beroep van het gezinshoofd, roken, bloeddruk, lengte, gewicht, tricepshuidplooi, Quetelet-index en gebruik van geslachtshormonen door vrouwen . Bezoek 2 procedures omvatten een interview, een lichamelijk onderzoek, een ECG, klinische chemie en een eendaagse voedingsherinnering.
Gezinsstudie:
De familiestudie leverde gegevens op over de relatie tussen familiale en genetische kenmerken en plasmalipiden en lipoproteïnen. Een steekproef van alle deelnemers aan bezoek 2 van de prevalentiestudie werd gekozen om deel te nemen als proefpersonen. Demografische basisinformatie werd verzameld en plasmalipiden en lipoproteïnen werden gemeten bij alle eerstegraads familieleden en probands. De gezinsstudie werd in 1975 gestart en de gegevensverzameling werd in 1978 voltooid.
Vervolgstudie:
De vervolgstudie, gestart in 1977, was bedoeld om de basiswaarneming die in 1972-1976 werd gedaan, te relateren aan de totale en oorzaakspecifieke mortaliteit. De studie beoordeelde de risicofactorstatus van plasmatriglyceriden, HDL-cholesterol en inspanningselektrocardiografie voor mortaliteit door hart- en vaatziekten in het algemeen en coronaire hartziekten in het bijzonder. Follow-up basislijnmetingen omvatten lipide en andere klinische chemie, eendaagse terugroepacties, medicatiegeschiedenis en lichamelijk onderzoek. Er werden ook beoordelingen gemaakt van de relaties tussen oestrogeengebruik bij vrouwen en daaropvolgende hart- en vaatziekten, coronaire hartziekten en kankersterfte en van de relaties tussen lipiden- en lipoproteïnespiegels en plaatsspecifieke kanker, evenals de relatie tussen retinolspiegels en kankersterfte. .
