Critério de inclusão:
- Homens e mulheres entre 18 e 80 anos de idade (inclusive) que são capazes de compreender as instruções e seguir os procedimentos do estudo e estão dispostos a assinar um Termo de Consentimento Livre e Esclarecido (TCLE)
- As mulheres com potencial para engravidar que são sexualmente ativas com um parceiro não estéril (parceiros estéreis são definidos como homens vasectomizados há pelo menos 6 meses) devem estar dispostas a usar os métodos contraceptivos durante o estudo e por 30 dias após a administração do medicamento do estudo.
- Por pelo menos 90 dias após a administração do medicamento do estudo, os homens não vasectomizados não devem doar esperma, estar dispostos a usar contracepção com parceiras em potencial para engravidar e qualquer indivíduo do sexo masculino com uma parceira grávida deve usar preservativo.
- Disposto a se abster de consumir produtos contendo toranja, pomelo, carambola ou laranja de Sevilha de 7 dias antes da dose da medicação do estudo até o dia da alta.
- Diagnóstico confirmado de PBC com evidência de cirrose e classificação de Child-Pugh de CP-A, CP-A + PHT, CP-B ou CP-C
Parâmetros laboratoriais de triagem:
- ALP, ALT e AST < 10 × LSN
- Bilirrubina total ≤ 5 × LSN
- Ácido ursodesoxicólico (UDCA) por um mínimo de 12 semanas de tratamento antes do Dia 1
- Na triagem confirmou o diagnóstico de PBC
- Escores MELD-Na de 6 a 24
Critério de exclusão:
- Clinicamente significativo ou história de doença hepática aguda ou crônica de etiologia diferente da CBP
- Pacientes com diagnóstico de sobreposição de CBP e hepatite autoimune
- Histórico, evidência ou alta suspeita de malignidade hepatobiliar com base em exames de imagem, triagem de valores laboratoriais e/ou sintomas clínicos.
- Infecção presumida ou diagnosticada que requer terapia sistêmica dentro de 12 semanas após a triagem e até o dia 1
- Sujeitos do sexo feminino que estão grávidas ou amamentando
- ECG de triagem que demonstra um intervalo QT ≥ 500 ms ou qualquer outro achado significativo de ECG com anormalidades clinicamente significativas, conforme determinado pelo investigador
- Positivo para HBsAg, HCV RNA ou anticorpo anti-HIV
- Qualquer condição aguda ou crônica não hepática que, na opinião do investigador, limitaria a capacidade do paciente de concluir e/ou participar do estudo ou comprometer a integridade dos dados
- Teve uma doença que é considerada pelo investigador como clinicamente significativa dentro de 2 semanas antes da administração do produto experimental
- Abuso de drogas ou álcool clinicamente relevante dentro de 6 meses da triagem. Uma triagem de drogas positiva excluirá indivíduos, a menos que possa ser explicada por um medicamento prescrito
- Uso de ácido obeticólico (OCA), qualquer medicamento da mesma classe ou fibratos (por exemplo, bezafibrato, fenofibrato, elafibranor, lanifibranor, pemafibrato, saroglitizar) dentro de 30 dias da linha de base
- Uso de um tratamento experimental ou não aprovado para PBC dentro de 30 dias da linha de base
- Complicação(ões) clinicamente evidente(s) de cirrose e hipertensão portal que exigiram visita ao pronto-socorro, internação hospitalar ou ambos durante o período de 12 semanas antes da administração do produto experimental

Participants with primary biliary cholangitis (PBC) and a Child-Pugh (CP) classification of CP-A (score 5 to 6) without portal hypertension (PHT) will receive a single dose of seladelpar 10 mg, orally, on Day 1.
