Studie efter godkännande av Wavefront-guided LASIK för monovisionbehandling av närsynta personer med presbyopi

Denna studie kommer att vara en 12 månader lång, prospektiv, multicenter, öppen, icke-jämförande, icke-randomiserad klinisk undersökning som genomförs på 8 till 14 platser. Totalt 383 försökspersoner kommer att registreras för att uppnå cirka 268 behandlade försökspersoner, vilket resulterar i minst 241 utvärderbara försökspersoner vid punkten för brytningsstabilitet. Efter att ha undertecknat det informerade samtycket kan försökspersoner som uppfyller alla inklusions- och uteslutningskriterier i båda ögonen schemaläggas för operation. Uppföljningsbesöksschemat kommer att vara detsamma för varje operativt öga.

Inklusionskriterier:

  1. Undertecknat informerat samtycke och HIPAA-auktorisering.
  2. Minst 40 år vid registreringen (datum undertecknat samtycke).
  3. Brytningsfelet i båda motivets ögon, baserat på iDesign-brytningen som valts för behandling ("4,0 mm Rx calc" vid 12,5 mm), måste vara närsynt med eller utan astigmatism upp till -6,00 D sfärisk ekvivalent (SE), med cylinder upp till -3,00 D och minsta preoperativ närsynthet i deras icke-dominanta öga minst lika stor som deras målinriktade närsynthetsretention.
  4. Minst ett öga måste kräva en närsynt behandling.
  5. Förväntad stromabäddtjocklek på minst 250 mikron baserat på preoperativ central korneal pachymetri minus det maximala behandlingsdjup som ska ableras (som beräknat av iDesign plus den avsedda fliktjockleken).
  6. Avstånd Bästa glasögonkorrigerad synskärpa (BSCVA) på 20/20 eller bättre.
  7. Kräv en tilläggseffekt på +1,25 D eller mer men inte större än +2,00 D vid 40 cm.
  8. Mindre än eller lika med ±0,50 D skillnad mellan cykloplegisk och manifest refraktionssfär.
  9. Ett stabilt brytningsfel (baserat på en tidigare undersökning, medicinska journaler, linsometri eller recept som ligger minst 12 månader före den preoperativa manifesta brytningen), som definieras av en förändring på mindre än eller lika med

    • 0,50 D i MRSE (sfär och cylinder).
  10. Försökspersoner med en historia av kontaktlinsbruk under de senaste 4 veckorna måste avbryta linsbruket för att uppvisa brytningsstabilitet enligt följande (brytningsstabilitet definieras som en förändring på högst 0,50 D i manifest refraktionssfär och cylinder samt genomsnittlig keratometri mellan mätningar): Efter att processen för informerat samtycke har slutförts och försökspersoner har registrerats i studien:

    • Styva eller långvariga kontaktlinser (toriska eller sfäriska) måste tas bort i minst 3 veckor och mjuka kontaktlinser (toriska eller sfäriska) i minst 2 veckor innan den första refraktionen används för att etablera stabilitet.
    • Efter att ha avstått från att använda kontaktlinser under angiven tid måste två på varandra följande manifesta refraktioner och keratometriska avläsningar utföras med minst 7 dagars mellanrum
    • I Om försökspersonen/ögat uppfyller kriterierna för brytningsstabilitet, är det inte tillåtet att använda kontaktlinser under 2 veckor före operationen förutom när det krävs för att testa monovision-acceptans enligt beskrivningen nedan i avsnittet Studieprocedurer, Kontaktlinsförsök OBS: Refraktiv stabilitet måste vara bekräftades endast en gång under preoperativ bedömning
  11. Överensstämmelse mellan manifest refraktion (justerad för optisk oändlighet) och iDesign System refraktion valt för behandling, enligt följande:

    • Sfärisk ekvivalent: storleken på skillnaden är mindre än 0,625 D.
    • Cylinder: storleken på skillnaden är mindre än eller lika med 0,5 D.
    • Cylinderaxeltolerans: Om antingen manifestcylindern som införts i iDesign-systemet eller iDesign-cylindern som valts för behandling är mindre än 0,5 D, finns det inget krav på axelöverensstämmelse. När båda cylindrarna har en storlek på minst 0,5 D reduceras axeltoleransen, som bestäms av iDesign-systemet, linjärt från 15 grader (0,5 D) till 7,5 grader (7,0 D) baserat på den genomsnittliga magnituden för båda cylindrarna. Obs: Om axeltoleransen inte ligger inom det beräknade området kommer iDesign-systemet att ge en varning och denna undersökning kanske inte används för behandlingsplanering.
  12. Villig och kapabel att följa uppföljande undersökningar under hela studietiden.

Exklusions kriterier:

  1. Kvinnor som är gravida, ammar eller tänker bli gravida under studien.

    Obs: Kvinnor som var gravida eller ammade får inte skrivas in förrän 6 månader efter antingen förlossningen eller har slutat amma och det finns dokumenterad brytningsstabilitet.

  2. Samtidig användning av systemiska (inklusive inhalerade) läkemedel som kan försämra läkningen, inklusive men inte begränsat till: antimetaboliter, isotretinoin (Accutane®) inom 6 månaders behandling och amiodaronhydroklorid (Cordarone®) inom 12 månaders behandling. OBS! Användning av inhalerade eller systemiska kortikosteroider, vare sig de är kroniska eller akuta, inom de senaste 6 månaderna anses ha negativ inverkan på läkningen och försökspersoner som använder sådana mediciner är specifikt uteslutna från behörighet.
  3. Historik av något av följande medicinska tillstånd, eller något annat tillstånd som kan påverka sårläkning: kollagenkärlsjukdom, autoimmun sjukdom, immunbristsjukdomar, okulär herpes zoster eller herpes simplex, endokrina störningar (inklusive men inte begränsat till instabila sköldkörtelsjukdomar och diabetes ), lupus och reumatoid artrit.

    OBS: Närvaron av diabetes (antingen typ 1 eller 2), oavsett sjukdomens varaktighet, svårighetsgrad eller kontroll, kommer specifikt att utesluta försökspersoner från berättigande.

  4. Försökspersoner med en pacemaker, implanterad defibrillator eller annan implanterad elektronisk enhet.
  5. Anamnes på tidigare intraokulär eller hornhinnekirurgi (inklusive kataraktextraktion), aktiv oftalmisk sjukdom eller abnormitet (inklusive, men inte begränsat till, symptomatisk blefarit, återkommande hornhinneerosion, torra ögonsyndrom, neovaskularisering > 1 mm från limbus), näthinneavlossning/reparation, kliniskt signifikant linsopacitet, kliniska tecken på trauma, korneal opacitet inom de centrala 9 mm och synlig på topografi, tidigare skelning, skelning eller risk för att utveckla skelning, eller med tecken på glaukom eller benägenhet för trångvinkelglaukom eller misstänkt glaukom.

  6. Bevis på keratokonus, hornhinnedystrofi eller oregelbundenhet eller onormal topografi.
  7. Känd känslighet eller olämplig lyhördhet för någon av de mediciner som används i det postoperativa förloppet.
  8. Intolerans mot monovisionkorrigering baserat på frågeformulärsvar på kontaktlinsförsök (se avsnittet Studieprocedurer, Kontaktlinsförsök nedan).

PROWL-M driving scale consists of three driving related items that were used to generate the driving scale score. Each item response was converted to a 0 to 100 scale so that the lowest and highest possible scores were set at 0 (not functioning) and 100 (functioning) points. A high score represents better functioning. The composite driving scale score was calculated by averaging the scores of all the three items, resulting in a total score ranging from 0 to 100 where higher score represent better functioning. Responses was collected in 6 categories: (1) completely satisfied, (2) very satisfied, (3) somewhat satisfied, (4) somewhat dissatisfied, (5) very dissatisfied, (6) completely dissatisfied. A questionnaire derived from the PROWL-M was given to participants after surgery. Satisfaction with LASIK via PROWL-M questionnaire was assessed with 8 items. Responses to individual items were linearly transformed to a 0 to 100 scale where 100 indicates best possible vision and satisfaction, while 0 indicates worst possible vision and satisfaction. Each item score was averaged together to produce a score ranging from 0 to 100. A higher score indicating greater satisfaction.
