美利坚合众国超重和肥胖的流行及其合并症继续扩大。 减肥手术已被证明是控制严重肥胖及其合并症的最有效和最持久的方法。 例如,2 型糖尿病在 76.8% 中完全解决,全身性动脉高血压在 61.7% 中得到解决,血脂异常在 70% 中得到改善,阻塞性睡眠呼吸暂停低通气综合征在 85.7% 中得到解决。 此外,减肥手术可显着延长预期寿命 (89%) 并降低总体死亡率 (30-40%),尤其是糖尿病、心脏病和癌症导致的死亡率。 最后,关于与减肥手术相关的下游节省的初步证据在 2 至 4 年内抵消了初始成本。
自 1998 年以来,减肥手术有了显着的进步。 2005 年,美国代谢和减肥手术协会 (ASMBS) 报告说,81% 的减肥手术是通过腹腔镜进行的。 2007 年,美国有 205,000 人接受了减肥手术,其中大约 80% 是胃旁路手术。 此外,减肥手术的资格和接受情况之间存在不匹配,只有不到 1% 的合格人群通过减肥手术治疗病态肥胖。 随着选择性初次减肥手术数量的增加,多达 20% 的 RYGB 术后患者在初次减肥手术后无法维持体重减轻超过 2 至 3 年。 因此,因减重不佳而进行的修正手术和因技术或机械并发症而进行的再手术将同时增加。

Long-term control of typical symptoms after primary laparoscopic Nissen fundoplication "LNF", in properly selected patients and with an experienced surgical team, is attained in more than 90 percent of patients confirming that LNF is the gold standard for the treatment of severe GERD.
