对比剂给药后肾功能不全试验 IV (REMEDIAL IV) 的目的是测试使用 DyeVert 系统是否能有效降低 ACS 患者接受紧急/立即(2 小时内)侵入性诊断和/或介入心血管手术。 DyeVert™ 系统(Osprey Medical Inc.,明尼苏达州明尼苏达州,美国)是一种新型设备,旨在减少冠脉手术期间的 CM 体积,同时保持透视图像质量。

方法 所有计划接受紧急/立即冠状动脉造影/血管成形术的 ACS 患者都将根据纳入/排除标准进行筛选。 ACS(ST 段抬高型心肌梗死 [STEMI] 和高危非 ST 段抬高型心肌梗死 [Non-STEMI])的诊断将根据指南进行,包括典型的胸痛病史、诊断性心电图变化和心脏生物标志物的连续增加。 所有满足纳入/排除标准并同意签署知情同意书的患者将被纳入试验。 REMEDIAL IV 试验将根据赫尔辛基宣言和良好临床实践的原则在意大利介入心脏病学中心进行,并已获得当地伦理委员会的批准。

纳入标准:
- 在急性冠状动脉综合征的情况下使用含碘造影剂进行紧急或立即(2 小时内)冠状动脉手术:
ST 段抬高型心肌梗死(根据心肌梗死的第四个通用定义);
高危非 ST 段抬高型心肌梗死(根据现行指南):
- 难治性心绞痛,
- 心力衰竭或新的或恶化的二尖瓣反流的体征或症状,
- 血流动力学不稳定,
- 尽管进行了强化药物治疗,但在静息或低水平活动时反复发生心绞痛或缺血,
- 持续性室性心动过速或心室颤动,
- 反复动态 ST-T 波变化,特别是间歇性 ST 抬高。
排除标准:
- 怀孕的妇女。
- 近期造影剂暴露:48 小时内造影剂暴露。
- 终末期慢性肾病慢性透析:血液透析和腹膜透析。
- 多发性骨髓瘤。
