Wirkung der transkraniellen Gleichstromstimulation (tDCS) auf den Tabakkonsum.
Der Hauptzweck des Protokolls besteht darin, die Wirkung einer wiederholten Anwendung eines echten tDCS an 5 aufeinanderfolgenden Tagen im Vergleich zur Anwendung eines Placebo-tDCS (Scheinverfahren) auf die kurzfristige Entwicklung des Tabakkonsums zwischen Tag 1 und Tag 5 zu bewerten .
Die Studienhypothese ist, dass eine wiederholte Anwendung eines echten tDCS an 5 aufeinanderfolgenden Tagen auf der linken dorsolateralen präfrontalen Kortexregion das induzierte Verlangen verringert, was zu einem Rückgang des täglichen Tabakkonsums zwischen Tag 1 und Tag 5 führt, der beim letzten Besuch zwischen Tag bestehen bleiben kann 15 und Tag 20.

Beschreibung
Einschlusskriterien:
- Im Alter von 18 bis 60 Jahren
- Rauchen ≥ 5 Jahre
- Rauchen von ≥ 15 Industriezigaretten pro Tag oder ≥ 10 selbstgedrehten Zigaretten pro Tag
- Fagerström-Test ≥ 5
- Raucher, die mindestens 7 Tage lang versucht haben, mit dem Rauchen aufzuhören
- Vom Patienten unterzeichnete schriftliche Einverständniserklärung
- Angeschlossen an eine Krankenversicherung
Ausschlusskriterien:
- Koabhängigkeit (Cannabis, Alkohol, andere Drogen)
- HAD-Test: D ≥ 8; A + D ≥ 19
- Behandlung durch Nikotinersatztherapie, Bupropion oder Vareniclin innerhalb von 30 Tagen vor Aufnahme
- Neuropsychiatrische Erkrankung, die vom Prüfer als schwerwiegend eingestuft wird
- Psychopharmaka-Behandlung (Antidepressivum, Anxiolytikum, Antipsychotikum)
- Dermatose der Kopfhaut
- Schwangerschaft oder Stillzeit. Positiver Schwangerschaftstest.
- Patient unter Vormundschaft, Treuhandschaft oder gerichtlichem Schutz
- Patient im Einschlusszeitraum für ein anderes klinisches Forschungsprotokoll

Experimental: Echtes tDCS
Direkte transkranielle Elektrostimulation (tDCS-Verfahren)

Das tDCS ist eine nicht-invasive Neuromodulationsmethode.
Ein Stimulator liefert einen konstanten Strom geringer Intensität (1-2 mA), der 20 Minuten lang an den Durchschnitt zweier bipolarer Elektroden angelegt wird, die auf der Kopfhaut angebracht sind.
Dieser konstante Strom erzeugt ein statisches elektrisches Feld, das die Aktivität kortikaler Neuronen selektiv moduliert.
Zu Beginn und am Ende der Stimulation tritt ein Kribbeln unter den Elektroden auf, das jedoch vorübergehend ist und nach 30 oder 60 Sekunden schnell verschwindet.

Schein-Komparator: Placebo-tDCS
Scheinverfahren
|
Das tDCS-Placebo unterscheidet sich vom echten tDCS durch die Unterbrechung der Stimulation nach 30 bis 60 Sekunden und die Reaktivierung dieser Stimulation 30 bis 60 Sekunden vor dem Ende der Sitzung, die 20 Minuten dauerte.
Das Kribbeln zu Beginn und am Ende der Sitzung wird das gleiche sein wie bei einer echten Stimulation.
Die Elektrodeneinrichtung unterscheidet sich nicht von der tDCS.

Messung des Verlangens von Tag 1 bis Tag 5 und bei den letzten Besuchen, bewertet durch ein VAS: visuelle Analogskala von 0 (überhaupt kein Drang zum Rauchen) bis 10 (starkes Verlangen, das zum Rauchen drängt) und der Fragebogen mit der Bezeichnung „French Tobacco Craving Questionnaire“ mit 12 Punkten (FCTQ-12).
