La cohorte se creó en 2005 con el objetivo general de definir mejor la historia natural de la coinfección VHC/VIH en términos de morbilidad y mortalidad. Además, la cohorte tiene como objetivo determinar los factores asociados con la morbilidad y la mortalidad y comprender mejor las interacciones entre los dos virus y sus respectivos tratamientos.
El ANRS CO13 Hepavih está compuesto actualmente por 28 centros clínicos metropolitanos franceses de enfermedades infecciosas y hepatológicas (en su mayoría hospitales universitarios).
El periodo de inclusión de pacientes se ha dividido en tres fases bien diferenciadas: la primera fase (Fase 1) que comenzó en 2005 y finalizó en 2008, estuvo dedicada a la inclusión de pacientes adultos coinfectados por VHC/VIH. La siguiente fase de inclusión (Fase 2) que se refiere a pacientes que recibieron una terapia anti-VHC de triple agente que incluye Boceprevir o Telaprevir, comenzó en 2008 y finalizó en marzo de 2016. La tercera (Fase 3) centrada en pacientes que inician un tratamiento anti-VHC AAD se inició en junio de 2014 y finalizó en noviembre de 2015. Con respecto a la Fase 3, los objetivos específicos se organizaron en cinco dominios temáticos que incluyen clínica, virología, farmacología, salud pública y metodología.
Desde el 31 de marzo de 2016 se cierran las inclusiones y se han incluido 1.850 pacientes. La finalización del seguimiento está prevista para el 31 de diciembre de 2018.
En el momento de la inclusión y durante el período de seguimiento (al menos una vez al año o cada 6 meses en caso de cirrosis, hasta 2018 y probablemente también más allá de esa fecha), a cada paciente coinfectado por el VHC/VIH se le ofrece también un examen clínico como una entrevista. Los pacientes incluidos completan autocuestionarios adicionales en los mismos momentos (datos de ciencias sociales). Los tipos de datos recopilados de cada paciente coinfectado incluido en la cohorte se organizan de acuerdo con cinco clases temáticas principales: sociodemografía y comportamiento, características del VIH, características del VHC, eventos clínicos relevantes, antropometría, datos paraclínicos.
La cohorte ANRS CO13 HEPAVIH tiene como objetivo convertirse en un centro de investigación específico sobre los aspectos fisiopatológicos y terapéuticos de la coinfección VHC/VIH a disposición de toda la comunidad científica.
En julio de 2016, la cohorte ha publicado 29 artículos originales y 10 como parte de colaboraciones nacionales y europeas.
La cohorte ANRS CO13 HEPAVIH ha adoptado una gobernanza funcional. El Comité Directivo, compuesto por médicos, metodólogos, el patrocinador y los coordinadores de la cohorte, se centra en los aspectos logísticos y operativos de la cohorte. El Comité Científico es el órgano científico decisorio, formado por científicos expertos en los principales campos de investigación, como metodología, hepatología, infectología, farmacología, virología y ciencias sociales, y representantes de la ANRS y de las organizaciones de pacientes. Evalúa las solicitudes de acceso a la cohorte y brinda asesoramiento sobre aspectos científicos, metodológicos y éticos para garantizar una conducción científica eficaz de la investigación. El Comité Directivo se reúne una vez al mes y el Comité Científico 2/3 veces al año.

Criterios de inclusión:

- Pacientes de ≥18 años,
- Cobertura por una seguridad social,
- Capaz de entender el consentimiento conforme.
Fase 1:
- VIH-1/VHC con infección crónica (prueba de anticuerpos contra el VIH positiva y ARN del VHC positivo al inicio o negativo después del tratamiento anti-VHC)
Fase 2:
- Pacientes con infección crónica por VIH-1/VHC (prueba de anticuerpos contra el VIH positiva y ARN del VHC positivo al inicio o negativo después del tratamiento anti-VHC) tratados con un tratamiento basado en un inhibidor de la proteasa (PegIFN, Ribavirina y Telaprevir o Boceprevir) o que hayan eliminado el VHC espontáneamente en ausencia del tratamiento anti-VHC
Fase 3:
- Infección crónica por VIH-1/VHC (prueba de anticuerpos contra el VIH positiva y ARN del VHC positivo al inicio o negativo después del tratamiento anti-VHC) tratada con tratamiento anti-VHC DAA (todas las combinaciones)
Criterio de exclusión:
- Pacientes <18 años
- Poblaciones denominadas vulnerables (menores de edad, personas bajo tutela o tutela, o persona física bajo tutela de tomar decisiones judiciales o administrativas)
- Sin un seguro de salud

Mejorar la eficacia y la calidad de la atención médica en Hepatitis C en pacientes con VIH
Periodo de tiempo: Hasta la finalización del estudio: una visita/año para cada participante + una visita adicional cada 6 meses para pacientes con cirrosis

Los CRF recopilan datos clínicos, biológicos, genéticos y virológicos: eventos clínicos, tratamientos (VIH, HVC, otros), exámenes clínicos (talla, peso, circunferencia de cadera y cintura), virológicos: CD4, CD8, ARN del VIH, ARN del HVC (+ genotipo), serología HVB, pruebas de función hepática y biología (fibroscan, fibrotest, ecografía, transaminasas, bilirrubina, fosfatasa alcalina, gamma GT,...), bioquímica metabólica (glucemia, insulina, colesterol, creatinina, lipasa).
Los pacientes también completaron cuestionarios de ciencias sociales (calidad de vida, depresión,...) Durante la visita anual se toman muestras: sangre para ADN, suero y plasma y se almacenan en un biobanco utilizado para proyectos científicos.
