Detaillierte Beschreibung
Die Kohorte wurde 2005 mit dem allgemeinen Ziel erstellt, den natürlichen Verlauf der HCV/HIV-Koinfektion im Hinblick auf Morbidität und Mortalität besser zu definieren. Darüber hinaus zielt die Kohorte darauf ab, Faktoren im Zusammenhang mit Morbidität und Mortalität zu bestimmen und die Wechselwirkungen zwischen den beiden Viren und ihren jeweiligen Behandlungen besser zu verstehen.
Das ANRS CO13 Hepavih besteht derzeit aus 28 französischen großstädtischen klinischen Zentren für Infektionskrankheiten und Lebererkrankungen (meistens Lehrkrankenhaus).
Der Zeitraum der Patientenaufnahme wurde in drei verschiedene Phasen unterteilt: Die erste Phase (Phase 1), die 2005 begann und 2008 endete, war der Aufnahme von erwachsenen HCV/HIV-koinfizierten Patienten gewidmet. Die folgende Einschlussphase (Phase 2), die Patienten betrifft, die eine Dreifach-Anti-HCV-Therapie einschließlich Boceprevir oder Telaprevir erhalten haben, begann 2008 und endete im März 2016. Die dritte (Phase 3) konzentrierte sich auf Patienten, die eine DAA-Anti-HCV-Behandlung begannen, begann im Juni 2014 und endete im November 2015. In Phase 3 wurden die spezifischen Ziele in fünf thematische Bereiche eingeteilt, darunter Klinik, Virologie, Pharmakologie, öffentliche Gesundheit und Methodik.
Seit dem 31. März 2016 sind die Einschlüsse geschlossen und 1.850 Patienten wurden eingeschlossen. Das Ende des Follow-ups ist für den 31. Dezember 2018 geplant.
Beim Einschluss und während der Nachbeobachtungszeit (mindestens einmal jährlich bzw. alle 6 Monate bei Zirrhose bis 2018 und voraussichtlich auch darüber hinaus) wird jedem HCV/HIV-koinfizierten Patienten auch eine klinische Untersuchung angeboten als Vorstellungsgespräch. Zusätzliche Selbstfragebögen werden von den eingeschlossenen Patienten zu den gleichen Zeitpunkten ausgefüllt (Sozialwissenschaftliche Daten). Die Arten von Daten, die von jedem koinfizierten Patienten in der Kohorte gesammelt werden, sind nach fünf thematischen Hauptklassen organisiert: Soziodemografie und Verhalten, HIV-Merkmale, HCV-Merkmale, klinisch relevante Ereignisse, Anthropometrie, paraklinische Daten.
Die ANRS CO13 HEPAVIH-Kohorte hat das Ziel, eine Einrichtung für spezifische Forschung zu den physiopathologischen und therapeutischen Aspekten der HCV/HIV-Koinfektion zu werden, die der gesamten wissenschaftlichen Gemeinschaft zur Verfügung steht.
Im Juli 2016 hat die Kohorte 29 Originalartikel und 10 im Rahmen nationaler und europäischer Kooperationen veröffentlicht.
Die ANRS CO13 HEPAVIH-Kohorte hat eine funktionale Governance übernommen. Der Lenkungsausschuss, bestehend aus Klinikern, Methodikern, dem Sponsor sowie den Kohortenkoordinatoren, konzentriert sich auf logistische und operative Aspekte der Kohorte. Das Wissenschaftliche Komitee ist die wissenschaftliche Entscheidungsfindungsorganisation, die sich aus wissenschaftlichen Experten in den Hauptforschungsbereichen wie Methodik, Hepathologie, Infektiologie, Pharmakologie, Virologie und Sozialwissenschaften sowie Vertretern der ANRS und Patientenorganisationen zusammensetzt. Es bewertet Anträge auf Zugang zur Kohorte und berät zu wissenschaftlichen, methodischen und ethischen Aspekten, um eine effektive wissenschaftliche Durchführung der Forschung zu gewährleisten. Der Lenkungsausschuss trifft sich einmal im Monat und der Wissenschaftliche Ausschuss 2/3 mal im Jahr.

Einschlusskriterien:
Alle Phasen der Inklusion:
- Patienten im Alter von ≥ 18 Jahren,
- Absicherung durch eine Sozialversicherung,
- In der Lage, die Einwilligungskonformität zu verstehen.
Phase 1:
- Chronisch mit HIV-1/HCV infiziert (positiver HIV-Antikörpertest und HCV-RNA-positiv zu Beginn oder negativ nach Anti-HCV-Behandlung)
Phase 2:
- Chronisch mit HIV-1/HCV infiziert (positiver HIV-Antikörpertest und HCV-RNA-positiv zu Beginn oder negativ nach Anti-HCV-Behandlung), die mit einer auf Protease-Inhibitoren basierenden Behandlung (PegIFN, Ribavirin und Telaprevir oder Boceprevir) behandelt werden oder die HCV spontan in Abwesenheit beseitigt haben der Anti-HCV-Behandlung
Phase 3:
- Chronisch mit HIV-1/HCV infiziert (positiver HIV-Antikörpertest und HCV-RNA-positiv zu Beginn oder negativ nach Anti-HCV-Behandlung), behandelt mit DAA-Anti-HCV-Behandlung (alle Kombinationen)
Ausschlusskriterien:
- Patienten <18 Jahre alt
- Sogenannte gefährdete Bevölkerungsgruppen (Minderjährige, Personen unter Vormundschaft oder Schutz oder eine Privatperson, die vor gerichtlichen oder behördlichen Entscheidungen geschützt ist)
- Ohne Krankenversicherung

Die CRFs erfassen klinische, biologische, genetische und virologische Daten: Klinische Ereignisse, Behandlungen (HIV, HVC, andere), klinische Untersuchungen (Größe, Gewicht, Hüft- und Taillenumfang), virologisch: CD4, CD8, HIV-RNA, HVC-RNA (+ Genotyp), HVB-Serologie, Leberfunktionstests und Biologie (Fibroscan, Fibrotest, Ultraschall, Transaminasen, Bilirubin, alkalische Phosphatase, Gamma-GT,...), metabolische Biochemie (Glykämie, Insulin, Cholesterin, Kreatinin, Lipase).
Die Patienten füllten auch sozialwissenschaftliche Fragebögen aus (Lebensqualität, Depression, ...). Während des jährlichen Besuchs werden Proben entnommen: Blut für DNA, Serum und Plasma und in einer Biobank gelagert, die für wissenschaftliche Projekte verwendet wird.
