Description détaillée
Plus précisément, les chercheurs étudieront l'immunité innée et adaptative, en mettant l'accent sur les sous-types de lymphocytes impliqués dans les résultats de substitution précoces et tardifs des patients atteints d'infarctus aigu du myocarde, leur caractérisation (lymphocytes B1, B2 et T) en culture cellulaire et par cytométrie en flux , et les réponses immunitaires (IgM et IgG pour oxLDL et épitopes spécifiques de l'apoB). De plus, le projet évaluera de nouveaux biomarqueurs identifiés par des études de métabolomique, ainsi que les voies de signalisation correspondantes. Les stratégies pharmacologiques thérapeutiques et les modifications du microbiote intestinal seront évaluées depuis la phase aiguë de l'infarctus du myocarde jusqu'à 6 mois.
Dans l'étude, les investigateurs compareront quatre bras de thérapie combinée : le clopidogrel avec la rosuvastatine ; ou clopidogrel avec simvastatine ; ou ticagrélor avec rosuvastatine ; ou ticagrelor avec simvastatine. L'hypothèse de l'investigateur est que l'amélioration de la microcirculation avec la rosuvastatine et le ticagrelor (effets pléiotropes synergiques) peut diminuer la surface de masse infarcie, entraînant une meilleure fraction d'éjection ventriculaire gauche par rapport aux autres thérapies combinées.
Le suivi et le génotype du microbiote seront examinés conjointement avec la métabolomique et les IRM cardiaques obtenues à la phase aiguë de l'IM et après UF 1 mois et 6 mois.

Critère d'intégration:
1. Patients stables avec infarctus du myocarde avec élévation du segment ST (STEMI) traités par thrombolytiques au cours des 6 premières heures ou des premiers symptômes de l'infarctus du myocarde.
Critère d'exclusion:
- Contre-indication ou intolérance connue au protocole du médicament à l'étude
- Ceux qui ont des comorbidités telles qu'un néoplasme, une insuffisance rénale (stade 4 ou plus)
Les patients doivent être randomisés au cours des 24 premières heures de l'IAM et traités par l'un des quatre traitements combinés au moins 2 h avant l'angiographie coronaire, suivis d'une intervention percutanée si nécessaire.
