Cím: 1. és 2. fázisú klinikai vizsgálat: Emberi önkéntesek immunizálása P. vivax irr-spz. (Rövid név: Irrad-SPZ) Populáció: 27 egészséges felnőtt önkéntes, 18-45 év közötti férfiak és nem terhes nők, akik megfelelnek a klinikai anamnézis és a szerológiai tesztek alapján meghatározott beválasztási/kizárási kritériumoknak (lásd alább). Körülbelül 20 élősködő véradó önkéntesre lesz szükség a fertőzött Anopheles szúnyogok kezeléséhez, akiket összesen 1000-1500 fertőzött szúnyoggal rendelkező önkéntesek immunizálására használnak fel 8-10 hónapon keresztül. Utána kihívás a védelem bizonyítására.
Helyszínek száma: 2 Tanulmányi időtartam: 2 év. Tárgy időtartama: 1. lépés: 1-2 óra véradóknak; 2. és 3. lépés: 2 év immunizált önkénteseknek Célok Elsődleges
• A P. vivax besugárzott sporozoita oltás biztonságosságának és védőhatékonyságának felmérése.
Másodlagos
- A P. vivax fertőzés által kiváltott immunválasz és időtartam meghatározása korábban irr-spz-vel immunizált humán Fy(+) önkéntesekben, összehasonlítva nem immunizált humán önkéntesekkel.
- Az irr-spz immunizálási séma által kiváltott immunválaszok és időtartam meghatározása humán Fy(+) önkéntesekben; összehasonlítva ugyanazzal az immunizálási sémával, nem irr-spz-vel a Fy(-) résztvevőknél, valamint azokkal a kontrollokkal, amelyek ugyanazon szúnyogcsípés sémának voltak kitéve, mint az immunizálás, de parazitafertőzés nélküli szúnyogokat alkalmaztak.
- Olyan új antigének tanulmányozása, amelyek potenciálisan hasznosak a P. vivax maláriafertőzés elleni vörösvérsejt-védelem kiváltására

Bevételi kritériumok:
• Egészséges 18-45 éves férfi vagy nem terhes nők.
- Lehetőségünk legyen a tájékozott hozzájárulást szabad és önkéntes módon aláírni.
- A hozzájárulási folyamat során megadott információkra vonatkozó kérdőív jóváhagyásával elfogadható megértéssel kell rendelkeznie a klinikai vizsgálatról.
- Megfelelő fogamzásgátlási módszer kötelező alkalmazása a toborzás és a szűrés kezdetétől az utolsó immunizálást követő három hónapig
- Ne legyen krónikus vagy akut betegsége. Ezeket a feltételeket a klinikai anamnézis, a fizikális vizsgálat és a laboratóriumi vizsgálatok határozzák meg.
- El kell fogadni, hogy a klinikai vizsgálat során ne utazzanak malária endémiás területekre
- Legyen otthon telefon vagy mobiltelefon, amely lehetővé teszi az állandó kapcsolattartást a nyomon követéshez
- Kinyilvánítja, hogy hajlandó részt venni a klinikai vizsgálat mindkét lépésében.
Kizárási kritériumok:
- 18 évnél fiatalabb vagy 45 évnél idősebb önkéntesek.
- A terhes és szoptató nők kizárásra kerülnek. A terhesség megállapítása interjúval és szérum B-alegység chorion gonadotropin teszttel történik.
- Középsúlyos vagy súlyos rovar- vagy ételallergia a kórtörténetben.
- G-6PD hiány vagy bármilyen Hb genetikai hiba (például sarlósejtes betegség)
- Korábbi maláriafertőzés, amelyet agykenet, PCR vagy specifikus maláriaellenes antitestek mutattak ki.
- Ha az alany korábban részt vett egy malária elleni vakcina kísérletben.
- A gyógyszer- vagy rovarcsípés okozta allergia klinikai feljegyzése.
- Tünetek, jelek vagy laboratóriumi vizsgálatokból származó adatok, amelyek bármilyen szisztémás rendellenességre utalnak az orvos számára, mint például vese-, máj-, szív- és érrendszeri, tüdő-, pszichiátriai rendellenességek vagy egyéb betegségek, amelyek befolyásolhatják a klinikai vizsgálat eredményeit, vagy veszélyeztethetik az önkéntes egészségét.
- A hepatitis C, VIH vagy hepatitis B felületi antigén és/vagy hepatitis B magantitestek elleni antitestek birtoklása.
- Bármilyen rendellenességet észlelni a laboratóriumi vérvizsgálatokkal értékelt paraméterekben. A kiindulási értékeket a klinikai vizsgálat megkezdése előtt határozzák meg.
- Autoimmun betegség jelenléte vagy kórtörténete, mint pl. Asztma, lupus, rheumatoid arthritis, Graves-kór, Hashimoto tiroiditis és mások.
- A lép műtéti eltávolítása (splenektómia) anamnézisében.
- Önkéntes orvosi kezelésen, amelyről ismert, hogy megváltoztatja az immunrendszert, mielőtt 3 hónappal a toborzásig, mint pl. kortikoszteroidok, kemoterápiás szerek, fludarabin, ciklosporin, takrolimusz, mikofenolát-mofetil, rapamicin, ATG, alemtuzumab.
- Alkoholizmus vagy kábítószerrel való visszaélés, amely megzavarhatja az egyén társas kapcsolatait.

Amint a szúnyogok pozitívak, 15000 cGy-vel besugározzák, és felhasználásig dobozokban tárolják. A kísérleti csoport 14 Fy(+) humán önkéntesét 1000-2000 P. vivax irrad-spz harapással immunizálják. A kontrollcsoport hét Fy(+) önkéntese lesz kitéve nem fertőzött szúnyogcsípésnek. Hat Fy(-) önkéntes lesz kitéve fertőző szúnyogcsípésnek.
