Titolo: Sperimentazione clinica di fase 1 e fase 2a: immunizzazione di volontari umani con P. vivax irr-spz. (Nome abbreviato: Irrad-SPZ) Popolazione 27 volontari adulti sani, maschi e femmine non gravide, di età compresa tra 18 e 45 anni, che soddisfano i criteri di inclusione/esclusione (descritti di seguito) come determinato dalla storia clinica e dai test sierologici. Saranno necessari circa 20 volontari donatori di sangue parassitizzati per infettare le zanzare Anopheles che saranno utilizzate per immunizzare i volontari con un totale di 1000-1500 zanzare infette, durante 8-10 mesi. Successivamente sfida per dimostrare protezione.
Numero di siti: 2 Durata dello studio: 2 anni. Durata del soggetto: Fase 1: 1-2 ore ai donatori di sangue; Fase 2 e 3: 2 anni ai volontari immunizzati Obiettivi Primario
• Valutare la sicurezza e l'efficacia protettiva della vaccinazione con sporozoite irradiato da P. vivax.
Secondario
- Per determinare le risposte immunitarie e la durata suscitata dalla sfida P. vivax in volontari umani Fy(+) precedentemente immunizzati con irr-spz, rispetto ai volontari umani non immunizzati.
- Determinare le risposte immunitarie e la durata provocate dallo schema di immunizzazione irr-spz nei volontari Fy(+) umani; confrontandolo con lo stesso schema di immunizzazione con non irr-spz nei partecipanti Fy(-), e anche con controlli esposti allo stesso schema di puntura di zanzara dell'immunizzazione ma utilizzando zanzare senza infezione parassitaria.
- Studiare nuovi antigeni potenzialmente utili per indurre protezione pre-eritrocitica contro l'infezione malarica da P. vivax

Descrizione
Criterio di inclusione:
• Uomini sani di età compresa tra 18 e 45 anni o donne non gravide.
- Avere la capacità di firmare un consenso informato in modo libero e volontario.
- Avere una comprensione accettabile della sperimentazione clinica attraverso l'approvazione di un questionario riguardante le informazioni fornite nel processo di consenso.
- Uso obbligatorio di un metodo contraccettivo adeguato dall'inizio del reclutamento e tempo di screening fino a tre mesi dopo l'ultima immunizzazione
- Non avere malattie croniche o acute. Queste condizioni saranno determinate dalla storia clinica, dall'esame fisico e dai test di laboratorio.
- Accettare di non recarsi in aree endemiche della malaria durante la sperimentazione clinica dovrebbe
- Avere un telefono a casa o un cellulare che consenta un contatto permanente per il follow-up
- Lui (lei) manifesta che è disposto a partecipare durante entrambe le fasi della sperimentazione clinica.
Criteri di esclusione:
- Volontari con meno di 18 anni o più di 45 anni.
- Saranno escluse le donne in gravidanza e in allattamento. La gravidanza sarà determinata sia dal colloquio, sia dal test della gonadotropina corionica della subunità B sierica.
- Storia di insetti moderata o grave, o allergie alimentari.
- Deficit di G-6PD o qualsiasi difetto genetico di Hb (ad esempio, anemia falciforme)
- Precedente infezione da malaria dimostrata da striscio, PCR o specifici anticorpi antimalarici.
- Se il soggetto ha precedentemente partecipato a una sperimentazione sul vaccino contro la malaria.
- Cartella clinica di allergia a farmaci o punture di insetti.
- Sintomi, segni o dati da test di laboratorio che suggeriscano al medico qualsiasi disturbo sistemico come disturbi renali, epatici, cardiovascolari, polmonari, psichiatrici o altre malattie che potrebbero interferire con i risultati della sperimentazione clinica o potrebbero compromettere la salute del volontario.
- Avere anticorpi contro l'antigene superficiale dell'epatite C, VIH o dell'epatite B e/o anticorpi centrali dell'epatite B.
- Avere qualsiasi anomalia nei parametri valutati dagli esami di laboratorio del sangue. I valori basali saranno stabiliti prima dell'inizio della sperimentazione clinica.
- Presenza o storia di una malattia autoimmune come; Asma, lupus, artrite reumatoide, morbo di Graves, tiroidite di Hashimoto e altri.
- Storia di rimozione chirurgica della milza (splenectomia).
- Volontariato con trattamento medico noto per alterare il sistema immunitario prima di 3 mesi dall'assunzione, come; corticosteroidi, agenti chemioterapici, fludarabina, ciclosporina, tacrolimus, micofenolato mofetile, rapamicina, ATG, alemtuzumab.
- Alcolismo o abuso di droghe che possono interferire con le relazioni sociali dell'individuo.

Le zanzare Anopheles albimanus saranno infettate mediante alimentazione artificiale a membrana utilizzando il sangue di donatori infetti da P. vivax. Una volta che le zanzare saranno positive verranno irradiate a 15000 cGy e mantenute in scatole fino all'utilizzo. 14 volontari umani Fy(+) nel gruppo sperimentale saranno immunizzati con 1.000-2.000 punture di P. vivax irrad-spz. Sette volontari Fy(+) nel gruppo di controllo saranno esposti a punture di zanzare non infette. Sei volontari Fy(-) saranno esposti a punture di zanzare infettive. Per il challenge sia il gruppo immunizzato che quello di controllo saranno esposti a 3 ± 1 punture di zanzare infette. I volontari saranno attentamente monitorati dopo l'infezione e saranno trattati con antimalarici.
