デクスメデトミジンは、鎮静、鎮痛、および抗不安特性を備えた高度に選択的なα2-アドレナリン受容体アゴニストです。 他の鎮静剤とは異なり、デクスメデトミジンは、内因性の睡眠促進経路を介して鎮静効果を発揮し、非急速眼球運動睡眠のような状態を生み出します。 人工呼吸器を装着している患者の場合、夜間に鎮静剤のデクスメデトミジンを注入すると、概日リズムが維持され、睡眠効率が向上し、睡眠構造が改善されます。 人工呼吸器を使用している患者の鎮静に使用すると、せん妄の発生率が低下します。
デクスメデトミジンは、睡眠の質を改善することにより、患者の転帰を改善する可能性があります。 以前のランダム化試験では、心臓以外の手術後に ICU に入院した高齢患者に対して、低用量のデクスメデトミジンを夜間に注入すると、睡眠の質が大幅に改善され、術後のせん妄の発生率が低下することがわかりました。 これらの患者の長期追跡調査では、低用量のデクスメデトミジン注入が 2 年までの生存期間を大幅に延長し、3 年生存者の認知機能と生活の質を改善したことが示されました。

包含基準:
- 年齢 65 歳以上;
- 非心臓手術後、集中治療室 (ICU) に入院。
- 翌朝までICUにいる予定。 気管内挿管を行っている患者の場合、人工呼吸器の予想持続時間は 24 時間未満です。
- 書面によるインフォームドコンセントを提供します。
除外基準:
- -統合失調症、てんかん、パーキンソニズム、または重症筋無力症の術前の病歴;
- 術前のコミュニケーション不能 (昏睡、重度の認知症、または言語障害);
- -睡眠時無呼吸の術前の病歴(以前の診断;または血清重炭酸塩≧28mmol / LでSTOP-Bangスコア≧3);
- 既知の洞不全症候群、重度の洞性徐脈 (1 分あたり 50 拍 [bpm] 未満)、またはペースメーカーを使用しない 2 度以上の房室ブロック;
- 低血圧(収縮期血圧 [SBP] <90 mmHg、平均動脈圧 [MAP] <70 mmHg、またはベースラインの SBP の低下 >30%)またはショック状態(MAP ≥65 mmHg を維持するために昇圧剤が必要であり、血清乳酸 >2 mmol/L);
- -重度の肝機能障害(Child-PughクラスC)、腎不全(腎代替療法の必要性)、または予想される生存期間が24時間未満;
- 外傷性脳損傷または神経外科;
- -手術前のせん妄の存在(混乱評価法[CAM] / CAM-ICUで評価);
- -デクスメデトミジンまたはクロニジンの治療を受けている;
- 研究参加に不適切と考えられるその他の条件。
