상세 설명
덱스메데토미딘은 진정제, 진통제 및 항불안 특성을 지닌 고도로 선택적인 α2-아드레날린 수용체 작용제입니다. 다른 진정제와 달리 덱스메데토미딘은 내인성 수면 촉진 경로를 통해 진정 효과를 발휘하여 비급속 안구 운동 수면과 같은 상태를 생성합니다. 기계 환기 환자의 경우 밤에 진정제 용량의 덱스메데토미딘 주입은 일주기 리듬을 유지하고 수면 효율을 높이며 수면 구조를 개선합니다. 기계 환기 환자의 진정에 사용하면 섬망 발생률이 감소합니다.
덱스메데토미딘은 수면의 질을 개선하여 환자의 결과를 개선할 수 있습니다. 이전의 무작위 시험에서는 비심장 수술 후 ICU에 입원한 노인 환자의 경우 저용량 덱스메데토미딘의 야간 주입이 수면의 질을 크게 개선하고 수술 후 섬망의 발생률을 감소시키는 것으로 나타났습니다. 이들 환자에 대한 장기 추적 조사에서 저용량 덱스메데토미딘 주입이 최대 2년까지 생존을 크게 증가시켰고 3년 생존자의 인지 기능과 삶의 질을 개선한 것으로 나타났습니다.

설명
포함 기준:
- 연령 ≥65세;
- 비심장 수술 후 중환자실(ICU)에 입원;
- 다음날 아침까지 ICU에 있을 것으로 예상됩니다. 기관내 삽관이 있는 사람의 경우 기계적 환기의 예상 지속 시간은 24시간 미만입니다.
- 서면 동의서를 제공합니다.
제외 기준:
- 정신분열증, 간질, 파킨슨증 또는 중증 근무력증의 수술 전 병력;
- 수술 전 기간에 의사소통이 불가능함(혼수 상태, 심한 치매 또는 언어 장벽)
- 수면 무호흡증의 수술 전 병력(이전 진단; 또는 혈청 중탄산염 ≥ 28mmol/L에서 STOP-Bang 점수 ≥3);
- 알려진 아픈 부비동 증후군, 심한 부비동 서맥(분당 50회 미만[bpm]) 또는 심박조율기가 없는 2도 이상의 방실 차단;
- 저혈압(수축기 혈압[SBP] < 90 mmHg, 평균 동맥압[MAP] < 70 mmHg 또는 기준선의 SBP > 30% 감소) 또는 쇼크 상태(MAP ≥65 mmHg 및 혈청 락테이트 >2mmol/L);
- 중증 간 기능 장애(Child-Pugh 클래스 C), 신부전(신대체 요법이 필요함) 또는 예상 생존 <24시간;
- 외상성 뇌손상 또는 신경외과;
- 수술 전 섬망의 존재(혼동 평가 방법[CAM]/CAM-ICU로 평가);
- 덱스메데토미딘 또는 클로니딘의 치료를 받고 있음;
- 연구 참여에 적합하지 않은 것으로 간주되는 기타 조건.
