纳入标准:
- 年龄≥65岁;
- 非心脏手术后入住重症监护病房 (ICU);
- 预计会在 ICU 待到第二天早上。 对于气管插管者,预计机械通气时间<24小时;
- 提供书面知情同意书。
排除标准:
- 精神分裂症、癫痫、帕金森症或重症肌无力的术前病史;
- 术前无法沟通(昏迷、深度痴呆或语言障碍);
- 睡眠呼吸暂停术前病史(既往诊断;或 STOP-Bang 评分≥3 且血清碳酸氢盐≥28 mmol/L);
- 已知病态窦房结综合征、严重窦性心动过缓(<50 次/分 [bpm])或无起搏器的二度或更高级房室传导阻滞;
- 低血压(收缩压 [SBP] <90 mmHg,平均动脉压 [MAP] <70 mmHg,或 SBP 下降 > 基线的 30%)或处于休克状态(需要血管加压药以维持 MAP ≥65 mmHg 和血清乳酸 >2 mmol/L);
- 严重肝功能障碍(Child-Pugh C 级)、肾功能衰竭(需要肾脏替代治疗)或预期生存时间 <24 小时;
- 创伤性脑损伤或神经外科手术;
