상세 설명
이 임상시험에 잠재적으로 자격이 있는 것으로 식별되어 당사에 추천된 환자는 연구에 참여하도록 초대되고 환자 정보 시트로 제공된 정보가 제공됩니다. 여기에는 DMD를 사용 중이고 최소 6개월 동안 MS가 재발하지 않았으며 장애가 진행되고 있다고 느끼거나(주관적) 관찰(객관적)된 임상적으로 확정된 MS 환자가 포함됩니다.
선별 검사를 위해 환자는 토론 후 정보에 입각한 동의서에 서명하고 포함 및 제외 기준을 충족하는지 확인하고 신경학 및 간단한 자살 가능성 평가를 받고 혈액 및 소변 안전 검사를 받게 됩니다. 환자는 CSF에서 NFL을 측정하기 위해 요추 천자를 받게 됩니다. 임계값을 초과하여 미엘린에 지속적인 손상이 있음을 나타내는 경우 시험을 계속하도록 초대합니다.
환자는 기본 뇌 및 척수 MRI 및 OCT, 임상/신경학적 검사를 받고 요추 천자를 반복하고 혈액, 소변 및 타액을 수집합니다. 환자는 맹목적으로 oxcarbazepine 대 위약으로 무작위 배정되고 각 참가자의 개별 라벨이 있는 약물 병이 제공됩니다.
기준선 방문 후 2주 및 4주 후에 조사관이 새로운 증상, 새로운 약물에 대한 세부 정보를 수집하고 참가자에게 일반적으로 조언할 때 환자는 전화로 방문하게 됩니다. 정제는 아침에 2정, 저녁에 2정으로 증량되어야 합니다.
약물 투여 시작 후 13주에 연구팀이 환자를 보게 될 것이며, OCT, 요추 천자, 일반, 시각, 신경학적 및 인지 평가 후 혈액, 소변 및 타액 수집이 있을 때 다시 25주 및 37주에 환자를 볼 것입니다. 설문지.
최종 방문은 일반, 시각, 신경학적 및 인지 평가/설문지, MRI, OCT 및 혈액, 소변 및 타액 수집을 포함한 임상 측정이 선행된 최종 요추 천자 시점인 48주차에 이루어집니다.
옥스카바제핀을 투여받은 다발성 경화증 환자가 CSF NFL 수준이 감소했는지 여부를 결정하기 위해 연구가 끝날 때 동일한 CSF 샘플에서 NFL 측정을 반복할 것입니다.

포함 기준:
- 명확한 다발성 경화증의 진단
- 기준선 방문 전 최소 6개월 동안 DMD로 치료*
- CSF NFL 수준 ≥ 0.380ng/mL
- EDSS 점수 3.5~6.0
- 기준선 방문 전 6개월 동안 재발 이력 없음
- 기준선 이전 최소 6개월의 기간 동안 객관적 또는 주관적 장애의 느린 진행 이력
18~60세
- [염증성 MS 재활성화를 방지하기 위해 최대 8주의 기간 동안 조사자의 재량에 따라 일시적 중단이 허용됩니다. 이러한 일이 발생할 수 있는 경우에는 예를 들어 임상 실습에 따라 휴약 기간이 필요한 DMD 전환이 포함됩니다. DMD에 의해 유도된 림프구 감소증 또는 기타 부작용과 같이 안전 문제가 있는 경우 중단 기간은 임상적 필요에 따라 8주를 초과할 수 있습니다. 재발과 함께 MS의 재활성화가 발생하는 경우 조사자는 이것이 철회 기준 6을 충족하는지 평가할 것입니다.]
제외 기준:
- 임신 또는 모유 수유 중이거나 적절한 피임법을 사용하지 않으려는 경우.*
- 1차 진행성 PP MS 또는 1차 재발성 PR MS 진단을 받은 참가자.
- 기준선 평가 이전 6개월 동안의 임상적 재발 또는 펄스 정맥내 또는 경구 스테로이드.
- 제대로 조절되지 않는 당뇨병 또는 동맥성 고혈압, 중증 심부전, 불안정한 허혈성 심장 질환, 비정상적인 간 기능 검사(>2.5배 ULN) 및 비정상적인 전체 혈구 수(특히 백혈구 감소증, 림프구 수에 의해 정의된 대로
- B형 간염 또는 C형 간염 또는 인간 면역결핍 바이러스에 의한 감염.
- 연구 등록 후 30일 이내에 다른 조사 약물에 대한 노출.
- 임상 면담 및 Columbia Suicide-Severity Rating Scale(CSSRS)을 기반으로 조사자의 의견에 따라 임상적으로 자살 위험이 있는 것으로 판단됨.
- 연구자가 예외를 허용하지 않는 한 악성의 이전 병력.
- 스크리닝 전 6개월 이내에 조절되지 않는 약물 또는 알코올 남용의 병력.
- OxCbz 또는 Cbz에 대한 과거의 부적절한 반응
