이것은 (i) 재발성 화학 반응성 악성(고등급) 신경 상피 및 기타 고형 종양이 있는 환자 또는 (ii) 조혈 줄기 세포 이식(HSCT)에 대한 다기관 파일럿 임상 시험입니다. 자가 조혈 전구 세포 가용성이 없는 상기 환자. 줄기 세포는 1) 일치하는 관련 기증자 또는 2) 일치하는 관련 없는 기증자(MUD)에서 파생됩니다.
이것은 고등급 및/또는 재발성 신경 상피 및 기타 고형 종양이 있는 환자를 위한 새로운 HSCT 프로토콜의 파일럿 연구입니다. 고등급 및/또는 재발성 신경 상피 및 기타 고형 종양이 있는 소아에 대한 티오테파 기반 골수 절제 화학요법(MAC) 후 동종 조혈 모세포 이식의 타당성을 결정합니다. 이 연구의 1차 종점은 조혈모세포이식 후 6개월에 무진행 생존(PFS)을 결정하는 것입니다. 2차 종료점은 다음을 포함합니다: (a) 1년에서의 전체 생존(OS) (b) +100일에서의 이식 관련 사망률(TRM) (c) 생착(d) 요법 관련 독성: 급성 및 만성 이식편의 빈도 및 중증도 -대숙주병(GVHD), 정현파 폐쇄 증후군 및 감염은 (e) HSCT 후 면역 재구성까지의 시간을 평가할 것이다. 탐구 목표는 다음을 포함합니다. 1) 고급 및/또는 재발성 신경 상피 및 기타 고형 종양이 있는 환자의 하위 집합에서 MRD 검출 기술로서 Taqman® 저밀도 어레이(TLDA) 분석의 타당성을 평가합니다. HSCT 전 및 후 골수에서 최소 잔존 질환(MRD)(해당되는 경우)은 TLDA를 사용하여 평가됩니다. 현재 고형 악성 종양의 경우 치료 실패 및/또는 질병 재발의 첫 번째 지표인 최소 잔류 질병을 탐지하기 위해 일상적으로 확립된 방법이 없습니다. 2) 이식편대숙주병과 관련된 이환율을 최소화하기 위한 노력의 일환으로, alemtuzumab은 관련이 없거나 관련이 없는 불일치 동종 이식편의 수용자를 위해 제안된 MAC 요법의 중요한 구성 요소를 형성합니다. 탐색적 목적으로 alemtuzumab 분석을 지정된 간격으로 수행하여 약물 제거 시간을 탐색합니다. 이것은 HSCT 치료에서 회복하는 동안 투여되는 면역 요법에 관한 향후 시험을 위한 림프구 고갈에 관한 중요한 정보를 제공할 수 있습니다.

포함 기준:
- 악성(고등급) 신경 상피 및 기타 고형 종양
- 환자는 최소한 다음과 같이 정의된 화학 반응성 질병 상태에 있어야 합니다. 치료 전 평가와 비교할 때 화학 요법으로의 모든 질병 퇴행
- 재발성(또는 불응성) 화학 반응성 질환이 있거나 적절한 자가 조혈 전구 세포 가용성이 없는 환자
- 크레아티닌 청소율 또는 사구체 여과율(GFR) ≥50 ml/min/1.73m2, 투석이 필요하지 않음
- 일산화탄소(DLCO)에 대한 폐의 확산 능력(헤모글로빈에 대해 보정됨) ≥ 50% 예측됨. 폐 기능 검사를 수행할 수 없는 경우 실내 공기 중 산소(O2) 포화도 ≥ 92%
- 빌리루빈 ≤3x 정상 상한치(ULN) 및 알라닌 트랜스아미나제(ALT) 및 아스파르테이트 트랜스아미나제(AST) ≤ 연령별 5x(길버트 증후군으로 인한 고립성 고빌리루빈혈증 제외)

- 조직 적합성 적합한 친족 또는 비혈연 기증자/이식편 공급원 부족
- 컨디셔닝을 포함하여 이식 절차를 견딜 수 있는 능력을 배제하는 말단 장기 부전
- 투석이 필요한 신부전
- 울혈성 심부전을 일으키는 선천성 심장병
- 환기 장애
- HIV 감염
- 통제되지 않은 세균, 바이러스 또는 진균 감염(현재 약물을 복용하고 있지만 임상 증상이 진행 중임) 치료와 함께 안정적이고 통제된 질병은 제외 기준이 아닙니다.
