Gedetailleerde beschrijving
Dit is een multi-institutioneel klinisch pilootonderzoek naar hematopoëtische stamceltransplantatie (HSCT) voor (i) patiënten met recidiverende op chemo reagerende kwaadaardige (hooggradige) neuro-epitheliale en andere solide tumoren die terugkeren na HSCT of (ii) voor zei patiënten zonder beschikbaarheid van autologe hematopoëtische voorlopercellen. De stamcellen zullen afkomstig zijn van een 1) gematchte verwante donor of 2) gematchte niet-verwante donor (MUD).
Dit is een pilootstudie van een nieuw HSCT-protocol voor patiënten met hooggradige en/of recidiverende neuro-epitheliale en andere solide tumoren. Vaststellen van de haalbaarheid van allogene HSCT na op thiotepa gebaseerde beenmergablatieve chemotherapie (MAC) voor kinderen met hooggradige en/of recidiverende neuro-epitheliale en andere solide tumoren. Het primaire eindpunt van deze studie is het bepalen van de progressievrije overleving (PFS) zes maanden na HSCT. Secundaire eindpunten zijn: (a) algehele overleving (OS) na één jaar (b) transplantatiegerelateerde mortaliteit (TRM) op dag +100 (c) implantatie (d) regime-gerelateerde toxiciteit: de frequentie en ernst van acute en chronische transplantaattransplantatie -versus-hostziekte (GVHD), sinusoïdaal obstructief syndroom en infecties zullen worden beoordeeld (e) tijd tot immuunreconstitutie na HSCT. Verkennende doelstellingen omvatten: 1) Het beoordelen van de haalbaarheid van de Taqman® Low Density Arrays (TLDA)-assay als een technologie voor MRD-detectie bij een subgroep van patiënten met hooggradige en/of recidiverende neuro-epitheliale en andere solide tumoren. Minimale residuele ziekte (MRD) (indien van toepassing) in beenmerg pre- en post-HSCT, zal worden beoordeeld met behulp van TLDA. Momenteel is er voor solide maligniteiten geen routinematig gevestigde methode om minimale resterende ziekte op te sporen, de eerste indicator van therapiefalen en/of recidief van ziekte. 2) In een poging om morbiditeit gerelateerd aan graft-versus-host-ziekte te minimaliseren, vormt alemtuzumab een belangrijk onderdeel van het voorgestelde MAC-regime voor ontvangers van niet-gerelateerde of gerelateerde niet-overeenkomende allogene transplantaten. Als verkennend doel zal op gezette tijden een alemtuzumab-assay worden uitgevoerd om de tijd tot geneesmiddelklaring te onderzoeken. Dit kan belangrijke informatie opleveren over lymfodepletie voor toekomstige onderzoeken met betrekking tot immunotherapie die wordt toegediend tijdens herstel van HSCT-therapie.
De belangrijkste voordelen van de voorgestelde aanpak zijn: 1) Overwin de uitdagingen bij het verzamelen van beenmerg/perifere bloedstamcellen (PBSC) bij zwaar voorbehandelde patiënten en/of bot-/beenmerginfiltratie met tumor. 2) Elimineer het risico van transplantaatbesmetting met tumorcellen, en 3) Graft-versus-tumoreffect (GVT) om residuele ziekte na conditionerende chemotherapie te elimineren. Het gebruik van allografting met het voorgestelde regime combineert de voordelen van chemotherapie met hoge doses en een immuunbenadering van ziektetherapie.

Beschrijving
Inclusiecriteria:
- Kwaadaardige (hooggradige) neuro-epitheliale en andere solide tumoren
- Patiënten moeten ten minste een op chemotherapie reagerende ziektestatus hebben, gedefinieerd als; elke ziekteregressie naar chemotherapie in vergelijking met de evaluatie vóór de behandeling
- Patiënten met recidiverende (of refractaire) chemo-responsieve ziekte of zonder geschikte beschikbaarheid van autologe hematopoëtische voorlopercellen
- Creatinineklaring of glomerulaire filtratiesnelheid (GFR) ≥50 ml/min/1,73 m2, en geen dialyse nodig
- Verspreidingscapaciteit van long voor koolmonoxide, of DLCO, (gecorrigeerd voor hemoglobine) ≥ 50% voorspeld. Als longfunctietesten niet kunnen worden uitgevoerd, dan zuurstofverzadiging (O2) ≥ 92% in kamerlucht
- Bilirubine ≤ 3x bovengrens van normaal (ULN) en alaninetransaminase (ALT) en aspartaattransaminase (AST) ≤ 5x voor leeftijd (met uitzondering van geïsoleerde hyperbilirubinemie als gevolg van het syndroom van Gilbert)
Uitsluitingscriteria:
- Gebrek aan histocompatibele geschikte verwante of niet-verwante donor/transplantaatbron
- Eindorgaanfalen dat het vermogen verhindert om de transplantatieprocedure, inclusief conditionering, te verdragen
- Nierfalen waarvoor dialyse nodig is
- Aangeboren hartafwijkingen die leiden tot congestief hartfalen
- Ventilatiestoring
- HIV-infectie
- Ongecontroleerde bacteriële, virale of schimmelinfecties (momenteel medicatie innemen maar klinische symptomen nemen toe); stabiele, gecontroleerde ziekte met behandeling is geen uitsluitingscriterium
- Vrouw die zwanger kan worden, zwanger wil worden tijdens het onderzoek of borstvoeding geeft
