Het doel van deze studie is om de veiligheid en effectiviteit aan te tonen van het Biosense Webster (BWI) ablatiesysteem (THERMOCOOL SMARTTOUCH surround flow [STSF]-katheter en TRUPULSE-generator) bij gebruik voor isolatie van de atriale longaders (PV's) bij de behandeling van deelnemers met paroxysmaal atriumfibrilleren (PAF), een onregelmatige hartslag die een abnormale bloedstroom veroorzaakt.

Deelname Criteria

Inclusiecriteria:
- Gediagnosticeerd met symptomatisch paroxysmaal atriumfibrilleren (PAF) met (a) ten minste twee symptomatische atriale fibrillatie (AF)-episodes binnen de afgelopen zes maanden na inschrijving en (b) ten minste één AF-episode elektrocardiografisch gedocumenteerd door middel van een elektrocardiogram (ECG); transtelefonische monitoring (TTM), holtermonitor, telemetriestrip of geïmplanteerd apparaat binnen 12 maanden voorafgaand aan inschrijving
- Mislukte ten minste één anti-aritmicum (AAD) (Klasse I of Klasse III), zoals blijkt uit terugkerende symptomatische AF, ondraaglijke bijwerkingen van de AAD of contra-indicatie voor de AAD
- Bereid en in staat om toestemming te geven
- In staat en bereid om te voldoen aan alle pre-, post- en vervolgtesten en vereisten
Uitsluitingscriteria:
- AF secundair aan verstoorde elektrolytenbalans, schildklieraandoening of omkeerbare of niet-cardiale oorzaak (bijvoorbeeld gedocumenteerde obstructieve slaapapneu, acute alcoholvergiftiging, enzovoort)
- Eerder gediagnosticeerd met aanhoudend AF (langer dan [>] 7 dagen)
- Eerdere ablatie of operatie van het linker atrium (LA).
- Deelnemers waarvan bekend is dat ze ablatie nodig hebben buiten het gebied van de longader (PV) (bijvoorbeeld atrioventriculaire herintredende tachycardie, atrioventriculaire nodale herintredende tachycardie, ventriculaire tachycardie en Wolff-Parkinson-White), behalve cavotricuspid isthmus (CTI)-lijnen voor de ablatie van typische rechter atriale flutter
- Gedocumenteerde ernstige dilatatie van de antero-posterieure diameter van de LA (linker anterieure dalende slagader [LAD] >50 mm) bij beeldvorming binnen 6 maanden vóór inschrijving
- Gedocumenteerde LA-trombus door beeldvorming binnen 48 uur na de procedure
- Gedocumenteerde ernstig aangetaste linkerventrikel-ejectiefractie (LVEF minder dan [<] 40 procent [%]) door beeldvorming binnen 6 maanden voorafgaand aan inschrijving
- Ongecontroleerd hartfalen of New York Heart Association (NYHA) klasse III of IV
- Voorgeschiedenis van bloedstolling, bloedingsafwijkingen of contra-indicatie voor antistolling (heparine), behalve bij personen met een eerdere sluiting van het linker atriumhartoor
- Gedocumenteerd trombo-embolisch voorval (waaronder TIA) in de afgelopen 12 maanden
- Deelnemers met instabiele angina, percutane coronaire interventie of acuut hartinfarct binnen 2 maanden
- Coronaire bypass-transplantatie (CABG)-operatie in de afgelopen 6 maanden (180 dagen)
- Valvulaire cardiale chirurgische/percutane procedure (dat wil zeggen ventriculotomie, atriotomie, klepreparatie of -vervanging en aanwezigheid van een prothetische klep)
- Instabiele angina pectoris binnen 6 maanden
- Verwachte harttransplantatie, hartoperatie of andere grote operatie binnen de komende 12 maanden
- Significante longziekte (bijvoorbeeld restrictieve longziekte, constrictieve of chronische obstructieve longziekte) of een andere ziekte of storing van de longen of het ademhalingssysteem die ernstige chronische symptomen veroorzaakt
- Significante aangeboren afwijking (bijvoorbeeld atriale septumdefecten [ASS]) inclusief gerepareerde defecten of medische problemen die naar de mening van de onderzoeker deelname aan dit onderzoek zouden uitsluiten
- Voorafgaande diagnose van longaderstenose
- Reeds bestaande hemi-diafragmatische verlamming
- Acute ziekte, actieve systemische infectie of sepsis
- Aanwezigheid van intracardiale trombus, myxoom, tumor, interatriale baffle of pleister of andere afwijking die introductie of manipulatie van de katheter uitsluit
- Ernstige mitralisinsufficiëntie (regurgitatievolume groter dan of gelijk aan [>=] 60 milliliter (ml)/slag, regurgitatiefractie >= 50% en/of effectief regurgitatieopeningsoppervlak >= 0,40 cm^2)
- Aanwezigheid van een geïmplanteerde pacemaker, implanteerbare cardioverter-defibrillator (ICD), recent geïmplanteerd (binnen 6 maanden) apparaat voor occlusie van het linker atriumhartoor (LAAO) of ander geïmplanteerd metalen hartapparaat in de hartruimte dat het energieveld kan verstoren dat tijdens de ablatieprocedure wordt gecreëerd
- Aanwezigheid van een aandoening die vasculaire toegang onmogelijk maakt
- Huidige deelname aan een onderzoeksstudie waarin een ander apparaat of medicijn wordt geëvalueerd
- Vrouwen die zwanger zijn (zoals blijkt uit een zwangerschapstest als ze premenopauzaal zijn), borstvoeding geven of in de vruchtbare leeftijd zijn en van plan zijn zwanger te worden in de loop van het klinische onderzoek
- Levensverwachting minder dan 12 maanden
- Het presenteren van contra-indicaties voor de apparaten die in het onderzoek worden gebruikt, zoals aangegeven in de betreffende gebruiksaanwijzing (IFU)

Experimenteel: Behandelingsgroep
Deelnemers met geneesmiddelresistente, symptomatische paroxysmale atriale fibrillatie zullen PFA/radiofrequentie (RF) ablatie ontvangen met behulp van een THERMOCOOL STSF-katheter in combinatie met de TRUPULSE-generator volgens het standaardprotocol van het ziekenhuis (naar goeddunken van de onderzoeker).

THERMOCOOL STSF zal worden gebruikt in combinatie met de TRUPULSE-generator voor ablatie met gepulseerd veld (PF) of ablatie met radiofrequentie (RF).
TRUPULSE Generator zal worden gebruikt om PF/RF-ablatie toe te passen.
