Prawie 2 miliony ludzi w Stanach Zjednoczonych miało amputację, a do 90% osób po amputacji kończyny doświadcza uporczywego uczucia brakującej kończyny, zjawiska znanego jako „kończyna fantomowa” (Weeks i in., 2010). Ponadto znaczna część osób z fantomową kończyną — do 85% w niektórych badaniach — doświadcza uporczywego i osłabiającego bólu w brakującej kończynie, stanu znanego jako ból fantomowej kończyny (dalej PLP). Chociaż istniejące terapie zapewniają ulgę w bólu w niektórych przypadkach, istnieje powszechna zgoda co do tego, że obecne podejścia nie przynoszą ulgi większości osób z PLP (Weeks i in., 2010).
Badacze zakończyli niedawno badanie potwierdzające słuszność koncepcji (Ambron i in., 2021), w którym ośmiu pacjentów z amputacją poniżej kolana (BKA) przeszło dwa zabiegi w wirtualnej rzeczywistości (VR) z powodu PLP. Podczas terapii Active VR badani grali w różne aktywne gry wymagające ruchów nóg, jednocześnie otrzymując wysokiej jakości wizualną informację zwrotną dotyczącą brakującej dolnej części nogi. Informacja zwrotna na temat pozycji nóg została przekazana za pośrednictwem systemu elektromagnetycznego wykorzystującego czujniki nóg (trackSTAR, Ascension Technologies Inc), a program wygenerował obraz brakującej dolnej części nogi, widoczny jako awatar z perspektywy pierwszej osoby. Zabieg ten kontrastował z zabiegiem „Distractor”, w którym uczestnicy byli zaangażowani w wizualnie wciągające doświadczenie wirtualnej rzeczywistości, które nie wymagało ruchów nogami (Cool!TM). Obie terapie wiązały się ze znacznym zmniejszeniem intensywności bólu, ale leczenie Active VR wiązało się również ze zmniejszeniem interferencji bólu, depresji i lęku.
Konkretne cele obecnego badania to (i) powtórzenie naszych wcześniejszych obserwacji skuteczności leczenia VR na większej próbie osób z BKA; (ii) testowanie terapii VR u pacjentów z amputacjami powyżej kolana; (iii) porównanie skuteczności terapii Active VR z terapią Distractor VR w przypadku PLP pod względem pomiaru bólu oraz zdrowia psychicznego i jakości życia.

Kryteria przyjęcia:
- Wiek 18 - 100 lat
- Zdolność do wyrażenia świadomej zgody
- Jednostronna amputacja powyżej lub poniżej kolana ponad 3 miesiące przed rejestracją
- Brak upośledzenia funkcji poznawczych, operacyjnie zdefiniowany jako wynik 24 lub wyższy w skali Montreal Cognitive Assessment
- Ból uśredniony w ciągu poprzedniego 1 miesiąca w kończynie fantomowej oceniony jako większy niż 40 w wizualnej skali analogowej Kwestionariusza bólu McGilla (Melzack, 1987)
Kryteria wyłączenia:
- Historia poważnych zaburzeń medycznych lub neurologicznych, takich jak udar lub umiarkowane lub ciężkie urazowe uszkodzenie mózgu (operacyjnie definiowane jako utrata przytomności na ponad 30 minut)
- Historia znaczących lub źle kontrolowanych zaburzeń psychicznych
- Obecna znacząca depresja lub niepokój oceniane za pomocą Hospital Anxiety and Depression Scale (HADS) (Zigmond i Snaith, 1983).
- Bieżące nadużywanie alkoholu lub narkotyków, na receptę lub w inny sposób

Eksperymentalny: Active VR treatment
Subjects assigned to the Active VR treatment began by selecting an avatar, with features such as gender and skin color that could be chosen according to preference.
During treatment, they participated in a variety of games and activities developed and used by our teams including Kick, Dog Food, Quest for Fire, Chess, Checkers, Sudoku, and surfing the internet.
Subjects had substantial flexibility to select games according to their interests but were required to spend at least 30 minutes in each session in games that required forceful, large-amplitude movements of the amputated lower limb (e.g., Quest for Fire, Kick).

8 twice-weekly interventions of virtual reality treatment focused on reducing phantom limb pain.

Eksperymentalny: Distractor VR treatment
Subjects assigned to the Distractor VR treatment participated in the REAL i-Series immersive VR experience (REAL system), which had been demonstrated to reduce pain in several studies but lacked the hypothesized "active ingredients" of our Active VR treatment (visual and auditory feedback of movement of an extrapolated amputated limb).
Subjects navigated through pleasant and relaxing VR environments; they did not see any rendering of their body and made no movements with their legs.

McGill Short Form Questionnaire (Melzack, 1987): a series of 4-point scales (0 none - 3 severe) assessing the intensity of 15 qualitative characteristics of pain (e.g., throbbing, shooting, cramping, etc.), where higher scores are worse (more severe).
For every participant we computed the total pain score (maximum 45 points indicating severe pain).
Then, we computed the difference in the total scores between time points (e.g.
post treatment vs baseline).
Therefore, negative scores indicate improvement in pain qualitative characteristics.
