Neurofeedback w leczeniu bólu neuropatycznego u osób chorych na cukrzycę

U 20-40% osób chorych na cukrzycę rozwija się polineuropatia cukrzycowa (DPN), która często objawia się bolesnym powikłaniem, silnie obniżającym jakość życia.
Obecne standardowe metody leczenia bólu neuropatycznego są często nieskuteczne i powodują znaczne skutki uboczne.
Dlatego istnieje pilna potrzeba opracowania lepszych możliwości leczenia.
Sposób, w jaki mózg interpretuje sygnały z obwodu, można modyfikować poprzez naukę pewnych technik, co może umożliwić pacjentom modyfikowanie sygnałów związanych z bolesnym DPN i w konsekwencji odczuwanie złagodzenia bólu.
Neurofeedback (NF) to obiecująca terapia neuromodulacyjna, w ramach której pacjenci otrzymują w czasie rzeczywistym informacje zwrotne na temat sygnałów neurofizjologicznych mózgu, zwiększając w ten sposób wolicjonalną kontrolę aktywności mózgu i zmniejszając odczuwanie bólu.
Neurofeedback wykorzystuje elektrody EEG skóry głowy przymocowane do ekranu komputera, które przekazują pacjentowi informację zwrotną w czasie rzeczywistym.
NF może złagodzić objawy, ucząc pacjentów samodzielnego regulowania odpowiednich wzorców aktywności.
Nagradzając osobę za każdym razem, gdy aktywność neuronowa zmienia się w pożądanym kierunku, można ją modulować.
NF nie był jeszcze badany w RCT u osób z bolesnym DPN.
Proponowany duńsko-brazylijski projekt jest pierwszym RCT z potrójną ślepą próbą rygorystycznie testującym interwencję EEG-NF w leczeniu bólu neuropatycznego (NP) w cukrzycy.
Leczenie będzie prowadzone w formie 10 sesji w dwóch randomizowanych grupach: grupa z prawdziwym EEG-NF i grupa pozorowana (placebo) EEG-NF.
Brazylijscy uczestnicy zostaną również poddani (funkcjonalnemu) rezonansowi magnetycznemu (fMRI), aby zbadać, w jaki sposób leczenie NF wpływa na mechanizmy nerwowe i je zmienia.
Jeśli okaże się skuteczne, niedrogie badanie EEG-NF będzie można udostępnić i wdrożyć na dużą skalę u osób chorych na cukrzycę i bolesną neuropatię, a także będzie dostępne dla krajów o niskich i średnich dochodach.

Wiek ≥18 i ≤82 lata
- Zdiagnozowano 1) cukrzycę typu 1 (od co najmniej 5 lat) lub 2) cukrzycę typu 2
- Kryteria konsensusu z Toronto zostaną zastosowane do definicji przypadku DPN, w którym u pacjentów musi występować co najmniej prawdopodobny DPN (31). Rozpoznanie DPN potwierdza nieprawidłowe badanie DPN. Bolesny DPN zostanie zdefiniowany przy użyciu systemu stopniowania bólu neuropatycznego (50) i będzie zgodny z definicją bólu neuropatycznego IASP, tj. „bólem spowodowanym uszkodzeniem lub chorobą układu somatosensorycznego” (kryteria konsensusu w Toronto).
- Wynik TCNS > 5
- Kwalifikujący się pacjenci z bolesnym DPN muszą odczuwać intensywność bólu wynoszącą co najmniej 4 w 11-punktowej numerycznej skali oceny (NRS, 0–10) przez co najmniej 3 miesiące przynajmniej raz dziennie i nie odczuwać silnego bólu innego niż ból spowodowany neuropatia (intensywność bólu będzie ustalana na podstawie bólu odczuwanego przez pacjentów podczas bieżącego leczenia bólu, jeśli takie istnieje).
- Stabilny lek przeciwbólowy przez > 1 miesiąc przed włączeniem. Kryteria wykluczenia
Kryteria wykluczenia:
- Współistniejące choroby neurologiczne (choroby neurodegeneracyjne, migrena, epilepsja, udar, nowotwór) lub klinicznie istotne choroby psychiczne
- Neuropatia lub ból neuropatyczny o przyczynie innej niż cukrzyca (niedobór witaminy B12, wcześniejsze leczenie chemioterapią neurotoksyczną, przewlekłe nadużywanie alkoholu, zwężenie kręgosłupa itp.)
- Zmiana dotychczasowego leczenia bólu w trakcie leczenia (doraźnie dopuszcza się paracetamol jako lek doraźny)
- Wcześniejsze lub obecne nadmierne spożycie alkoholu (odpowiednio >14 lub >21 jednostek/tydzień dla kobiet i mężczyzn) lub nadużywanie nielegalnych substancji
- Dodatni wynik testu hCG w moczu wskazuje na ciążę
- Stosowanie morfiny > 20 mg/dzień
- Ślepota lub poważnie upośledzone widzenie
- Badacz uzna, że pacjent nie nadaje się do badania (np. z powodu używania alkoholu lub narkotyków, niepełnosprawności umysłowej, niechęci lub bariery językowej uniemożliwiającej odpowiednie zrozumienie lub współpracę lub obecność jakiegokolwiek stanu, który w opinii badaczy może prowadzić do złego przestrzegania protokołu badania).

Traditional neurofeedback uses one or two electrodes to modulate activity within a specific frequency band.
Standardized Weighted Low Resolution Electromagnetic Tomography (swLORETA) analyzes the 3D distribution of intracortical brain electrical activity based on surface EEG recordings, enabling real-time brainwave imaging with a spatial resolution under one cubic centimeter.
This divides the brain into over 12,000 voxels, offering localization similar to fMRI while maintaining EEG's faster temporal resolution.
Source-localized NF can target specific, deeper brain regions, multiple Brodmann areas simultaneously, and provide feedback on connectivity between neural sources, enabling the training of specific neural networks.
swLORETA metrics are compared to a normative database of neurotypical brains to produce z-scores for each area and metric.
NeuroGuide is used within the FDA 510(k)-cleared NeuroGuide Analysis System (K041263); clearance does not imply treatment validation.

Neurophysiological outcomes will be assessed using normative z-scores within the predefined pain-related network.
A composite pain-network abnormality burden will be derived across EEG features, with domain-specific measures for current source density (CSD), instantaneous coherence, and lagged coherence also examined.
Changes from pre- to post-intervention will be compared between the real EEG-NF and sham EEG-NF groups.
