描述
纳入标准
要纳入试验,患者必须满足以下所有标准:
- 大于或等于 18 岁。 酮康唑和高剂量骨化三醇对 18 岁以下患者的影响尚未得到充分研究,儿童患前列腺癌也未见报道。
- 经组织学或细胞学证实的腺癌,临床上与雄激素无关的前列腺癌
- PSA升高的可测量疾病或可评估疾病(PSA升高将构成可评估疾病)。
- 不允许在进入研究之前对广泛性疾病进行细胞毒性化疗;鉴于有关多西紫杉醇 + ADT 在晚期疾病开始 ADT 患者中的作用的最新数据,此类“辅助化疗将被允许(不超过 6 个周期)类视黄醇、维生素 D 类似物、PPAR 激动剂或拮抗剂、抗雄激素、孕激素、雌激素、PC-SPES、LHRH 类似物、疫苗、细胞因子将不被视为“细胞毒素”。 之前接受过酮康唑 + 糖皮质激素治疗的患者将不符合该试验的资格。
- 已接受抗雄激素或促孕药物治疗前列腺癌的患者必须停止治疗并证明 PSA 升高 > 28 天后停止(抗雄激素戒断 - AAW)(比卡鲁胺或尼鲁米特 > 42 天)。 接受醋酸甲地孕酮治疗“潮热”且剂量小于每天 40 毫克的患者可在本试验期间继续使用该疗法。 在方案治疗期间不应改变醋酸甲地孕酮的剂量。 使用 LHRH 类似物进行雄激素剥夺的患者必须继续使用此类药物或进行睾丸切除术以维持去势的睾酮水平。
- 患者必须患有晚期或复发性前列腺癌。
- 患者在进入研究前至少 28 天不应接受任何化疗或研究药物。
- Eastern Clinical Oncology Group 表现状态 0 或 1
- 预期寿命>3个月。
患者必须具有如下定义的正常器官和骨髓功能:

- 高剂量骨化三醇和酮康唑对发育中的人类胎儿的影响尚不清楚。 出于这个原因,并且因为已知这些药物具有致畸作用,男性必须同意在进入研究之前和参与研究期间使用充分的避孕措施(激素或屏障避孕方法;禁欲)。 如果女性在她的伴侣参与这项研究时怀孕或怀疑自己怀孕,她应该立即通知主治医生。
- 能够理解并愿意签署书面知情同意书。
- 必须在之前 12 个月内使用阿比特龙发生疾病进展,并且患者不得接受恩杂鲁胺治疗。

排除标准
以下任何一项是排除试验的标准:
- 已知对酮康唑、骨化三醇或这些产品的任何赋形剂严重过敏。
- 归因于与骨化三醇、酮康唑或研究中使用的其他药物具有相似化学或生物成分的化合物的过敏反应史。
- 任何其他重要的临床疾病或实验室发现的证据使患者不希望参加试验。
- 过去 5 年内有肾脏、输尿管或膀胱结石病史
- 心力衰竭或重大心脏病,包括严重心律失常、最近 3 个月内的心肌梗塞、不稳定型心绞痛、记录的射血分数 <30%,或目前接受地高辛治疗。
- 进入研究后 7 天内接受噻嗪类药物治疗。
- 需要以大于生理替代剂量的同步全身性糖皮质激素治疗
- 不愿意停止补钙。
- 根据研究者的判断,任何严重或不受控制的全身性疾病(例如不稳定或失代偿性呼吸、心脏、肝脏或肾脏疾病)或并发疾病的证据,包括但不限于持续或活动性感染、有症状的充血性心力衰竭,不稳定型心绞痛、心律失常或精神疾病/社交情况会限制对研究要求的依从性。
- 接受联合抗逆转录病毒治疗的人类免疫缺陷病毒阳性患者被排除在研究之外,因为 PK 可能与研究期间施用的酮康唑或其他药物发生相互作用。 当有指征时,将对接受联合抗逆转录病毒治疗的患者进行适当的研究。
- 同时使用苯妥英、卡马西平、巴比妥类药物、利福平、苯巴比妥或圣约翰草、阿芬太尼、阿夫唑嗪、阿莫曲坦、阿普唑仑、胺碘酮、阿米替林、安普那韦、阿瑞吡坦、阿立哌唑、苄普地尔、硼替佐米、波生坦、布地奈德、丁丙诺啡、卡西平、卡西隆、西洛他唑,西沙必利,环孢菌素,地拉韦定,去羟肌苷,地高辛,丙吡胺多非利特,多奈哌齐,依来曲坦,依普利酮,氟替卡松,福沙那韦,加兰他敏,全身性灰黄霉素,茚地那韦,左旋布比卡因,洛匹那韦,咪达唑仑,米非司酮,莫达非尼,那格列奈,奈法扎维,奈芬扎维酮、喹硫平、奎尼丁、瑞格列奈、利福布汀、利福平、利福喷丁、利托那韦、沙奎那韦、西地那非、西罗莫司、他克莫司、他达拉非、托特罗定、茶碱、托特罗定、三唑仑、伐地昔布、伐地那非、齐拉西酮、唑尼沙胺、他汀类药物,普伐他汀除外) 或其他不被 CYP3A4 代谢或诱导的“他汀类药物”、钙通道阻滞剂和大环内酯类药物或其他药物,这些药物会以临床上重要的方式被酮康唑的 P450 抑制特性显着干扰
- 同时使用质子泵抑制剂或 H2 阻滞剂
- 在试验治疗第 1 天前的 28 天内使用未经批准或正在研究的药物或药剂进行治疗。
