Calcitriol oral avec kétoconazole dans le CPRC
Une étude de phase II sur le calcitriol oral en association avec le kétoconazole dans le cancer de la prostate résistant à la castration, progressant malgré l'ADT primaire et l'abiratérone

Description détaillée
Cette étude visera à décrire les réponses tumorales objectives à la combinaison de calcitriol oral et de kétoconazole et d'hydrocortisone chez les patients atteints d'une maladie mesurable en utilisant les critères RECIST 1.1 modifiés.
De plus, nous déterminerons les toxicités et la tolérabilité de l'association orale de calcitriol avec du kétoconazole oral quotidien et de l'hydrocortisone dans cette population de patients.
il s'agit d'un essai de phase II à un seul bras portant sur le kétoconazole (400 mg QD + hydrocortisone 20 mg AM, 10 mg PM) chez des hommes atteints de CPRC chez qui la maladie a progressé malgré l'abiratérone

Critère d'intégration
Pour être inclus dans l'essai, un patient doit remplir tous les critères suivants :
- Âge supérieur ou égal à 18 ans. Les effets du kétoconazole et du calcitriol à haute dose n'ont pas été suffisamment étudiés chez les patients de moins de 18 ans et le cancer de la prostate n'a pas été décrit chez les enfants.
- Adénocarcinome confirmé histologiquement ou cytologiquement compatible cliniquement avec un androgène Cancer indépendant de la prostate
- Maladie mesurable avec PSA élevé ou maladie évaluable (l'élévation du PSA constituera une maladie évaluable).
- Aucune chimiothérapie cytotoxique pour une maladie étendue avant l'entrée à l'étude ne sera autorisée ; compte tenu des données récentes concernant le rôle du docétaxel + ADT chez les patients débutant une ADT pour une maladie avancée, une telle "chimiothérapie adjuvante sera autorisée (pas plus de 6 cycles) rétinoïdes, analogues de la vitamine D, agonistes ou antagonistes des PPAR, antiandrogènes, agents progestatifs, œstrogènes , PC-SPES, analogues de la LHRH, vaccins, cytokines ne seront pas considérés comme des « cytotoxiques ». Les patients qui ont déjà reçu du kétoconazole + des glucocorticoïdes ne seront PAS éligibles pour cet essai.
- Les patientes qui ont reçu des antiandrogènes ou des agents progestatifs comme traitement du cancer de la prostate doivent arrêter le traitement et présenter une augmentation du PSA > 28 jours après l'arrêt (sevrage antiandrogène - AAW) (> 42 jours pour le bicalutamide ou le nilutamide). Les patients qui reçoivent de l'acétate de mégestrol comme traitement des « bouffées de chaleur » à une dose < 40 mg par jour peuvent poursuivre ce traitement pendant cet essai. La dose d'acétate de mégestrol ne doit pas être modifiée pendant le protocole de traitement. Les patients subissant une privation androgénique utilisant des analogues de la LHRH doivent continuer à utiliser ces agents ou subir une orchidectomie pour maintenir les niveaux de testostérone castrés.
- Les patients doivent avoir un cancer de la prostate avancé ou récurrent.
- Les patients ne doivent pas avoir reçu de chimiothérapie ou d'agents expérimentaux pendant au moins 28 jours avant d'entrer dans l'étude.
- Statut de performance du groupe d'oncologie clinique de l'Est 0 ou 1
- Espérance de vie > 3 mois.
Les patients doivent avoir une fonction normale des organes et de la moelle, telle que définie ci-dessous :
leucocytes : >3 000/μl hémoglobine : > 8 g/dl nombre absolu de neutrophiles (ANC) : >1 500/μl plaquettes : >75 000/μl bilirubine totale : dans la limite institutionnelle normale AST/ALT : <2,5 X limite supérieure institutionnelle de la créatinine normale : < 2 mg/dL de calcium : pas au-dessus de la limite institutionnelle normale
- Les patients doivent pouvoir recevoir des médicaments par voie orale.
- Les patients présentant des métastases cérébrales stables et ayant été traités par chirurgie et/ou irradiation seront éligibles pour cet essai.
- Les effets de fortes doses de calcitriol et de kétoconazole sur le fœtus humain en développement sont inconnus. Pour cette raison et parce que ces agents sont également connus pour être tératogènes, les hommes doivent accepter d'utiliser une contraception adéquate (méthode hormonale ou barrière de contrôle des naissances ; abstinence) avant l'entrée à l'étude et pendant toute la durée de la participation à l'étude. Si une femme tombe enceinte ou soupçonne qu'elle est enceinte alors que son partenaire participe à cette étude, elle doit en informer immédiatement le médecin traitant.
- Capacité à comprendre et volonté de signer un document écrit de consentement éclairé.
- La maladie évolutive doit être survenue sous abiratérone au cours des 12 mois précédents et le patient ne doit pas avoir reçu de traitement par enzalutamide.
Les hommes de tous les groupes ethniques sont éligibles pour cet essai. Des efforts seront faits pour inclure les groupes minoritaires et toutes les ethnies et races représentatives de la communauté.
Critère d'exclusion
L'un des éléments suivants est un critère d'exclusion de l'essai :
- Hypersensibilité sévère connue au kétoconazole, au calcitriol ou à l'un des excipients de ces produits.
- Antécédents de réactions allergiques attribuées à des composés de composition chimique ou biologique similaire au calcitriol, au kétoconazole ou à d'autres agents utilisés dans l'étude.
- Preuve de tout autre trouble clinique important ou résultat de laboratoire rendant indésirable la participation du patient à l'essai.
- Antécédents de calculs rénaux, urétéraux ou vésicaux au cours des 5 dernières années
- Insuffisance cardiaque ou maladie cardiaque importante, y compris arythmies importantes, infarctus du myocarde au cours des 3 derniers mois, angor instable, fraction d'éjection documentée < 30 % ou traitement actuel à la digoxine.
- Traitement thiazidique dans les 7 jours suivant l'entrée dans l'étude.
- Nécessité d'une corticothérapie systémique concomitante à des doses de remplacement supérieures aux doses physiologiques
- Refus d'arrêter la supplémentation en calcium.
- Selon l'investigateur, tout signe de maladie systémique grave ou non contrôlée (par exemple, maladie respiratoire, cardiaque, hépatique ou rénale instable ou non compensée) ou de maladie intercurrente, y compris, mais sans s'y limiter, une infection en cours ou active, une insuffisance cardiaque congestive symptomatique, angine de poitrine instable, arythmie cardiaque ou maladie psychiatrique / situations sociales qui limiteraient la conformité aux exigences de l'étude.
- Les patients positifs pour le virus de l'immunodéficience humaine recevant une thérapie antirétrovirale combinée sont exclus de l'étude en raison d'interactions pharmacocinétiques possibles avec le kétoconazole ou d'autres agents administrés au cours de l'étude. Des études appropriées seront entreprises chez les patients recevant une thérapie antirétrovirale combinée, le cas échéant.
- Utilisation concomitante de phénytoïne, carbamazépine, barbituriques, rifampicine, phénobarbital ou millepertuis, alfentanil, alfuzosine, almotriptan, alprazolam, amiodarone, amitriptyline, amprénavir, aprépitant, aripiprazole, bépridil, bortézomib, bosentan, budésonide, buprénorphine, carbamazépine, cilostazol, cisapride, cyclosporine, delavirdine, didanosine, digoxine, disopyramidedofétilide, donépézil, élétriptan, éplérénone, fluticasone, fosamprénavir, galantamine, griséofulvine systémique, indinavir, lévobupivacaïne, lopinavir, midazolam, mifépristone, modafinil, natéglinide, néfazadone, nelfazépine, mooxcarbazépine, mooxcarbazépine , quétiapine, quinidine, répaglinide, rifabutine, rifampicine, rifapentine, ritonavir, saquinavir, sildénafil, sirolimus, tacrolimus, tadalafil, toltérodine, théophyllines, toltérodine, triazolam, valdécoxib, vardénafil, ziprasidone, zonisamide, statines, à l'exception de la pravastatine (Pravachol ) ou d'autres "statines" qui ne sont pas métabolisées par ou n'induisent pas le CYP3A4, les inhibiteurs calciques et les macrolides ou d'autres agents qui seront significativement perturbés d'une manière cliniquement importante par les propriétés inhibitrices de P450 du kétoconazole
- Utilisation concomitante d'inhibiteurs de la pompe à protons ou d'anti-H2
- Traitement avec un médicament ou un agent non approuvé ou expérimental dans les 28 jours précédant le jour 1 du traitement d'essai.
- Toute toxicité chronique non résolue supérieure au grade CTC 2 d'un traitement anticancéreux antérieur.
- Guérison incomplète suite à des traitements oncologiques antérieurs ou à une autre intervention chirurgicale majeure.
- Incapacité à avaler des capsules orales.
- Les patients sous digoxine seront exclus de cette étude.
Produits Posologie et mode d'administration
- Kétoconazole, comprimés de 200 mg, 2 comprimés par voie orale TID
- Calcitriol (caplets de 0,5 mcg) administré à des doses croissantes, par voie orale QD X3 jours consécutifs chaque semaine
- Hydrocortisone 20 mg le matin, 10 mg le soir par voie orale en commençant le soir avant la première dose de Calcitriol.
