颈椎退行性脊髓病的再生——一项多中心、双盲、随机、安慰剂对照试验,评估异丁司特作为退行性脊髓型颈椎病减压手术辅助治疗的疗效
退化性(脊柱磨损性关节炎)颈椎病(关于颈部)脊髓病(脊髓损伤)DCM 是成年期最常见的脊髓疾病。 在 DCM 中,脊柱关节炎会导致脊髓受压。
DCM 的症状常常被误认为是衰老的自然后果,包括手麻木和笨拙、失去协调、不平衡、膀胱和肠道问题。 这种虚弱会发展为严重的瘫痪。 每年 100,000 人中大约有 4 人接受 DCM 手术;然而,据认为还有更多人患有 DCM。
DCM 的主要治疗方法是手术。 手术的目的是创造空间并消除脊髓的压迫。 众所周知,这是为了防止进一步受伤。 不幸的是,术后改善往往是不完整的,许多患者仍然严重残疾。 改善手术后的结果代表了一个重要的未满足的临床需求。
临床和临床前研究结果表明,异丁司特药物可以刺激脊髓的神经保护和再生过程。 Ibudilast 耐受性良好,在日本用于治疗哮喘和中风后头晕,目前正在研究用于治疗其他神经系统疾病。
本研究将调查是否每天口服异丁司特最多 34 周可以改善手部功能、力量、平衡、泌尿问题并减轻疼痛。
该研究最初将在英国的三个地点进行,必要时会增加更多地点。 年龄在 18-80 岁之间、被诊断患有 DCM 并计划首次接受手术的个人将被邀请参加试验。 该研究将在手术前、手术后不久以及手术后 3、6 和 12 个月进行患者问卷调查和临床评估。 此外,患者将在术前和术后 6 个月进行 MRI 扫描,以确定治疗是否成功。

DCM 是一种常见的致残性疾病。 这也是老年人步态障碍和不平衡的主要原因。 因此,DCM 会导致行动不便和虚弱。 NHS England 认识到 1) 降低过早死亡率和 2) 提高患有长期疾病的人的生活质量很重要。 DCM 患者有反复跌倒的风险,这是一个主要问题。 事实上,一项调查髋部骨折患者的研究表明,25% 的患者患有未确诊的扩张型心肌病。 手术减压是目前唯一的治疗方式。 它被证明可以阻止疾病进展。 然而,手术后的神经功能恢复往往令人失望。 没有批准的 DCM 药物治疗。 减轻由 DCM 引起的长期残疾代表了未满足的临床需求。 腿部和手臂功能的恢复以及疼痛的改善是患者康复的首要任务。
在 DCM 中,脊髓上的机械压力会导致神经细胞及其突起的逐渐丢失以及髓鞘的丢失,髓鞘是由少突胶质细胞形成的神经元绝缘层。 手术减压可以阻止疾病进展,但自然脊髓修复有限。 临床前研究表明,抑制 PDE4 能够刺激再生反应,这可能有益于 DCM:髓鞘再生和轴突可塑性。 使用异丁司特(一种磷酸二酯酶 4 抑制剂)的临床研究已证明对多发性硬化症有有益作用。 观察到的有益作用可能反映了异丁司特在人类中枢神经系统中的再生诱导和神经保护作用。

RECEDE-Myelopathy 是一项多中心、双盲、III 期随机、安慰剂对照试验,评估使用异丁司特作为 DCM 减压手术的辅助治疗,涉及多达 10 个英国站点。 总共将招募362名参与者。
每个参与者将接受大约 15 个月(±21 天)的试用。 从筛选到随机化最多间隔 1 周,从随机化到治疗开始最多间隔 1 周。 Ibudilast 治疗将在手术前 10 周内开始,并将持续至手术后 24 周。 手术前 5 天将停止治疗,并在手术后两天后尽快恢复之前的最大剂量。 参与者将服用 Ibudilast 最多 34 周;如果手术延迟超过10周,术后治疗将相应缩短,以保持34周的最长治疗期。 参与者将在手术后接受最多 12 个月的随访。

纳入标准:
- 根据既定标准患有退行性脊髓型颈椎病并已同意参与试验的患者
- 术前 mJOA 评分≥8 且≤14
- 作为常规 NHS 临床实践的一部分,计划进行首次手术减压
排除标准:
- 既往 DCM 手术
- 颈椎外伤引起的 DCM 症状(由研究者自行决定)
- 对异丁司特或任何制剂成分过敏
- 通过临床和实验室评估,包括 ALP > 1.5x ULN 的急性肝炎证据、临床显着慢性肝炎或临床显着肝功能受损的证据; ALT 或 AST > 2x ULN; GGT > 3x 正常值上限
- 活动性恶性肿瘤定义为侵袭性恶性肿瘤病史,除非患者已接受治疗且至少五年未出现临床体征和症状
- 最近(少于 3 年)的化学物质依赖史或可能影响结果或研究参与的显着心理社会障碍
