第一阶段试验将招募三名受试者进入接受低剂量 CFL001 的初始组。 如果这些受试者对该剂量的耐受性良好,将继续将三名受试者纳入接受中等剂量 CFL001 的组。 如果这些受试者能够很好地耐受该剂量,则将继续将三名受试者纳入接受最高剂量 CFL001 的组。

纳入标准:
- 年龄≥18岁且≤90岁。
- SIJ 综合征的诊断基于临床表现,包括 Fortin、FABER 和压迫征。
- SIJ 综合征的严重程度,基线 ODI 评分≥ 30%,SI 关节疼痛评分在 VAS 100 mm 量表上≥ 50 且≤ 90。
- 患有单侧或双侧 SIJ 关节炎的个体都可以成为注册候选人;如果根据所有其他纳入标准,两个关节都被认为适合测试药物的给药,那么将治疗参与者报告为更疼痛的 SIJ,或者如果两个关节同样疼痛,那么我们将使用随机生成器方法(“翻转硬币”)来确定将处理哪个关节。
筛选前 3 个月内,在图像引导下仅向 SIJ 注射局部麻醉剂(不含类固醇)后 2 天内疼痛减轻 ≥75%。
或 根据筛选前 3 个月在 SIJ 中通过图像引导注射局部麻醉剂和类固醇后疼痛减轻来确定 SIJ 状况。
- 体重指数 < 40 kg/m2。
- 能够遵守学习要求。
- 能够理解并提供书面知情同意书。
- 所有育龄/生育能力的参与者均须确认在研究期间采取了适当的避孕措施。
- 所有参与者都应该尝试过保守疗法,例如药物(对乙酰氨基酚和/或非甾体抗炎药或曲马多),但失败了;每日家庭锻炼或家庭伸展运动,包括臀带和核心锻炼,目标为20分钟;如果后勤可行的话,每周一次在机构进行物理治疗指导,持续六周。 上述保守治疗方法失败被定义为尽管尝试上述方法但三个月后仍持续疼痛。
排除标准:
- 之前接受过 SIJ 放射治疗。
- 在测试产品给药前 15 天以内使用任何止痛药物或疗法,且在测试药物给药前至少 3 个月内没有或不会有稳定的剂量、频率或强度。 对于与 SIJ 综合征无关的病症,使用对乙酰氨基酚以外的预定止痛药物,且在测试药物给药前至少 3 个月没有稳定的剂量。 在每次随访评估之前至少 24 小时内不愿意考虑避免使用止痛药。
- 筛选前 3 个月内使用皮质类固醇进行关节内治疗或全身使用类固醇。
- 在筛查前的任何时间点使用再生药物(例如血浆、干细胞、胎盘产品)进行关节内治疗。
- 过去 6 个月内参加了另一项临床试验。
- 生命体征绝对值超出以下范围:收缩压 >170 或 <100,脉搏率 >100 或 <50 bpm,呼吸频率 >22。 如果受试者在到达诊所之前经历了压力环境,研究者可以酌情提供合理的延迟(即一小时),以评估是否恢复到可接受的参数。
- 筛选前 6 个月内接受过透明质酸关节内治疗。
- 筛选前对 SIJ 指数 < 12 个月进行手术干预,或进行关节镜检查 < 3 个月。
- 非卧床状态。
- 过去或当前诊断为纤维肌痛或炎性关节炎、痛风、类风湿性关节炎、狼疮关节病、银屑病关节炎、缺血性坏死、严重骨畸形、SIJ或注射部位的活动性感染、鹅足滑囊炎、神经源性或血管性跛行,或未受控制的糖尿病(HbA1C > 8%)。
- 过去或当前诊断出的并发疾病,包括不受控制的心律失常、3 级或 4 级充血性心力衰竭、活动性乙型或丙型肝炎、肝酶 ≥ 2 倍 ULN(如果还存在胆红素升高)、高凝状态、eGFR <45 mL/min CKD-EPI,以及未经治疗的恶性肿瘤或 6 个月内诊断出的恶性肿瘤。
- 控制不良的情况预计在试验过程中可能需要类固醇,这可能会混淆结果。
- 过去或当前诊断出癌症,或有很高的复发风险。

- 酗酒或滥用药物或任何其他导致受试者不太可能遵守研究程序的原因。
