第 1 相試験では、低用量の CFL001 を投与される初期グループに 3 人の被験者が登録されます。 これらの被験者がこの用量に十分耐えることを条件として、中用量の CFL001 を投与されるグループに 3 人の被験者を登録することに進みます。 これらの被験者がこの用量に十分耐えられるという条件で、最高用量の CFL001 を投与されるグループに 3 人の被験者を登録することに進みます。

包含基準:
- 年齢 18 歳以上 90 歳以下。
- フォーチン、FABER、圧迫徴候などの臨床所見に基づくSIJ症候群の診断。
- ベースラインODIスコアが30%以上、VAS 100mmスケールでSIJ関節痛スコアが50以上90以下のSIJ症候群の重症度。
- 片側または両側の仙腸関節炎を患っている人は登録の候補者となることができます。両方の関節が他のすべての包含基準によって試験薬剤の投与に適切であるとみなされる場合、参加者がより痛みがあると報告したSIJが治療されます。または、両方の関節が同等の痛みである場合、ランダムジェネレータアプローチを使用します(「フリップa」)。コイン") を使用して、どの関節を処理するかを決定します。
スクリーニング前の3か月以内に局所麻酔薬のみ(ステロイドは含まない)を画像ガイドに基づいてSIJに注射した後、2日以内に痛みが75%以上減少する。
または、スクリーニングの3か月前に画像に基づいて局所麻酔薬とステロイドをSIJに注射した後の痛みの減少に基づいてSIJの状態が確立されている。
- BMI が 40 kg/m2 未満。
- 研究の要件に準拠する能力。
- 書面によるインフォームドコンセントを理解し、提供する能力。
- 研究期間中に適切な避妊の使用を確認できる生殖年齢/能力のあるすべての参加者。
- すべての参加者は、薬物療法(アセトアミノフェンおよび/または NSAID またはトラマドール)などの保存的療法を試して失敗したはずです。ヒップガードルとコアエクササイズを含む、20分を目標とした毎日の自宅エクササイズまたは自宅ストレッチ。そして、ロジスティック的に現実的であれば、週に1回、6週間にわたって施設でのガイド付き理学療法。 上記の保存的治療アプローチの失敗は、上記の試みにもかかわらず 3 か月後に持続する痛みとして定義されます。
除外基準:
- SIJへの事前の放射線照射。
- -試験薬投与前少なくとも3ヶ月間安定した用量、頻度、または強度を持たなかった、または今後安定しない鎮痛薬または治療法を試験薬投与前15日以内に使用した場合。 SIJ症候群とは関係のない症状に対する、アセトアミノフェン以外の予定された鎮痛薬の使用で、試験薬投与前の少なくとも3か月間安定した用量がなかった場合。 各フォローアップ評価の少なくとも24時間前までは鎮痛剤の使用を避けることを検討したがらない。
- -スクリーニング前の3か月以内にコルチコステロイドによる関節内治療またはステロイドの全身使用。
- スクリーニング前の任意の時点での再生医療(血漿、幹細胞、胎盤製品など)による関節内治療。
- 過去 6 か月以内に別の臨床試験に参加した。
- 以下の範囲外の絶対値バイタルサイン: 収縮期血圧 > 170 または < 100、脈拍数 > 100 または < 50 bpm、および呼吸数 > 22。 被験者が診療所に到着する前にストレスの多い状況にさらされていた場合、許容可能なパラメータへの復帰を評価するために、研究者の裁量により、合理的な遅延(すなわち、1時間)を設けることができる。
- -スクリーニング前の6か月以内のヒアルロン酸による関節内治療。
- スクリーニング前に指標SIJに対する外科的介入が12か月未満、または関節鏡検査が3か月未満である。
- 歩行不可能な状態。
- -線維筋痛症または炎症性関節炎、痛風、関節リウマチ、ループス関節症、乾癬性関節炎、無血管性壊死、重度の骨変形、SIJまたは注射部位の活動性感染、アンセリン滑液包炎、神経因性または血管性跛行の過去または現在の診断、またはコントロールされていない糖尿病(HbA1C >8%)。
- -制御不能な不整脈、クラス3または4のうっ血性心不全、活動性B型またはC型肝炎、肝酵素がULNの2倍以上(ビリルビンの上昇もある場合)、凝固亢進状態、eGFR <45 mL/分などの併発疾患の過去または現在の診断。 CKD-EPI、および未治療の悪性腫瘍、または6か月以内に悪性腫瘍と診断された。
- 状態の管理が不十分な場合は、試験中にステロイドが必要になる可能性があり、結果を混乱させる可能性があります。
- 過去または現在がんと診断されている、または再発のリスクが高い。
- スクリーニング後2年以内の指標SIJにおける外傷による二次性関節炎の診断。
- 診断目的または症状軽減のためにドレナージを必要とする、スクリーニング時の指標 SIJ における SIJ 滲出液。
- 臨床的に重大な進行中の疾患または医学的状態。治験責任医師の意見では、治験への参加に対する安全上のリスクとなるか、または治験の目的、実施、または評価の達成を妨げる可能性があります。
- 妊娠中または授乳中の女性。
- 抗凝固薬の定期的な使用(アスピリン<325 mgの毎日の使用は許容されます)。
- 活発なアルコールまたは薬物乱用、または被験者が研究手順に従う可能性が低いその他の理由。
- 臨床現場での標準スクリーニングを使用した、禁止物質または対象者が有効な処方箋を持たない物質の尿中薬物スクリーニングで陽性結果が得られた場合。
- 調査の判断により、研究の実施または研究結果の解釈に支障をきたすと判断される精神疾患または精神疾患を患っている被験者。 過去 6 か月間安定した用量の抗うつ薬および不安薬を服用しており、研究中に用量の変更が予想されない、安定した不安症およびうつ病を患っている被験者を含めることができます。
- -治験責任医師の判断で、被験者のリスクベネフィットに悪影響を与える可能性が高い、または治験の遵守または安全性または有効性の評価を妨げる可能性がある、臨床的に重大な医学的、外科的、精神医学的、または検査室の異常。
- -局所麻酔薬またはDMSOを含む治験薬の成分に対する既知のアレルギー。
- X線造影剤に対する既知のアレルギー。
- 自己免疫疾患を患っている被験者、または後天性免疫不全症候群(AIDS)またはヒト免疫不全ウイルス(HIV)の既知の病歴がある被験者。
- 他の原因(腰椎椎間板変性、脊柱管狭窄症など)による重度の腰痛は、臨床研究者の意見では、SIJの痛みの評価が困難または曖昧になると考えられます。
- 最近(1年未満)の骨盤に対する大きな外傷の病歴。
- 代謝性骨疾患(誘発性または特発性)。
- 訴訟への関与。
- 腰痛または仙腸関節痛に対する障害給付金または労災補償の受給。
