OVERSIGT: Dette er et randomiseret, dobbeltblindt, placebokontrolleret, crossover-studie. Patienterne er stratificeret efter tidligere neurotoksisk middel (paclitaxel vs oxaliplatin vs. andre taxanmidler uden paclitaxel vs platinmidler [cisplatin] uden oxaliplatin) og høj risiko for at udvikle smertefuld kemoterapi-induceret perifer neuropati (nej vs ja). Patienterne randomiseres til 1 ud af 2 behandlingsarme.

SYGDOMS KARAKTERISTIKA:

  • Diagnose af kræft

    • CNS malignitet tilladt med undtagelse af leptomeningeal carcinomatose

  • Skal have smertefuld sensorisk kemoterapi-induceret perifer neuropati (CIPN) som følge af tidligere behandling med enkeltstof taxan eller platinmidler (paclitaxel, docetaxel, nab-paclitaxel, oxaliplatin, cisplatin) (måske ikke have modtaget lægemidler fra begge klasser)

    • CIPN > grad 1 målt ved NCI-CTCAE v 4.0
    • Gennemsnitlig neuropatisk smertescore ≥ 4

  • Patienter med følgende sygdomme, der vides at forårsage perifer neuropati, er kvalificerede, forudsat at de ikke har tegn på neuropati fra disse sygdomme:

    • Diabetes mellitus
    • Perifer vaskulær sygdom
    • HIV-infektion
    • Betydelig degenerativ eller familiær neurologisk lidelse, der vides at forårsage perifer neuropati
  • Ingen klinisk eller subklinisk neuropati fra nervekompressionsskader (dvs. karpaltunnelsyndrom, brachial plexopati, spinal stenose eller spinal nerverodskompression)

FORUDSÆTNING SAMTYDLIG TERAPI:

  • Mindst 3 måneder siden tidligere og ingen samtidig taxan- eller platinmiddel
  • Mindst 14 dage siden tidligere og ingen samtidige monoaminoxidasehæmmere eller andre antidepressiva
  • Ingen andre tidligere eller samtidige neurotoksiske lægemidler (f.eks. vincristin, vinblastin, cytarabin, thalidomid, bortezomib, carboplatin eller procarbazin)
  • Ingen samtidige antikonvulsiva

  • Ingen samtidig B- eller E-vitamintilskud i doser større end den anbefalede daglige tilførsel (RDA)

    • Centrum (standard formel) og One-A-Day "essentiel" formel, som indeholder 100 % RDA for vitamin B6, E og B12 tilladt
