Eine multizentrische Studie zur Untersuchung des akuten Leberversagens bei Erwachsenen (ALFSG)

Detaillierte Beschreibung
Obwohl ALF wirklich eine seltene Krankheit ist, von der nur etwa 2.000 Personen pro Jahr betroffen sind, rechtfertigen ihre Schwere, ihre Häufigkeit bei jungen Erwachsenen und ihre hohe Ressourcennutzung die Aufmerksamkeit, die ihr geschenkt wird. Darüber hinaus hat ALF aufgrund seiner einzigartigen Pathogenese und seiner extremen Schwere das Interesse und die Aufmerksamkeit der Forscher auf sich gezogen und uns ermutigt, die Prozesse zu verstehen, die allen Formen von Leberschäden zugrunde liegen, indem wir uns auf diese tödlichste Manifestation konzentrieren.
Die Ätiologien im Zusammenhang mit ALF haben sich im Laufe der Jahre weiter verändert, mit einem offensichtlichen Rückgang der Virushepatitis und einem bemerkenswerten Anstieg der Paracetamol-Toxizität auf das derzeitige Niveau von etwa 44-50 % der Fälle. Ein weiteres Problem bei der Untersuchung von ALF ist, dass die Zahl der Fälle einer bestimmten Ätiologie, die an einer Institution beobachtet werden, verschwindend gering ist. Die ersten Ziele der ALF-Studie bestanden damals darin, die Ätiologien von ALF auf nationaler Ebene sorgfältiger zu definieren und schließlich eine eingehende Untersuchung spezifischer ALF-Ursachen wie Autoimmun-ALF, Virushepatitis und Morbus Wilson (WD) zu ermöglichen.
Eine zweite Gruppe von Patienten, die es wert sind, untersucht zu werden, sind Patienten mit akuter Leberschädigung. Es wäre aus mehreren Gründen von Wert, Patienten zu untersuchen, bei denen ALF möglicherweise früher in ihrer Krankheit auftritt: Erstens können wir besser vorhersagen, wer vollständig fortschreiten wird Leberversagen; Zweitens begrenzt die aktuelle Definition, die eine Enzephalopathie erfordert, die Anzahl der Patienten, die an jedem Standort für Studien zur Verfügung stehen. Schließlich könnten therapeutische Studien eine größere Wirksamkeit haben, wenn sie in früheren Krankheitsstadien begonnen werden.
Eingeschriebene Patienten werden von Gastroenterologen/Hepatologen und Kollegen an die klinischen Standorte von ALFSG überwiesen. Detaillierte klinische Daten und Bioproben (Seren, Urin, Plasma, DNA und Gewebe, sofern verfügbar) werden gesammelt. Die Probanden werden langfristig nach 6 Monaten und 12 Monaten beobachtet. ALF-Einschlusskriterien:
- Schriftliche Einverständniserklärung der nächsten Angehörigen des Patienten
- Veränderte Mentalität jeglichen Grades (Enzephalopathie)
- Nachweis einer mittelschweren Koagulopathie (INR ≥ 1,5)
- Ein vermuteter akuter Krankheitsbeginn von weniger als 26 Wochen
- Die NIH-Richtlinien zur Einbeziehung von Frauen und Minderheiten als Versuchspersonen in die klinische Forschung werden eingehalten
ALF-Ausschlusskriterien:
- Patienten mit Zirrhose
- Alkoholinduziertes Leberversagen
- Bekannte vorbestehende chronische Lebererkrankung
ALI-Einschlusskriterien:
Paracetamol (APAP) Ätiologie: akute Erkrankung < 2 Wochen
- INR ≥ 2,0, ALT ≥ 10X ULN Nicht-Acetaminophen Ätiologie: akute Erkrankung < 26 Wo
- INR ≥ 2,0, ALT ≥ 10X ULN, TBili ≥ 3 mg/dl
