AMBINEB : essai clinique pour évaluer la tolérance et l'innocuité de l'ambisome d'amphotéricine B liposomale nébulisée pour la prophylaxie de l'aspergillose pulmonaire invasive chez les patients atteints de leucémie myéloïde aiguë et de greffe de cellules progénitrices hématopoïétiques allogéniques (Alo-HPCT)

L'infection fongique invasive (IFI) est la cause la plus fréquente de mortalité liée à la greffe autologue de cellules souches. Tenant compte du fait que l'Aspergillus saprophyte est généralement acquis par inhalation, protéger l'arbre bronchique juste avant l'invasion des tissus est tout à fait attrayant. Chez les patients hématologiques, ainsi que ceux soumis à une greffe de cellules progénitrices hématopoïétiques allogéniques, il existe un autre groupe de patients à haut risque d'aspergillose pulmonaire invasive (IPA). Il s'agit des patients atteints de leucémie aiguë myéloïde (LMA), soumis à une polychimiothérapie d'induction, d'intensification ou de consolidation. Le taux d'incidence de l'IPA chez ces patients, à tout moment de leur évolution, atteint 18 à 20 %, avec les traitements habituels. De plus, contrairement aux allogreffes de cellules souches hématopoïétiques, la neutropénie était le seul facteur de risque d'IPA. De nos jours, la prophylaxie pharmacologique contre l'IPA, chez les patients allogreffes de cellules progénitrices hématopoïétiques et les patients atteints de LAM en traitement d'induction ou d'intensification est loin d'être optimale, en raison de problèmes liés à la tolérance et aux interactions médicamenteuses.
La prophylaxie de l'amphotéricine B liposomale nébulisée (Ambisome) contre l'IPA a montré une bonne tolérance, sécurité et efficacité chez les receveurs de greffe pulmonaire.
En extrapolant les résultats obtenus chez les greffés pulmonaires, nous arrivons à la conclusion qu'il serait essentiel d'étudier l'innocuité et la tolérance d'AMBISOME nébulisé dans le groupe de patients présentant différentes particularités, mucosite secondaire à la chimiothérapie et incidence élevée d'IPA afin d'atteindre le objectif d'évaluer son efficacité comme prophylaxie contre l'IPA chez ce type de patients

Critère d'intégration:
- Le patient a décidé volontairement de consentir à sa participation en signant le formulaire de consentement avant l'exécution de toute procédure liée à l'étude ne faisant pas partie des soins médicaux normaux, étant entendu que le consentement peut être retiré par le patient à tout moment sans préjudice de ses soins médicaux futurs .
- Les patients atteints de leucémie myéloïde aiguë (LAM) qui commenceront une chimiothérapie d'induction ou les patients soumis à une greffe de cellules progénitrices hématopoïétiques allogéniques.
- Le patient a > 18 ans.
Critère d'exclusion:
- Patient ayant des antécédents d'aspergillose pulmonaire invasive (IPA).
- Antécédents d'allergie ou d'hypersensibilité à l'amphotéricine B.
- Patient présentant un déficit intellectuel ou des patients présentant des altérations psychologiques qui rendent impossible la compréhension de l'essai.
- Grossesse ou allaitement.
- Le patient a reçu un autre médicament expérimental ou un produit non commercialisé dans les 30 jours précédant le début de l'essai.
- Le patient est inscrit à une autre étude de recherche clinique ou/et reçoit un agent expérimental pour une raison quelconque.
- Le patient a subi une intervention chirurgicale majeure dans les 4 semaines précédant l'inscription.

EXPÉRIMENTAL: 1
Les patients atteints de LAM en traitement de chimiothérapie d'induction recevront une prophylaxie avec de l'amphotéricine B liposomale nébulisée (24 mg/semaine). Elle sera maintenue pendant la chimiothérapie d'intensification et dans les périodes inter-cycles. Si le patient a besoin d'ALO-TPH, la prophylaxie doit être suivie. |
