Évaluation longitudinale de sujets atteints de démence frontotemporale familiale (LEFFTDS)

Cette étude multicentrique recrutera 300 membres de familles de démence frontotemporale familiale (FTD) dans 8 centres de recherche expérimentés sur la FTD avec une mutation connue dans MAPT, PGRN ou C9ORF72 (100 porteurs de mutation atteints de démence légère ou peu symptomatiques mais non déments, 100 mutations asymptomatiques porteurs et 100 parents cliniquement normaux qui ne sont pas porteurs de mutation) pour obtenir des évaluations annuelles, y compris T1-MRI, FLAIR, imagerie du tenseur de diffusion (DTI), perfusion ASL (ASLp), IRM fonctionnelle de connectivité intrinsèque (icfMRI), spectroscopie RM (MRS ), LCR, sang et évaluation comportementale, neuropsychologique et fonctionnelle, pour un total de trois évaluations par participant.
L'un des principaux objectifs de cette étude est d'identifier les méthodes les plus robustes et les plus fiables pour suivre la progression de la maladie dans la DFT familiale afin que les essais thérapeutiques modificateurs de la maladie puissent être conçus de manière appropriée.

Critère d'intégration:
- Doit être un membre de la famille avec une mutation connue dans l'un des trois principaux gènes liés au FTLD : MAPT, PGRN ou C9ORF72.
- Au moins 18 ans.
- Le phénotype prédominant dans la parenté doit être cognitif/comportemental (c.-à-d., les parents chez qui le parkinsonisme ou la SLA est le phénotype clinique prédominant parmi les parents affectés peuvent être exclus)
- Avoir un informateur fiable qui parle personnellement avec ou voit ce sujet au moins une fois par semaine.
- Le sujet parle suffisamment couramment l'anglais pour effectuer toutes les mesures
- Le sujet doit être disposé et capable de consentir au protocole et de subir des évaluations annuelles sur 3 ans.
- Le sujet doit être disposé et capable de subir des tests neuropsychologiques (au moins lors de la visite de référence).
- Le sujet ne doit avoir aucune contre-indication à l'imagerie IRM.
Critère d'exclusion
- Présence connue d'une lésion cérébrale structurelle (par ex. tumeur, infarctus cortical).
- Présence d'un autre trouble neurologique qui pourrait avoir un impact sur les résultats (par exemple, la sclérose en plaques).
- Le sujet ne veut pas revenir pour un suivi annuel, subir des tests neuropsychologiques et une imagerie par résonance magnétique.
- Le sujet n'a pas d'informateur fiable.
