Longitudinel evaluering af familiære frontotemporale demenspersoner (LEFFTDS)

Denne multicenterundersøgelse vil indskrive 300 medlemmer af familiær Frontotemporal Demens (FTD) familier på tværs af 8 erfarne FTD forskningscentre med en kendt mutation i MAPT, PGRN eller C9ORF72 (100 mutationsbærere med mild demens eller minimalt symptomatisk, men ikke-dement, 100 asymptomatisk mutation bærere og 100 klinisk normale slægtninge, som er ikke-mutationsbærere) for at opnå årlige vurderinger, herunder T1-MRI, FLAIR, diffusionstensor-billeddannelse (DTI), ASL-perfusion (ASLP), funktionel MRI med intrinsic connectivity (icfMRI), MR-spektroskopi (MRS). ), CSF, blod og adfærdsmæssig, neuropsykologisk og funktionel vurdering, for i alt tre vurderinger pr. deltager.
Et primært mål med denne undersøgelse er at identificere de mest robuste og pålidelige metoder til at spore sygdomsprogression i familiær FTD, så sygdomsmodificerende terapeutiske forsøg kan designes passende.

Beskrivelse
Inklusionskriterier:
- Skal være et familiemedlem med en kendt mutation i et af de tre store FTLD-relaterede gener: MAPT, PGRN eller C9ORF72.
- Mindst 18 år.
- Den dominerende fænotype hos slægtninge skal være kognitiv/adfærdsmæssig (dvs. slægtninge, hvor parkinsonisme eller ALS er den dominerende kliniske fænotype blandt berørte slægtninge, kan udelukkes)
- Hav en pålidelig informant, der personligt taler med eller ser emnet mindst en gang om ugen.
- Emnet er tilstrækkeligt flydende i engelsk til at fuldføre alle foranstaltninger
- Forsøgspersonen skal være villig og i stand til at give samtykke til protokollen og gennemgå årlige evalueringer over 3 år.
- Forsøgspersonen skal være villig og i stand til at gennemgå neuropsykologisk test (i det mindste ved baseline besøg).
- Forsøgspersonen må ikke have nogen kontraindikation for MR-billeddannelse.
Eksklusionskriterier
- Kendt tilstedeværelse af en strukturel hjernelæsion (f. tumor, kortikalt infarkt).
- Tilstedeværelse af en anden neurologisk lidelse, som kan påvirke fund (f.eks. multipel sklerose).
- Forsøgspersonen er uvillig til at vende tilbage til opfølgning årligt, gennemgå neuropsykologisk test og MR-billeddannelse.
- Forsøgspersonen har ingen pålidelig informant.
