Evaluación longitudinal de sujetos con demencia frontotemporal familiar (LEFFTDS)

Este estudio multicéntrico inscribirá a 300 miembros de familias con demencia frontotemporal familiar (FTD) en 8 centros de investigación de FTD experimentados con una mutación conocida en MAPT, PGRN o C9ORF72 (100 portadores de mutaciones con demencia leve o mínimamente sintomática pero sin demencia, 100 portadores de mutaciones asintomáticas). portadores y 100 familiares clínicamente normales que no son portadores de mutaciones) para obtener evaluaciones anuales que incluyen T1-MRI, FLAIR, imágenes de tensor de difusión (DTI), perfusión de ASL (ASLp), resonancia magnética funcional de conectividad intrínseca (icfMRI), espectroscopia de RM (MRS ), LCR, sangre y evaluación conductual, neuropsicológica y funcional, para un total de tres evaluaciones por participante.
Un objetivo principal de este estudio es identificar los métodos más sólidos y confiables para rastrear la progresión de la enfermedad en la FTD familiar para que los ensayos terapéuticos modificadores de la enfermedad puedan diseñarse adecuadamente.

Descripción
Criterios de inclusión:
- Debe ser un miembro de la familia con una mutación conocida en uno de los tres genes principales relacionados con FTLD: MAPT, PGRN o C9ORF72.
- Al menos 18 años de edad.
- El fenotipo predominante en la familia debe ser cognitivo/conductual (es decir, las familias en las que el parkinsonismo o la ELA es el fenotipo clínico predominante entre los familiares afectados pueden ser excluidas)
- Tenga un informante confiable que personalmente hable o vea a ese sujeto al menos una vez por semana.
- El sujeto tiene suficiente fluidez en inglés para completar todas las medidas.
- El sujeto debe estar dispuesto y ser capaz de dar su consentimiento al protocolo y someterse a evaluaciones anuales durante 3 años.
- El sujeto debe estar dispuesto y ser capaz de someterse a pruebas neuropsicológicas (al menos en la visita inicial).
- El sujeto no debe tener contraindicaciones para la resonancia magnética.
Criterio de exclusión
- Presencia conocida de una lesión cerebral estructural (p. tumor, infarto cortical).
- Presencia de otro trastorno neurológico que podría afectar los hallazgos (p. ej., esclerosis múltiple).
- El sujeto no está dispuesto a regresar para el seguimiento anual, someterse a pruebas neuropsicológicas y resonancia magnética.
- El sujeto no tiene informante fiable.
