Une étude de phase 2, multicentrique, ouverte, à un seul bras pour évaluer l'efficacité et l'innocuité du TAK-226 pour l'anémie transfusionnelle chez les patients japonais atteints de syndromes myélodysplasiques à risque très faible, faible ou intermédiaire

L'objectif principal de l'étude est d'évaluer comment le TAK-226 améliore les symptômes de l'anémie transfusionnelle chez les patients japonais atteints de syndromes myélodysplasiques à faible risque.

L'étude comprend une période de dépistage (jusqu'à 6 semaines), une période de traitement, une période de suivi de sécurité (8 semaines) et une période de suivi à long terme (5 ans à partir de la première dose du médicament de l'étude ou 3 ans après la dernière dose, selon la période la plus longue).

Les participants à cette étude recevront le TAK-226 pendant la période de traitement. Par la suite, les participants seront surveillés pour les effets secondaires liés au traitement de l'étude pendant la période de suivi de sécurité et la période de suivi à long terme. La durée approximative de participation pour un participant est d'environ 6 ans.

Pendant la période de l'étude, les participants se rendront plusieurs fois à la clinique/l'hôpital de l'étude selon le calendrier de l'étude. Pendant la période de traitement, les participants se rendront à la clinique/l'hôpital environ toutes les deux à quatre semaines.

Critères d'inclusion :

  1. Les participants ou leur représentant légalement autorisé doivent être disposés et capables de signer le consentement éclairé et de respecter les exigences du protocole.
  2. Participant adulte japonais de sexe masculin ou féminin âgé de ≥18 ans au moment de la signature du consentement éclairé.
  3. Diagnostic de SMD avec ou sans RS (tel que déterminé dans un aspirat de moelle osseuse évaluable prélevé lors du dépistage pour confirmer le diagnostic) selon la classification OMS 2016 qui répond à la classification IPSS-R des SMD à risque très faible, faible ou intermédiaire.

    Remarque : En raison des impacts attendus de la transfusion, les valeurs d'Hgb provenant d'échantillons sanguins prélevés dans les 14 jours suivant une transfusion de globules rouges et la numération plaquettaire obtenue dans les 7 jours suivant une transfusion de plaquettes ne peuvent pas être utilisées pour évaluer l'IPSS-R pour l'éligibilité.

  4. Dépendance transfusionnelle évaluée dans les 16 semaines précédant immédiatement l'inclusion en deux blocs de 8 semaines classés comme suit :

    a. LTB, définie comme 4 à 7 unités de globules rouges par 16 semaines ; ou b. HTB, définie comme ≥8 unités de globules rouges par 16 semaines ; et c. Pour tous les participants : i. Seuls les événements transfusionnels pour une Hgb pré-transfusion <10 g/dL sont comptabilisés pour l'éligibilité ; ii. Au moins 1 événement transfusionnel dans chaque bloc de 8 semaines et un minimum de 2 événements transfusionnels séparés d'au moins 7 jours dans la période de 16 semaines précédant immédiatement l'inclusion ; et iii. Aucune période consécutive de 56 jours ne peut être exempte de transfusion de globules rouges pendant la période de 16 semaines précédant immédiatement l'inclusion.

    Remarque : Seules les transfusions pour la maladie étudiée seront comptabilisées pour la classification des participants LTB ou HTB. Les transfusions pour des maladies intercurrentes (saignement, intervention chirurgicale, infection, etc.) ne sont pas prises en compte.

  5. Réfractaire ou intolérant à un traitement antérieur par ASE (arrêté ≥4 semaines avant l'inclusion), ou peu susceptible de répondre à un traitement par ASE, défini comme suit :

    a. Réfractaire à un traitement antérieur par ASE : documentation d'une absence de réponse ou d'une réponse qui n'était plus maintenue avec un régime antérieur contenant un ASE, en monothérapie ou en association (par exemple, avec le facteur de stimulation des colonies de granulocytes [G-CSF]) ; le régime d'ASE doit avoir été soit : i. Érythropoïétine humaine recombinante ≥40 000 UI/semaine pour ≥8 doses ou équivalent ; ou ii. Darbépoétine alpha ≥500 mcg toutes les 3 semaines pour ≥4 doses ou équivalent. b. Intolérant à un traitement antérieur par ASE : documentation de l'arrêt d'un régime antérieur contenant un ASE, en monothérapie ou en association (par exemple, avec le G-CSF), à tout moment après l'introduction en raison d'une intolérance ou d'un effet indésirable. c. Peu susceptible de répondre à un traitement par ASE : faible probabilité de réponse à l'ASE basée sur un taux sérique d'EPO endogène >200 U/L.

    Remarque : En raison des impacts attendus de la transfusion sur les taux d'EPO, les échantillons sanguins prélevés dans les 14 jours suivant une transfusion de globules rouges ou dans les 7 jours suivant une transfusion de plaquettes ne peuvent pas être utilisés pour évaluer le taux sérique d'EPO pour l'éligibilité.

  6. Moins de 5 % de blastes dans un aspirat de moelle osseuse évaluable prélevé lors du dépistage.
  7. État fonctionnel ECOG de 0 à 2.
  8. Femmes en âge de procréer (FEP), définies comme une femme sexuellement mature qui n'a pas subi de stérilisation chirurgicale ou qui n'a pas été naturellement ménopausée pendant au moins 12 mois consécutifs doivent

    1. Accepter d'utiliser 1 méthode de contraception hautement efficace et 1 méthode supplémentaire efficace (barrière) en même temps, à partir du moment de la signature du consentement éclairé jusqu'à 60 jours après la dernière dose du médicament à l'étude ; ou
    2. Accepter de pratiquer une abstinence vraie, lorsque cela correspond au mode de vie préféré et habituel du participant. (L'abstinence périodique [par exemple, calendrier, ovulation, symptothermale, méthodes post-ovulation], le retrait, les spermicides uniquement et l'aménorrhée lactationnelle ne sont pas des méthodes de contraception acceptables.)
  9. Les participants de sexe masculin doivent, même s'ils sont stérilisés chirurgicalement (c'est-à-dire, après vasectomie),

    1. Accepter de pratiquer une contraception barrière efficace à partir du moment de la signature du consentement éclairé jusqu'à 60 jours après la dernière dose du médicament à l'étude ; ou
    2. Accepter de pratiquer une abstinence vraie, lorsque cela correspond au mode de vie préféré et habituel du participant. (L'abstinence périodique [par exemple, calendrier, ovulation, symptothermale, méthodes post-ovulation], le retrait, les spermicides uniquement et l'aménorrhée lactationnelle ne sont pas des méthodes de contraception acceptables.)

Critères d'exclusion :

Antécédents médicaux

  1. SMD del(5q) ou SMD lié à un traitement (secondaire).
  2. Anémie due à toute autre cause connue (par exemple, thalassémie, anémie hémolytique, événements hémorragiques ou carence en fer, B12 et/ou folate).
  3. Réception d'une transfusion de globules rouges pour toute raison autre que le SMD sous-jacent dans les 16 semaines avant l'inclusion.
  4. Maladie cardiovasculaire cliniquement significative définie comme :

    1. Maladie cardiaque de classe III ou IV selon la New York Heart Association ;
    2. Intervalle QT corrigé de Fridericia (QTcF) >500 millisecondes lors du dépistage ;
    3. Présence d'hypertension non contrôlée définie comme une pression artérielle systolique moyenne ≥160 mm Hg ou une pression artérielle diastolique ≥100 mm Hg lors du dépistage ; ou
    4. Arythmie non contrôlée, infarctus du myocarde ou angor instable dans les 6 mois précédant le dépistage.
  5. Fraction d'éjection connue <35 %, confirmée par un échocardiogramme local réalisé lors du dépistage, ou un échocardiogramme précédemment réalisé s'il a été collecté dans les 6 mois précédant le dépistage.
  6. Accident vasculaire cérébral, thrombose veineuse profonde ou embolie pulmonaire dans les 6 mois précédant le dépistage.
  7. Tout antécédent connu de leucémie myéloïde aiguë (LMA).
  8. Antécédents de malignités, autres que le SMD, sauf si le participant est exempt de la maladie (y compris l'achèvement de tout traitement, y compris un traitement d'entretien, pour une malignité antérieure) pendant ≥5 ans.     1. Carcinome basocellulaire ou spinocellulaire de la peau ;

    4. Découverte histologique fortuite de cancer de la prostate (T1a ou T1b selon le système de stadification clinique TNM [tumeur, ganglion, métastase]).
  9. Antécédents de transplantation d'organe solide ou de moelle osseuse.
  10. Infection active nécessitant des antibiotiques intraveineux dans les 28 jours ou des antibiotiques oraux dans les 14 jours avant l'inclusion.
  11. Antécédents ou infection active ou chronique connue avec le VIH, le virus de l'hépatite B (VHB) infectieux actif ou le virus de l'hépatite C (VHC) infectieux actif. Les participants positifs pour l'antigène de surface de l'hépatite B (HBsAg), l'anticorps anti-core de l'hépatite B (HBcAb) ou l'anticorps de surface de l'hépatite B (HBsAb) peuvent être éligibles à l'inclusion si leur charge virale de l'hépatite B est inférieure à la limite de détection. Les participants positifs pour les anticorps anti-virus de l'hépatite C (HCVAb) peuvent être inclus si leur charge virale de l'hépatite C est inférieure à la limite de détection.
  12. Indice de masse corporelle ≥40 kg/m².
  13. Chirurgie majeure dans les 28 jours avant l'inclusion.
  14. Antécédents d'allergie/anaphylaxie aux excipients du produit médicinal expérimental (PME) (se référer à la brochure d'information actuelle de l'elritercept [TAK 226] pour une liste des excipients) ou aux protéines recombinantes.

    Antécédents de traitement

  15. Utilisation antérieure de TAK 226, luspatercept, imetelstat ou sotatercept.
  16. Utilisation antérieure d'agents hypométhylants (AHM), d'inhibiteur de l'isocitrate déshydrogénase, de lénalidomide ou d'une thérapie immunosuppressive administrée pour le traitement du SMD.
  17. Thérapie par chélation du fer initiée dans les 8 semaines avant l'inclusion. Les participants sous doses stables de thérapie par chélation du fer pendant ≥8 semaines sont autorisés.
  18. Thérapie par vitamine B12 ou folate initiée dans les 4 semaines avant l'inclusion. Les participants sous doses de remplacement stables pendant ≥4 semaines et sans carence concomitante en vitamine B12 ou folate en cours sont autorisés.
  19. Utilisation d'androgènes dans les 8 semaines avant l'inclusion. Les participants sous doses stables d'androgènes pour l'hypogonadisme pendant ≥8 semaines sont autorisés.
  20. Utilisation de corticostéroïdes à haute dose dans les 4 semaines avant l'inclusion. Les participants sous doses chroniques stables de prednisone/prednisolone ≤10 mg/jour ou équivalent en corticostéroïdes pendant ≥4 semaines sont autorisés.
  21. Traitement avec tout médicament expérimental dans les 28 jours avant le dépistage ou, si la demi-vie du produit est connue, dans les 5 fois la demi-vie avant le dépistage, selon la période la plus longue.
  22. Participation en cours à une autre étude clinique interventionnelle. Exclusions de laboratoire (pendant le dépistage)
  23. Taux sérique d'EPO >500 U/L.
