Eine Phase-2-, multizentrische, offene, einarmige Studie zur Bewertung der Wirksamkeit und Sicherheit von TAK-226 bei transfusionsabhängiger Anämie bei japanischen Patienten mit Myelodysplastischen Syndromen mit sehr geringem, geringem oder mittlerem Risiko

Das Hauptziel der Studie ist die Bewertung, wie TAK-226 die Symptome einer transfusionsabhängigen Anämie bei japanischen Patienten mit niedrigrisiko myelodysplastischen Syndromen verbessert.

Die Studie besteht aus einer Screening-Phase (bis zu 6 Wochen), einer Behandlungsphase, einer Sicherheits-Nachbeobachtungsphase (8 Wochen) und einer Langzeit-Nachbeobachtungsphase (5 Jahre ab der ersten Dosis des Studienmedikaments oder 3 Jahre nach der letzten Dosis, je nachdem, was länger ist).

Die Teilnehmer dieser Studie erhalten TAK-226 während der Behandlungsphase. Anschließend werden die Teilnehmer während der Sicherheits-Nachbeobachtungsphase und der Langzeit-Nachbeobachtungsphase auf Nebenwirkungen im Zusammenhang mit der Studienbehandlung überwacht. Einschlusskriterien:

  1. Teilnehmer oder ihr gesetzlicher Vertreter müssen bereit und in der Lage sein, die Einwilligungserklärung zu unterzeichnen und die Protokollanforderungen einzuhalten.
  2. Japanische erwachsene männliche oder weibliche Teilnehmer ≥18 Jahre alt zum Zeitpunkt der Unterzeichnung der Einwilligungserklärung.
  3. Diagnose von MDS mit oder ohne RS (bestimmt in einer auswertbaren Knochenmarkaspiration, die während des Screenings zur Bestätigung der Diagnose entnommen wurde) gemäß WHO-Klassifikation 2016, die der IPSS-R-Klassifikation von MDS mit sehr niedrigem, niedrigem oder mittlerem Risiko entspricht.

    Hinweis: Aufgrund der erwarteten Auswirkungen von Transfusionen können Hgb-Werte aus Blutproben, die innerhalb von 14 Tagen nach einer Erythrozyten-Transfusion entnommen wurden, und Thrombozytenzahlen, die innerhalb von 7 Tagen nach einer Thrombozyten-Transfusion ermittelt wurden, nicht zur Bewertung von IPSS-R für die Eignung verwendet werden.

  4. Transfusionsabhängigkeit, bewertet in den 16 Wochen unmittelbar vor der Einschreibung in zwei 8-Wochen-Blöcken, klassifiziert als entweder:

    a. LTB, definiert als 4 bis 7 Erythrozyten-Einheiten pro 16 Wochen; oder b. HTB, definiert als ≥8 Erythrozyten-Einheiten pro 16 Wochen; und c. Für alle Teilnehmer: i. Nur Transfusionsereignisse mit einem prätransfusionellen Hgb <10 g/dL werden für die Eignung gezählt; ii. Mindestens 1 Transfusionsereignis in jedem 8-Wochen-Block und ein Minimum von 2 Transfusionsereignissen im Abstand von ≥7 Tagen innerhalb des 16-wöchigen Zeitraums unmittelbar vor der Einschreibung; und iii. Kein aufeinanderfolgender 56-Tage-Zeitraum darf während des 16-wöchigen Zeitraums unmittelbar vor der Einschreibung frei von Erythrozyten-Transfusionen sein.

    Hinweis: Nur Transfusionen für die untersuchte Erkrankung werden für die Klassifizierung von LTB- oder HTB-Teilnehmern gezählt. Transfusionen für interkurrente Erkrankungen (Blutung, chirurgischer Eingriff, Infektion usw.) werden nicht berücksichtigt.

  5. Refraktär oder intolerant gegenüber vorheriger ESA-Behandlung (abgesetzt ≥4 Wochen vor der Einschreibung) oder unwahrscheinlich, auf ESA-Behandlung anzusprechen, definiert wie folgt:

    a. Refraktär gegenüber vorheriger ESA-Behandlung: Dokumentation von Nichtansprechen oder einem Ansprechen, das mit einem vorherigen ESA-haltigen Regime nicht mehr aufrechterhalten werden konnte, entweder als Monotherapie oder Kombination (z. B. mit Granulozyten-Kolonie-stimulierendem Faktor [G-CSF]); das ESA-Regime muss entweder gewesen sein: i. Rekombinantes humanes EPO ≥40.000 IE/Woche für ≥8 Dosen oder Äquivalent; oder ii. Darbepoetin alfa ≥500 µg alle 3 Wochen für ≥4 Dosen oder Äquivalent. b. Intolerant gegenüber vorheriger ESA-Behandlung: Dokumentation der Beendigung eines vorherigen ESA-haltigen Regimes, entweder als Monotherapie oder Kombination (z. B. mit G-CSF), zu einem beliebigen Zeitpunkt nach Einführung aufgrund von Unverträglichkeit oder einer unerwünschten Wirkung.

    c. Unwahrscheinlich, auf ESA-Behandlung anzusprechen: Geringe Chance auf Ansprechen auf ESA basierend auf einem endogenen Serum-EPO-Spiegel >200 U/L.

    Hinweis: Aufgrund der erwarteten Auswirkungen von Transfusionen auf EPO-Spiegel können Blutproben, die innerhalb von 14 Tagen nach einer Erythrozyten-Transfusion oder innerhalb von 7 Tagen nach einer Thrombozyten-Transfusion entnommen wurden, nicht zur Bewertung des Serum-EPO-Spiegels für die Eignung verwendet werden.

  6. Weniger als 5% Blasten in einer auswertbaren Knochenmarkaspiration, die während des Screenings entnommen wurde.
  7. ECOG-Leistungsstatus von 0 bis 2.
  8. Frauen im gebärfähigen Alter (WOCBP), definiert als eine sexuell reife Frau, die sich keiner chirurgischen Sterilisation unterzogen hat oder die nicht natürlich seit mindestens 12 aufeinanderfolgenden Monaten postmenopausal war, müssen

Kalender-, Ovulations-, symptothermale, postovulatorische Methoden], Coitus interruptus, nur Spermizide und laktatorische Amenorrhoe sind keine akzeptablen Verhütungsmethoden.)
  9. Männliche Teilnehmer müssen, auch wenn sie chirurgisch sterilisiert sind (d. h. Zustand nach Vasektomie),

Ausschlusskriterien:

Medizinische Vorgeschichte

  1. Del(5q)-MDS oder therapiebezogenes (sekundäres) MDS.
  2. Anämie aufgrund einer anderen bekannten Ursache (z. B. Thalassämie, hämolytische Anämie, Blutungsereignisse oder Mangel an Eisen, B12 und/oder Folsäure).
  3. Erhalt von Erythrozyten-Transfusionen aus anderen Gründen als dem zugrunde liegenden MDS innerhalb von 16 Wochen vor der Einschreibung.
  4. Klinisch signifikante kardiovaskuläre Erkrankung definiert als:

    1. Herzerkrankung der New York Heart Association Klasse III oder IV;
    2. Fridericia-korrigiertes QT-Intervall (QTcF) >500 Millisekunden während des Screenings;
    3. Vorliegen von unkontrolliertem Bluthochdruck definiert als mittlerer systolischer Blutdruck ≥160 mm Hg oder diastolischer Blutdruck ≥100 mm Hg während des Screenings; oder
    4. Unkontrollierte Arrhythmie, Myokardinfarkt oder instabile Angina pectoris innerhalb von 6 Monaten vor dem Screening.
  5. Bekannte Ejektionsfraktion <35%, bestätigt durch eine lokale Echokardiographie während des Screenings oder eine zuvor durchgeführte Echokardiographie, wenn sie innerhalb von 6 Monaten vor dem Screening durchgeführt wurde.
  6. Schlaganfall, tiefe Venenthrombose oder Lungenembolie innerhalb von 6 Monaten vor dem Screening.
  7. Jede bekannte Vorgeschichte von akuter myeloischer Leukämie (AML).
  8. Frühere Vorgeschichte von Malignomen außer MDS, es sei denn, der Teilnehmer ist seit ≥5 Jahren frei von der Erkrankung (einschließlich Abschluss jeglicher Behandlung, einschließlich Erhaltungstherapie, für das vorherige Malignom).   9. Vorgeschichte von Transplantation fester Organe oder Knochenmark.
  10. Aktive Infektion, die intravenöse Antibiotika innerhalb von 28 Tagen oder orale Antibiotika innerhalb von 14 Tagen vor der Einschreibung erfordert.
  11. Vorgeschichte von oder bekannte aktive oder chronische Infektion mit HIV, aktivem infektiösem Hepatitis-B-Virus (HBV) oder aktivem infektiösem Hepatitis-C-Virus (HCV). Teilnehmer, die positiv auf Hepatitis-B-Oberflächenantigen (HBsAg), Hepatitis-B-Core-Antikörper (HBcAb) oder Hepatitis-B-Oberflächenantikörper (HBsAb) sind, können für die Einschreibung in Frage kommen, wenn ihre Hepatitis-B-Viruslast unter der Nachweisgrenze liegt. Teilnehmer, die positiv auf Hepatitis-C-Virus-Antikörper (HCVAb) sind, können eingeschlossen werden, wenn ihre Hepatitis-C-Viruslast unter der Nachweisgrenze liegt.
  12. Body-Mass-Index ≥40 kg/m².
  13. Größere Operation innerhalb von 28 Tagen vor der Einschreibung.
  14. Vorgeschichte von Allergie/Anaphylaxie gegen Hilfsstoffe des Prüfpräparats (IMP) (siehe aktuelle elritercept [TAK 226] IB für eine Liste der Hilfsstoffe) oder rekombinante Proteine.

    Behandlungsgeschichte

  15. Frühere Anwendung von TAK 226, Luspatercept, Imetelstat oder Sotatercept.
  16. Frühere Anwendung von hypomethylierenden Agenzien (HMAs), Isocitrat-Dehydrogenase-Inhibitoren, Lenalidomid oder immunsuppressiver Therapie zur Behandlung von MDS.
  17. Eisenchelationstherapie, die innerhalb von 8 Wochen vor der Einschreibung begonnen wurde. Teilnehmer mit stabilen Dosen von Eisenchelationstherapie für ≥8 Wochen sind erlaubt.
  18. Vitamin-B12- oder Folsäuretherapie, die innerhalb von 4 Wochen vor der Einschreibung begonnen wurde. Teilnehmer mit stabilen Ersatzdosen für ≥4 Wochen und ohne gleichzeitigen Vitamin-B12- oder Folsäuremangel sind erlaubt.
  19. Androgengebrauch innerhalb von 8 Wochen vor der Einschreibung. Teilnehmer mit stabiler Androgendosierung für Hypogonadismus für ≥8 Wochen sind erlaubt.
  20. Hochdosierte Kortikosteroidanwendung innerhalb von 4 Wochen vor der Einschreibung. Teilnehmer mit stabilen chronischen Steroiddosen von Prednison/Prednisolon ≤10 mg/Tag oder Kortikosteroid-Äquivalent für ≥4 Wochen sind erlaubt.
  21. Behandlung mit einem beliebigen Prüfpräparat innerhalb von 28 Tagen vor dem Screening oder, wenn die Halbwertszeit des Produkts bekannt ist, innerhalb des 5-fachen der Halbwertszeit vor dem Screening, je nachdem, was länger ist.

  32. Schwangere oder stillende Frau.
