Tutkimus TAK-226:sta verensiirtoon riippuvaisesta anemiaa sairastavista japanilaisista potilaista, joilla on matalan riskin myelodysplastinen oireyhtymä
Fase 2 -tutkimus, monikeskuksinen, avoimimerkkinen, yksihaarainen tutkimus, jossa arvioidaan TAK-226:n tehokkuutta ja turvallisuutta verensiirrosta tarvitsevien anemiapotilaiden hoidossa japanilaisilla potilailla, joilla on erittäin matalan, matalan tai keskiriskin myelodysplastinen oireyhtymä
Tutkimuksen päätavoitteena on arvioida, kuinka TAK-226 parantaa transfusioon riippuvaisen anemian oireita japanilaisilla potilailla, joilla on matalan riskin myelodysplastinen oireyhtymä.
Tutkimus koostuu seulontajaksosta (enintään 6 viikkoa), hoitojaksosta, turvallisuusseurantajaksosta (8 viikkoa) ja pitkäaikaiseurantajaksosta (5 vuotta tutkimuslääkkeen ensimmäisestä annoksesta tai 3 vuotta viimeisestä annoksesta, kumpi on pidempi).
Tutkimukseen osallistuville annetaan TAK-226 hoitojakson aikana. Tämän jälkeen osallistujia seurataan tutkimushoitoihin liittyvien sivuvaikutusten varalta turvallisuusseuranta- ja pitkäaikaiseurantajaksojen aikana. Yhden osallistujan arvioitu osallistumisaika on enintään noin 6 vuotta.
Tutkimusjakson aikana osallistujat vierailevat tutkimusklinikalla/sairaalassa useita kertoja tutkimusaikataulun mukaisesti. Hoitojakson aikana osallistujat tulevat klinikalle/sairaalaan noin kahden neljän viikon välein.

Kuvaus
Osallistumiskriteerit:
- Osallistujien tai heidän laillisten edustajiensa on oltava halukkaita ja kykeneviä allekirjoittamaan tietoon perustuvan suostumuksen (ICF) ja noudattamaan tutkimussuunnitelman vaatimuksia.
- Japanilainen täysi-ikäinen mies- tai naisosallistuja, joka on ≥18 vuotta täysi-ikäinen tietoon perustuvan suostumuksen allekirjoitushetkellä.
Diagnoosi MDS:stä, jossa on tai ei ole RS:ää (kuten määritetään seulontavaiheessa kerätystä arvioitavasta luuytimenäytteestä diagnoosin vahvistamiseksi) WHO:n 2016 luokituksen mukaisesti, joka täyttää IPSS-R-luokituksen erittäin matalan, matalan tai keskimääräisen riskin MDS:n.
Huomio: Transfuusion odotettujen vaikutusten vuoksi verinäytteistä mitattuja hemoglobiiniarvoja, jotka on kerätty 14 päivän kuluessa punasolusiirron jälkeen, ja verihiutalelukumäärää, joka on saatu 7 päivän kuluessa verihiutalesiirron jälkeen, ei voida käyttää IPSS-R:n arviointiin kelpoisuuden määrittämiseksi.
Transfuusiontarve arvioitu 16 viikon ajalta välittömästi ennen osallistumista kahdessa 8 viikon lohkossa luokiteltuna joko:
a. LTB, määriteltynä 4–7 punasoluyksikköä 16 viikon aikana; tai b. HTB, määriteltynä ≥8 punasoluyksikköä 16 viikon aikana; ja c. Kaikille osallistujille: i. Vain transfuusion tapahtumat, joissa pretransfuusion hemoglobiini on <10 g/dL, lasketaan kelpoisuuteen; ii. Vähintään yksi transfuusion tapahtuma kussakin 8 viikon lohkossa ja vähintään kaksi transfuusion tapahtumaa, joiden välinen aika on ≥7 päivää, 16 viikon ajalta välittömästi ennen osallistumista; ja iii. Ei yhtäjaksoista 56 päivän ajanjaksoa, joka olisi punasolusiirtojen vapaa, 16 viikon ajalta välittömästi ennen osallistumista.
Huomio: Vain tutkittavaan sairauteen liittyvät transfuusion lasketaan LTB- tai HTB-osallistujien luokitteluun. Sattuvaisuuksissa esiintyviin sairauksiin (verenvuoto, leikkaus, infektio jne.) liittyviä transfuusion ei oteta huomioon.
Refraktori tai sietämätön aiempaa ESA-hoitoa kohtaan (lopetettu ≥4 viikkoa ennen osallistumista) tai ei todennäköisesti reagoi ESA-hoitoon, määritelty seuraavasti:
a. Refraktori aiempaa ESA-hoitoa kohtaan: dokumentoitu ei-vastetta tai vastetta, jota ei enää pystytty ylläpitämään aiemman ESA-sisältävän hoidon aikana, joko yksittäisenä lääkkeenä tai yhdistelmänä (esim. granulocyyttien koloniastimuloiva tekijä [G-CSF]); ESA-hoito on täytynyt olla joko: i. Rekombinanttia ihmisen EPO:ta ≥40 000 IU/viikko vähintään 8 annokselle tai vastaava; tai ii. Darbepoetiini-alfaa ≥500 mcrg 3 viikon välein vähintään 4 annokselle tai vastaava. b. Sietämätön aiempaa ESA-hoitoa kohtaan: dokumentoitu aiemman ESA-sisältävän hoidon keskeyttäminen, joko yksittäisenä lääkkeenä tai yhdistelmänä (esim. G-CSF), milloin tahansa käyttöönoton jälkeen sietämättömyyden tai haittavaikutuksen vuoksi.
c. Ei todennäköisesti reagoi ESA-hoitoon: alhainen todennäköisyys reagoida ESA:han perustuen endogeeniseen seerumin EPO-tasoon >200 U/L.
Huomio: Transfuusion odotettujen vaikutusten vuoksi EPO-tasoihin verinäytteitä, jotka on kerätty 14 päivän kuluessa punasolusiirron jälkeen tai 7 päivän kuluessa verihiutalesiirron jälkeen, ei voida käyttää seerumin EPO-tason arviointiin kelpoisuuden määrittämiseksi.
- Alle 5 % blasteja arvioitavassa luuytimenäytteessä, joka on kerätty seulontavaiheessa.
- ECOG suorituskykyluokka 0–2.
Lapsen saanti-ikäiset naiset (WOCBP), määriteltynä seksuaalisesti kypsäksi naiseksi, joka ei ole käynyt läpi kirurgista sterilisaatiota tai joka ei ole luonnollisesti ollut menopaussin jälkeinen vähintään 12 peräkkäistä kuukautta, on
- Sovittava käyttämään yhtä erittäin tehokasta ehkäisykeinoa ja yhtä lisätehokasta (esteellistä) keinoa samanaikaisesti tietoon perustuvan suostumuksen allekirjoitushetkestä tutkimuslääkkeen viimeisen annoksen jälkeen 60 päivän ajan; tai
- Sovittava harjoittamaan todellista pidättyvyyttä, kun tämä on osallistujan suosiman ja tavallisen elämäntavan mukainen. (Ajoittainen pidättyvyys [esim. kalenteri-, ovulaatio-, oirelämpö-, jälkiovulaatiomenetelmät], keskeytys, vain spermasidat ja imetyksen aiheuttama amenorrea eivät ole hyväksyttäviä ehkäisymenetelmiä.)
Miesosallistujien on, vaikka hän olisi kirurgisesti sterilisoitu (eli vasektomian jälkeen),
- Sovittava harjoittamaan tehokasta esteellistä ehkäisyä tietoon perustuvan suostumuksen allekirjoitushetkestä tutkimuslääkkeen viimeisen annoksen jälkeen 60 päivän ajan; tai
- Sovittava harjoittamaan todellista pidättyvyyttä, kun tämä on osallistujan suosiman ja tavallisen elämäntavan mukainen. (Ajoittainen pidättyvyys [esim. kalenteri-, ovulaatio-, oirelämpö-, jälkiovulaatiomenetelmät], keskeytys, vain spermasidat ja imetyksen aiheuttama amenorrea eivät ole hyväksyttäviä ehkäisymenetelmiä.)
Poissulkemiskriteerit:
Sairaushistoria
- Del(5q) MDS tai lääkehoidon aiheuttama (toissijainen) MDS.
- Anemia minkä tahansa muun tunnetun syyn vuoksi (esim. talassemia, hemolyyttinen anemia, verenvuototapahtumat tai raudan, B12-vitamiinin ja/tai foolihapon puute).
- Punasolusiirto saatu mistä tahansa muusta syystä kuin taustalla olevan MDS:n vuoksi 16 viikon aikana ennen osallistumista.
Kliinisesti merkittävä sydän- ja verisuonitauti määriteltynä:
- New York Heart Associationin sydäntautiluokka III tai IV;
- Friderician korjattu QT (QTcF) -väli >500 millisekuntia seulontavaiheessa;
- Hallitsematon verenpaine määriteltynä keskiarvoltaan systolinen verenpaine ≥160 mm Hg tai diastolinen verenpaine ≥100 mm Hg seulontavaiheessa; tai
- Hallitsematon rytmihäiriö, sydäninfarkti tai epävakaa angina 6 kuukauden aikana ennen seulontaa.
- Tunnettu ejektiofraktio <35 %, vahvistettuna paikallisella seulontavaiheessa suoritetulla ekokardiogrammilla tai aiemmin suoritetulla ekokardiogrammilla, jos se on kerätty 6 kuukauden aikana ennen seulontaa.
- Aivohalvaus, syvä laskimotukos tai keuhkoveritulppa 6 kuukauden aikana ennen seulontaa.
- Mikä tahansa tunnettu akuutti myelooinen leukemia (AML) -historia.
Aiempi syöpähistoria MDS:ää lukuun ottamatta, ellei osallistuja ole ollut sairaudesta vapaa (mukaan lukien minkä tahansa hoidon, kuten ylläpitohoidon, suorittaminen aiempaan syöpään) ≥5 vuotta. Kuitenkin osallistujat, joilla on seuraavien tilojen historia tai samanaikainen diagnoosi, ovat sallittuja, jos he eivät vaadi systemaattista hoitoa:
- Ihon basaali- tai levyepiteelisolu karsinooma;
- Kohdunkaulan karsinooma in situ;
- Rinnan karsinooma in situ; ja/tai
- Sattumanvarainen histologinen löydös eturauhassyövästä (T1a tai T1b käyttäen TNM-kliinistä luokitusjärjestelmää).
- Historia kiinteiden elinten tai luuytimen siirroista.
- Aktiivinen infektio, joka vaatii laskimonsisäisiä antibiootteja 28 päivän aikana tai suun kautta annettavia antibiootteja 14 päivän aikana ennen osallistumista.
- Historia tai tunnettu aktiivinen tai krooninen HIV-infektio, aktiivinen tarttuva hepatiitti B -virus (HBV) tai aktiivinen tarttuva hepatiitti C -virus (HCV). Osallistujat, jotka ovat positiivisia hepatiitti B pintaanigeenille (HBsAg), hepatiitti B ydinantigeenille (HBcAb) tai hepatiitti B pintaanigeenille (HBsAb), voivat olla kelvollisia osallistumaan, jos heidän hepatiitti B -viruskuormansa on alle havaitsemistason. Osallistujat, jotka ovat positiivisia hepatiitti C -virusantigeeneille (HCVAb), voivat osallistua, jos heidän hepatiitti C -viruskuormansa on alle havaitsemistason.
- Painoindeksi ≥40 kg/m².
- Suuri leikkaus 28 päivän aikana ennen osallistumista.
Allergia-/anafylaksiahistoria tutkimuslääkeaineen (IMP) apuaineille (katso nykyinen elritercept [TAK 226] IB apuaineiden luettelosta) tai rekombinanttiproteiineille.
Hoitohistoria
- Aiempi TAK 226:n, luspaterceptin, imetelstatin tai sotaterceptin käyttö.
- Aiempi hypometyloivien aineiden (HMA), isositraattidehydrogenaasin estäjän, lenalidomidin tai immunosupressiivisen hoidon käyttö MDS:n hoidossa.
- Rautakelaushoito aloitettu 8 viikon aikana ennen osallistumista. Osallistujat, jotka ovat vakailla rautakelaushoidon annoksilla ≥8 viikkoa, ovat sallittuja.
- B12-vitamiini- tai foolihappohoito aloitettu 4 viikon aikana ennen osallistumista. Osallistujat, jotka ovat vakailla korvausannoksilla ≥4 viikkoa ja joilla ei ole samanaikaista B12-vitamiinin tai foolihapon puutetta, ovat sallittuja.
- Androgeenien käyttö 8 viikon aikana ennen osallistumista. Osallistujat, jotka ovat vakailla androgeeniannoksilla hypogonadismin vuoksi ≥8 viikkoa, ovat sallittuja.
- Korkea-annoksisen kortikosteroidin käyttö 4 viikon aikana ennen osallistumista. Osallistujat, jotka ovat vakailla kroonisilla steroidiannoksilla prednisolonia/prednisolonia ≤10 mg/päivä tai kortikosteroidivastaavaa ≥4 viikkoa, ovat sallittuja.
- Hoidettu millä tahansa tutkimuslääkkeellä 28 päivän aikana ennen seulontaa tai, jos tuotteen puoliintumisaika on tiedossa, 5 kertaa puoliintumisajan aikana ennen seulontaa, kumpi on pidempi.
- Käynnissä oleva osallistuminen toiseen interventiiviseen kliiniseen tutkimukseen. Laboratoriopoissulkemiset (seulontavaiheessa)
- Seerumin EPO-taso >500 U/L. Huomio: Transfuusion odotettujen vaikutusten vuoksi EPO-tasoihin laboratoriotuloksia verinäytteistä, jotka on kerätty 14 päivän kuluessa punasolusiirron jälkeen, ei voida käyttää seerumin EPO-tason arviointiin kelpoisuuden määrittämiseksi.
- Verihiutalelukumäärä ≥450×10³/mcrL tai ≤25×10³/mcrL. Huomio: Transfuusion odotettujen vaikutusten vuoksi laboratoriotuloksia verinäytteistä, jotka on kerätty 7 päivän kuluessa verihiutalesiirron jälkeen, ei voida käyttää verihiutalelukumäärän arviointiin kelpoisuuden määrittämiseksi.
- Absoluuttinen neutrofiililukumäärä ≤500/mcrL
- Seerumin AST tai ALT ≥3×ylärajan (ULN).
- Kokonaisbilirubiini ≥2×ULN, ellei se ole Gilberten oireyhtymän aiheuttama.
- Ferritiini ≤50 mcrg/L.
- Foolihappo ≤2,0 ng/mL.
- B12-vitamiini ≤200 pg/mL.
Arvioitu glomerulaarinen suodatusnopeus <30 mL/min/1,73 m² Japanilaisen nefrologian seuran määrittämänä. Laskettu Japanilaisille korjauskaavalla.
eGFR=194×(seerumin kreatiniiniarvo)^-1,094×(ikä)^-0,287×(sukupuolen korjauskerroin), Sukupuolen korjauskerroin: 0,739 naisilla.
Muut
- Raskaana oleva tai imettävä nainen. Huomio: Osallistujat, jotka voivat olla hyvin varhaisessa raskausvaiheessa lääkärin haastattelun perusteella negatiivisella raskaustestillä, suljetaan tutkimuksesta. Osallistujat, jotka imettävät, ovat kelvollisia, jos he lopettavat imetyksen ennen tutkimuslääkkeen ensimmäistä annosta ja jatkavat 60 päivää tutkimuslääkkeen viimeisen annoksen jälkeen.
- Mikä tahansa muu edellä erikseen mainitsematon tila, joka tutkijan mielestä estäisi osallistujaa osallistumasta tutkimukseen tai voisi hämärtää tutkimuksen tietojen tulkintaa.

An AE is any untoward medical occurrence in a clinical trial participant, temporally associated with the use of the trial intervention, whether or not the occurrence is considered related to the trial intervention.
An AE can therefore be any unfavorable and unintended sign (including an abnormal laboratory finding), symptom, or disease (new or exacerbated) temporally associated with the use of the trial intervention.
A TEAE is defined as an AE that commences on or after the first dose of the study treatment and within 60 days after the last dose of the study treatment, or analysis cutoff date, whichever is earlier.
An SAE is any untoward medical occurrence that, at any dose: results in death, is life threatening, requires in-patient hospitalization or prolongation of existing hospitalization, results in persistent or significant disability/incapacity, is a congenital anomaly/birth defect, is a medically important event.
