Een fase 2, multicenter, open-label, single-arm studie om de werkzaamheid en veiligheid van TAK-226 te evalueren voor transfusie-afhankelijke anemie bij Japanse patiënten met zeer laag, laag of intermediair risico myelodysplastische syndromen

Het hoofddoel van de studie is om te evalueren hoe TAK-226 de symptomen van transfusieafhankelijke anemie verbetert bij Japanse patiënten met myelodysplastische syndromen met een lager risico.

De studie bestaat uit een screeningsperiode (tot 6 weken), een behandelingsperiode, een veiligheidsopvolgingsperiode (8 weken) en een langetermijnopvolgingsperiode (5 jaar vanaf de eerste dosis van het studiegeneesmiddel of 3 jaar na de laatste dosis, afhankelijk van welke langer is).

Deelnemers aan deze studie krijgen TAK-226 toegediend tijdens de behandelingsperiode. Vervolgens worden de deelnemers gemonitord op bijwerkingen gerelateerd aan de studiebehandeling tijdens de veiligheidsopvolgingsperiode en de langetermijnopvolgingsperiode. De geschatte duur van deelname voor een deelnemer is tot ongeveer 6 jaar.

Tijdens de studieperiode bezoeken deelnemers meerdere keren de studiepolikliniek/het ziekenhuis volgens het studieschema. Tijdens de behandelingsperiode komen de deelnemers ongeveer elke twee tot vier weken naar de polikliniek/het ziekenhuis.

Inclusiecriteria:

  1. Deelnemers of hun wettelijk vertegenwoordiger moeten bereid en in staat zijn om de geïnformeerde toestemmingsverklaring (ICF) te ondertekenen en zich aan de protocolvereisten te houden.
  2. Japanse volwassen mannelijke of vrouwelijke deelnemer ≥18 jaar op het moment van ondertekening van de geïnformeerde toestemming.
  3. Diagnose van MDS met of zonder RS (zoals bepaald in een beoordeelbaar beenmergaspiraat afgenomen tijdens de screening om de diagnose te bevestigen) volgens de WHO 2016-classificatie die voldoet aan de IPSS-R-classificatie van MDS met zeer laag, laag of gemiddeld risico.

    Opmerking: Vanwege de verwachte effecten van transfusie kunnen Hgb-waarden uit bloedmonsters die binnen 14 dagen na een erytrocytentransfusie zijn afgenomen en het aantal bloedplaatjes verkregen binnen 7 dagen na een bloedplaatjestransfusie niet worden gebruikt om IPSS-R voor geschiktheid te evalueren.

  4. Transfusieafhankelijkheid beoordeeld in de 16 weken direct voorafgaand aan inschrijving in twee blokken van 8 weken, geclassificeerd als:

    a. LTB, gedefinieerd als 4 tot 7 erytrocytentransfusies per 16 weken; of b. HTB, gedefinieerd als ≥8 erytrocytentransfusies per 16 weken; en c. Voor alle deelnemers: i. Alleen transfusiegebeurtenissen met een pretransfusie Hgb <10 g/dL tellen mee voor geschiktheid; ii. Minstens 1 transfusiegebeurtenis in elk blok van 8 weken en minimaal 2 transfusiegebeurtenissen met tussenpozen van ≥7 dagen binnen de 16 weken direct voorafgaand aan inschrijving; en iii. Geen opeenvolgende periode van 56 dagen mag vrij zijn van erytrocytentransfusies tijdens de 16 weken direct voorafgaand aan inschrijving.

    Opmerking: Alleen transfusies voor de bestudeerde ziekte tellen mee voor classificatie voor LTB- of HTB-deelnemers. Transfusies voor intercurrente ziekten (bloeding, chirurgische ingreep, infectie, enz.) worden niet meegerekend.

  5. Refractair of intolerant voor eerdere ESA-behandeling (gestopt ≥4 weken voor inschrijving), of onwaarschijnlijk te reageren op ESA-behandeling, gedefinieerd als volgt:

    a. Refractair voor eerdere ESA-behandeling: documentatie van non-respons of een respons die niet langer werd gehandhaafd met een eerdere ESA-bevattende behandeling, als monotherapie of combinatie (bijv. met granulocyt-koloniestimulerende factor [G-CSF]); ESA-behandeling moet zijn geweest: i. Recombinant humaan EPO ≥40.000 IE/week voor ≥8 doses of equivalent; of ii. Darbepoëtine alfa ≥500 mcrg elke 3 weken voor ≥4 doses of equivalent. b. Intolerant voor eerdere ESA-behandeling: documentatie van stopzetting van een eerdere ESA-bevattende behandeling, als monotherapie of combinatie (bijv. met G-CSF), op enig moment na introductie wegens intolerantie of een bijwerking. c. Onwaarschijnlijk te reageren op ESA-behandeling: lage kans op respons op ESA op basis van een endogeen serum EPO-gehalte >200 U/L.

    Opmerking: Vanwege de verwachte effecten van transfusie op EPO-gehalten kunnen bloedmonsters die binnen 14 dagen na een erytrocytentransfusie of binnen 7 dagen na een bloedplaatjestransfusie zijn afgenomen niet worden gebruikt om het serum EPO-gehalte voor geschiktheid te evalueren.

  6. Minder dan 5% blasten in een beoordeelbaar beenmergaspiraat afgenomen tijdens de screening.
  7. ECOG-prestatiestatus van 0 tot 2.
  8. Vrouwen met vruchtbaarheidspotentieel (WOCBP), gedefinieerd als een seksueel volwassen vrouw die geen chirurgische sterilisatie heeft ondergaan of die niet natuurlijk postmenopauzaal is geweest gedurende ten minste 12 opeenvolgende maanden, moeten:

    1. Akkoord gaan met het gebruik van 1 zeer effectieve anticonceptiemethode en 1 aanvullende effectieve (barrière)methode tegelijkertijd, vanaf het ondertekenen van de geïnformeerde toestemming tot 60 dagen na de laatste dosis van het studiegeneesmiddel; of
    2. Akkoord gaan met ware onthouding, wanneer dit in overeenstemming is met de voorkeur en gebruikelijke levensstijl van de deelnemer. (Periodieke onthouding [bijv. kalender, ovulatie, symptotherm, postovulatiemethoden], terugtrekking, alleen zaaddodende middelen en lactatie-amenorroe zijn geen aanvaardbare anticonceptiemethoden.)
  9. Mannelijke deelnemers moeten, zelfs als ze chirurgisch gesteriliseerd zijn (d.w.z. status na vasectomie):

    1. Akkoord gaan met effectieve barrière-anticonceptie vanaf het ondertekenen van de geïnformeerde toestemming tot 60 dagen na de laatste dosis van het studiegeneesmiddel; of

Exclusiecriteria:

Medische geschiedenis

  1. Del(5q) MDS of therapiegerelateerde (secundaire) MDS.
  2. Anemie door enige andere bekende oorzaak (bijv. thalassemie, hemolytische anemie, bloedingen, of tekort aan ijzer, B12 en/of foliumzuur).
  3. Ontvangst van erytrocytentransfusie om andere reden(en) dan onderliggende MDS binnen 16 weken voor inschrijving.
  4. Klinisch significante cardiovasculaire ziekte gedefinieerd als:

    1. New York Heart Association-hartziekte klasse III of IV;
    2. Fridericia gecorrigeerde QT (QTcF)-interval >500 milliseconden tijdens de screening;
    3. Aanwezigheid van ongecontroleerde hypertensie gedefinieerd als gemiddelde systolische bloeddruk ≥160 mm Hg of diastolische bloeddruk ≥100 mm Hg tijdens de screening; of
    4. Ongecontroleerde aritmie, myocardinfarct of instabiele angina binnen 6 maanden voor de screening.
  5. Bekende ejectiefractie <35%, bevestigd door een lokale echocardiografie uitgevoerd tijdens de screening, of een eerder uitgevoerde echocardiografie indien afgenomen binnen 6 maanden voor de screening.
  6. Beroerte, diepe veneuze trombose of longembolie binnen 6 maanden voor de screening.
  7. Enige bekende voorgeschiedenis van acute myeloïde leukemie (AML).
  8. Eerdere voorgeschiedenis van maligniteiten, anders dan MDS, tenzij de deelnemer ≥5 jaar vrij is van de ziekte (inclusief voltooiing van elke behandeling, inclusief onderhoud, voor eerdere maligniteit). Echter, deelnemers met een voorgeschiedenis of gelijktijdige diagnose van de volgende aandoeningen zijn toegestaan indien geen systemische therapie vereist is:

    1. Basale of plaveiselcelcarcinoom van de huid;
    2. Carcinoom in situ van de baarmoederhals;
    3. Carcinoom in situ van de borst; en/of
    4. Incidentele histologische bevinding van prostaatkanker (T1a of T1b volgens het tumor, node, metastase [TNM] klinisch stagingssysteem).
  9. Voorgeschiedenis van vaste orgaan- of beenmergtransplantatie.
  10. Actieve infectie die intraveneuze antibiotica vereist binnen 28 dagen of orale antibiotica binnen 14 dagen voor inschrijving.
  11. Voorgeschiedenis van of bekende actieve of chronische infectie met HIV, actief infectieus hepatitis B-virus (HBV) of actief infectieus hepatitis C-virus (HCV). Deelnemers die positief zijn voor hepatitis B-oppervlakteantigeen (HBsAg), hepatitis B-kernantilichaam (HBcAb) of hepatitis B-oppervlakteantilichaam (HBsAb) kunnen in aanmerking komen voor inschrijving als hun hepatitis B-virale lading onder de detectielimiet ligt. Deelnemers die positief zijn voor hepatitis C-virusantilichamen (HCVAb) kunnen worden ingeschreven als hun hepatitis C-virale lading onder de detectielimiet ligt.
  12. Body mass index ≥40 kg/m².
  13. Grote chirurgie binnen 28 dagen voor inschrijving.
  14. Voorgeschiedenis van allergie/anafylaxie voor hulpstoffen van het onderzoeksgeneesmiddel (IMP) (zie de huidige elritercept [TAK 226] IB voor een lijst van hulpstoffen) of recombinante eiwitten.

    Behandelingsgeschiedenis

  15. Eerder gebruik van TAK 226, luspatercept, imetelstat of sotatercept.
  16. Eerder gebruik van hypomethylerende middelen (HMA's), isocitraatdehydrogenaseremmer, lenalidomide of immunosuppressieve therapie gegeven voor behandeling van MDS.
  17. Ijzerchelatie-therapie gestart binnen 8 weken voor inschrijving. Deelnemers op stabiele doses ijzerchelatie-therapie gedurende ≥8 weken zijn toegestaan.
  18. Vitamine B12- of foliumzuurtherapie gestart binnen 4 weken voor inschrijving. Deelnemers op stabiele vervangingsdoses gedurende ≥4 weken en zonder gelijktijdig vitamine B12- of foliumzuurtekort zijn toegestaan.
  19. Androgeengebruik binnen 8 weken voor inschrijving. Deelnemers op stabiele androgeendosering voor hypogonadisme gedurende ≥8 weken zijn toegestaan.
  20. Hooggedoseerd corticosteroidgebruik binnen 4 weken voor inschrijving. Deelnemers op stabiele chronische steroïdedoses van prednison/prednisolon ≤10 mg/dag of corticosteroid-equivalent gedurende ≥4 weken zijn toegestaan.
  21. Behandeling met enig onderzoeksgeneesmiddel binnen 28 dagen voor de screening of, indien de halfwaardetijd van het product bekend is, binnen 5 keer de halfwaardetijd voor de screening, afhankelijk van welke langer is.
  22. Lopende deelname aan een andere interventionele klinische studie. Laboratoriumuitsluitingen (tijdens screening)
  23. Serum EPO-gehalte >500 U/L. Opmerking: Vanwege de verwachte effecten van transfusie op EPO-gehalten kunnen laboratoriumresultaten van bloedmonsters die binnen 14 dagen na een erytrocytentransfusie zijn afgenomen niet worden gebruikt om het serum EPO-gehalte voor geschiktheid te evalueren.
  24. Aantal bloedplaatjes ≥450×10³/mcrL of ≤25×10³/mcrL.   32. Zwangere of lacterende vrouw. Opmerking: Deelnemers die mogelijk in een zeer vroeg stadium van zwangerschap zijn op basis van het artseninterview met een negatieve zwangerschapstest, zijn uitgesloten van de studie. Deelnemers die lacteren komen in aanmerking als ze stoppen met borstvoeding vanaf voor de eerste dosis van het studiegeneesmiddel tot 60 dagen na de laatste dosis van het studiegeneesmiddel.

Takeda biedt toegang tot de geanonimiseerde individuele deelnemersgegevens (IPD) voor in aanmerking komende onderzoeken om gekwalificeerde onderzoekers te helpen bij het aanpakken van legitieme wetenschappelijke doelstellingen (Takeda's gegevensdelingstoewijding is beschikbaar op https://clinicaltrials.takeda.com/takedas-commitment?commitment=5). Deze IPD's zullen worden verstrekt in een beveiligde onderzoeksomgeving na goedkeuring van een verzoek om gegevensdeling, en onder de voorwaarden van een gegevensdelings-overeenkomst.

IPD-toegangscriteria voor delen

IPD van in aanmerking komende studies zal worden gedeeld met gekwalificeerde onderzoekers volgens de criteria en het proces beschreven op https://vivli.org/ourmember/takeda/. Voor goedgekeurde aanvragen krijgen de onderzoekers toegang tot geanonimiseerde gegevens (om de privacy van patiënten te respecteren in overeenstemming met toepasselijke wetten en regelgeving) en met de informatie die nodig is om de onderzoeksdoelstellingen te bereiken onder de voorwaarden van een data-delingsovereenkomst.
