Um Estudo do TAK-226 para Anemia Dependente de Transfusão em Pacientes Japoneses com Síndromes Mielodisplásicas de Baixo Risco
Um Estudo de Fase 2, Multicêntrico, Aberto, de Braço Único para Avaliar a Eficácia e Segurança do TAK-226 para Anemia Dependente de Transfusão em Doentes Japoneses com Síndromes Mielodisplásicas de Risco Muito Baixo, Baixo ou Intermédio
O principal objetivo do estudo é avaliar como o TAK-226 melhora os sintomas de anemia dependente de transfusão em pacientes japoneses com síndromes mielodisplásicas de baixo risco.
O estudo consiste em Período de Triagem (até 6 semanas), Período de Tratamento, Período de Seguimento de Segurança (8 semanas) e Período de Seguimento a Longo Prazo (5 anos a partir da primeira dose do medicamento do estudo ou 3 anos após a última dose, o que for mais longo).
Os participantes deste estudo receberão TAK-226 durante o Período de Tratamento. Posteriormente, os participantes serão monitorizados quanto a efeitos secundários relacionados com o tratamento do estudo durante o Período de Seguimento de Segurança e o Período de Seguimento a Longo Prazo. A duração aproximada da participação de um participante é de até aproximadamente 6 anos.
Durante o período do estudo, os participantes visitarão a clínica/hospital do estudo várias vezes, de acordo com o calendário do estudo. Durante o Período de Tratamento, os participantes irão à clínica/hospital aproximadamente a cada duas a quatro semanas.

Critérios de Inclusão:
- Os participantes ou o seu representante legal autorizado devem estar dispostos e ser capazes de assinar o TCLE e de aderir aos requisitos do protocolo.
- Participante adulto japonês, do sexo masculino ou feminino, com idade ≥18 anos no momento da assinatura do consentimento informado.
Diagnóstico de SMD com ou sem AS (conforme determinado numa aspirado de medula óssea avaliável recolhido no Rastreio para confirmar o diagnóstico) de acordo com a classificação da OMS 2016, que preencha a classificação IPSS-R de SMD de risco muito baixo, baixo ou intermédio.
Nota: Devido ao impacto esperado da transfusão, os valores de Hgb de amostras de sangue recolhidas no prazo de 14 dias após uma transfusão de glóbulos vermelhos e a contagem de plaquetas obtida no prazo de 7 dias após uma transfusão de plaquetas não podem ser utilizados para avaliar o IPSS-R para elegibilidade.
Dependência transfusional avaliada nas 16 semanas imediatamente anteriores à inscrição, em dois blocos de 8 semanas, classificada como:
a. BTB, definida como 4 a 7 unidades de glóbulos vermelhos por 16 semanas; ou b. ATB, definida como ≥8 unidades de glóbulos vermelhos por 16 semanas; e c. Para todos os participantes: i. Apenas eventos de transfusão com Hgb pré-transfusão <10 g/dL são contabilizados para elegibilidade; ii. Pelo menos 1 evento de transfusão em cada bloco de 8 semanas e um mínimo de 2 eventos de transfusão separados por ≥7 dias no período de 16 semanas imediatamente anterior à inscrição; e iii. Nenhum período consecutivo de 56 dias pode estar livre de transfusões de glóbulos vermelhos durante o período de 16 semanas imediatamente anterior à inscrição.
Nota: Apenas as transfusões para a doença em estudo serão contabilizadas para a classificação de participantes BTB ou ATB. Transfusões para doenças intercorrentes (hemorragia, procedimento cirúrgico, infeção, etc.) não são consideradas.
Refratário ou intolerante a tratamento prévio com ESA (interrompido ≥4 semanas antes da inscrição), ou com baixa probabilidade de resposta ao tratamento com ESA, definido como segue:
a. Refratário a tratamento prévio com ESA: documentação de não resposta ou de uma resposta que deixou de ser mantida com um regime prévio contendo ESA, seja como agente único ou em combinação (por exemplo, com fator estimulador de colónias de granulócitos [G-CSF]); o regime de ESA deve ter sido: i. EPO humana recombinante ≥40.000 UI/semana durante ≥8 doses ou equivalente; ou ii. Darbepoetina alfa ≥500 mcrg a cada 3 semanas durante ≥4 doses ou equivalente. b. Intolerante a tratamento prévio com ESA: documentação de descontinuação de um regime prévio contendo ESA, seja como agente único ou em combinação (por exemplo, com G-CSF), a qualquer momento após a introdução devido a intolerância ou a um EA.
c. Baixa probabilidade de resposta ao tratamento com ESA: baixa probabilidade de resposta ao ESA com base num nível endógeno de EPO sérico >200 U/L.
Nota: Devido ao impacto esperado da transfusão nos níveis de EPO, amostras de sangue recolhidas no prazo de 14 dias após uma transfusão de glóbulos vermelhos ou no prazo de 7 dias após uma transfusão de plaquetas não podem ser utilizadas para avaliar o nível de EPO sérico para elegibilidade.
- Menos de 5% de blastos num aspirado de medula óssea avaliável recolhido no Rastreio.
- Estado funcional ECOG de 0 a 2.
Mulheres em idade fértil (MIF), definidas como uma mulher sexualmente madura que não tenha sido submetida a esterilização cirúrgica ou que não tenha estado naturalmente na menopausa há pelo menos 12 meses consecutivos, devem:
- Concordar em utilizar 1 método altamente eficaz de contraceção e 1 método adicional eficaz (de barreira) em simultâneo, desde o momento da assinatura do consentimento informado até 60 dias após a última dose do medicamento em estudo; ou
- Concordar em praticar a abstinência verdadeira, quando esta estiver de acordo com o estilo de vida preferido e habitual do participante. (A abstinência periódica [por exemplo, métodos de calendário, ovulação, sintotermal, pós-ovulação], coito interrompido, apenas espermicidas e amenorreia lactacional não são métodos aceitáveis de contraceção.)
Participantes do sexo masculino devem, mesmo que tenham sido submetidos a esterilização cirúrgica (ou seja, estado pós-vasectomia):
- Concordar em praticar contraceção de barreira eficaz desde o momento da assinatura do consentimento informado até 60 dias após a última dose do medicamento em estudo; ou
- Concordar em praticar a abstinência verdadeira, quando esta estiver de acordo com o estilo de vida preferido e habitual do participante. (A abstinência periódica [por exemplo, métodos de calendário, ovulação, sintotermal, pós-ovulação], coito interrompido, apenas espermicidas e amenorreia lactacional não são métodos aceitáveis de contraceção.)
Critérios de Exclusão:
Historial Médico
- SMD del(5q) ou SMD relacionado com terapia (secundário).
- Anemia devido a qualquer outra causa conhecida (por exemplo, talassemia, anemia hemolítica, eventos hemorrágicos ou deficiência de ferro, B12 e/ou folato).
- Receção de transfusão de glóbulos vermelhos por qualquer motivo(s) que não a SMD subjacente, no prazo de 16 semanas antes da inscrição.
Doença cardiovascular clinicamente significativa definida como:
- Doença cardíaca classe III ou IV da Associação de Cardiologia de Nova Iorque;
- Intervalo QT corrigido de Fridericia (QTcF) >500 milissegundos durante o Rastreio;
- Presença de hipertensão não controlada definida como pressão arterial sistólica média ≥160 mm Hg ou pressão arterial diastólica ≥100 mm Hg durante o Rastreio; ou
- Arritmia não controlada, enfarte do miocárdio ou angina instável no prazo de 6 meses antes do Rastreio.
- Fracção de ejecção conhecida <35%, confirmada por um ecocardiograma local realizado durante o Rastreio, ou por um ecocardiograma previamente realizado se recolhido no prazo de 6 meses antes do Rastreio.
- Acidente vascular cerebral, trombose venosa profunda ou embolia pulmonar no prazo de 6 meses antes do Rastreio.
- Qualquer historial conhecido de leucemia mieloide aguda (LMA).
Historial prévio de neoplasias malignas, para além do SMD, a menos que o participante esteja livre da doença (incluindo a conclusão de qualquer tratamento, incluindo manutenção, para neoplasia maligna prévia) há ≥5 anos. No entanto, participantes com historial ou diagnóstico simultâneo das seguintes condições são permitidos se não requererem terapia sistémica:
- Carcinoma basocelular ou espinocelular da pele;
- Carcinoma in situ do colo do útero;
- Carcinoma in situ da mama; e/ou
- Achado histológico incidental de cancro da próstata (T1a ou T1b utilizando o sistema de estadiamento clínico tumor, nódulo, metastase [TNM]).
- Historial de transplante de órgão sólido ou de medula óssea.
- Infeção ativa que requeira antibióticos intravenosos no prazo de 28 dias ou antibióticos orais no prazo de 14 dias antes da inscrição.
- Historial de ou infeção ativa ou crónica conhecida com VIH, vírus da hepatite B (VHB) infecioso ativo ou vírus da hepatite C (VHC) infecioso ativo. Participantes que sejam positivos para o antigénio de superfície da hepatite B (HBsAg), anticorpo do núcleo da hepatite B (HBcAb) ou anticorpo de superfície da hepatite B (HBsAb) podem ser elegíveis para inscrição se a sua carga viral da hepatite B estiver abaixo do limite de deteção. Participantes que sejam positivos para anticorpos do vírus da hepatite C (VHCAb) podem ser inscritos se a sua carga viral da hepatite C estiver abaixo do limite de deteção.
- Índice de massa corporal ≥40 kg/m^2.
- Cirurgia maior no prazo de 28 dias antes da inscrição.
Historial de alergia/anafilaxia a excipientes do medicamento em investigação (MEI) (consultar o IB atual do elritercept [TAK 226] para uma lista de excipientes) ou a proteínas recombinantes.
Historial de Tratamento
- Utilização prévia de TAK 226, luspatercept, imetelstat ou sotatercept.
- Utilização prévia de agentes hipometilantes (AHM), inibidor da isocitrato desidrogenase, lenalidomida ou terapia imunossupressora administrada para tratamento de SMD.
- Terapia de quelação de ferro iniciada no prazo de 8 semanas antes da inscrição. Participantes com doses estáveis de terapia de quelação de ferro há ≥8 semanas são permitidos.
- Terapia com vitamina B12 ou folato iniciada no prazo de 4 semanas antes da inscrição. Participantes com doses de substituição estáveis há ≥4 semanas e sem deficiência concomitante em curso de vitamina B12 ou folato são permitidos.
- Utilização de androgénios no prazo de 8 semanas antes da inscrição. Participantes com dosagem estável de androgénios para hipogonadismo há ≥8 semanas são permitidos.
- Utilização de corticosteroides em dose elevada no prazo de 4 semanas antes da inscrição. Participantes com doses crónicas estáveis de prednisona/prednisolona ≤10 mg/dia ou equivalente de corticosteroides há ≥4 semanas são permitidos.
- Tratamento com qualquer medicamento em investigação no prazo de 28 dias antes do Rastreio ou, se a meia-vida do produto for conhecida, no prazo de 5 vezes a meia-vida antes do Rastreio, o que for mais longo.
- Participação em curso noutro estudo clínico interventivo. Exclusões Laboratoriais (durante o Rastreio)
- Nível de EPO sérico >500 U/L. Nota: Devido ao impacto esperado da transfusão nos níveis de EPO, resultados laboratoriais de amostras de sangue recolhidas no prazo de 14 dias após uma transfusão de glóbulos vermelhos não podem ser utilizados para avaliar o nível de EPO sérico para elegibilidade.
- Contagem de plaquetas ≥450×10^3/mcrL ou ≤25×10^3/mcrL. Nota: Devido ao impacto esperado da transfusão, resultados laboratoriais de amostras de sangue recolhidas no prazo de 7 dias após uma transfusão de plaquetas não podem ser utilizados para avaliar a contagem de plaquetas para elegibilidade.
- Contagem absoluta de neutrófilos ≤500/mcrL
- AST ou ALT sérica ≥3×o limite superior do normal (LSN).
- Bilirrubina total ≥2×LSN, a menos que atribuível à síndrome de Gilbert.
- Ferritina ≤50 mcrg/L.
- Folato ≤2,0 ng/mL.
- Vitamina B12 ≤200 pg/mL.
Taxa de filtração glomerular estimada <30 mL/min/1,73m^2 conforme determinado pela Sociedade Japonesa de Nefrologia. Calculada pela fórmula de correção para japoneses. - Mulher grávida ou a amamentar. Nota: Participantes que possam estar num estágio muito inicial de gravidez com base na entrevista do médico com um teste de gravidez negativo são excluídos do estudo. Participantes que estão a amamentar serão elegíveis se descontinuarem a amamentação desde antes da primeira dose do medicamento em estudo até 60 dias após a última dose do medicamento em estudo.
- Qualquer outra condição não especificamente mencionada acima que, na opinião do investigador, impeça a participação do participante no estudo ou possa confundir a interpretação dos dados do estudo.
- Membros da equipa do local de investigação diretamente envolvidos na condução do estudo e membros da equipa do local de outra forma supervisionados pelo investigador, funcionários do Patrocinador ou organização de pesquisa por contrato (ORC) diretamente envolvidos na condução do estudo, ou familiares imediatos (definidos como cônjuge, pai, filho ou irmão, seja biológico ou legalmente adotado).
