Studio di Fase 2, Multicentrico, in Aperto, a Braccio Singolo per Valutare l'Efficacia e la Sicurezza di TAK-226 per l'Anemia Dipendente da Trasfusione in Pazienti Giapponesi con Sindromi Mielodisplastiche a Rischio Molto Basso, Basso o Intermedio

L'obiettivo principale dello studio è valutare come TAK-226 migliora i sintomi dell'anemia trasfusione-dipendente nei pazienti giapponesi con sindromi mielodisplastiche a basso rischio.

Lo studio è composto dal Periodo di Screening (fino a 6 settimane), Periodo di Trattamento, Periodo di Follow-Up di Sicurezza (8 settimane) e Periodo di Follow-Up a Lungo Termine (5 anni dalla prima dose del farmaco in studio o 3 anni dopo l'ultima dose, a seconda di quale sia più lungo).

I partecipanti a questo studio riceveranno TAK-226 durante il Periodo di Trattamento. Successivamente, i partecipanti verranno monitorati per gli effetti collaterali correlati al trattamento in studio durante il Periodo di Follow-Up di Sicurezza e il Periodo di Follow-Up a Lungo Termine. Criteri di inclusione:

  1. I partecipanti o il loro rappresentante legale autorizzato devono essere disposti e in grado di firmare il consenso informato e di aderire ai requisiti del protocollo.
  2. Partecipante adulto giapponese di sesso maschile o femminile di età ≥18 anni al momento della firma del consenso informato.
  3. Diagnosi di MDS con o senza RS (come determinato in un aspirato midollare valutabile raccolto durante lo Screening per confermare la diagnosi) secondo la classificazione WHO 2016 che soddisfa la classificazione IPSS-R di MDS a rischio molto basso, basso o intermedio.

    Nota: a causa degli impatti previsti della trasfusione, i valori di emoglobina da campioni di sangue raccolti entro 14 giorni da una trasfusione di globuli rossi e la conta piastrinica ottenuta entro 7 giorni da una trasfusione di piastrine non possono essere utilizzati per valutare l'IPSS-R per l'idoneità.

  4. Dipendenza trasfusionale valutata nelle 16 settimane immediatamente precedenti l'arruolamento in due blocchi di 8 settimane classificati come:

    a. LTB, definita come 4-7 unità di globuli rossi per 16 settimane; o b. HTB, definita come ≥8 unità di globuli rossi per 16 settimane; e c. Per tutti i partecipanti: i. Solo gli eventi trasfusionali per un'emoglobina pre-trasfusione <10 g/dL sono conteggiati per l'idoneità; ii. Almeno 1 evento trasfusionale in ogni blocco di 8 settimane e un minimo di 2 eventi trasfusionali separati da ≥7 giorni nel periodo di 16 settimane immediatamente precedente l'arruolamento; e iii. Nessun periodo consecutivo di 56 giorni può essere privo di trasfusioni di globuli rossi durante il periodo di 16 settimane immediatamente precedente l'arruolamento.

    Nota: Solo le trasfusioni per la malattia in studio saranno conteggiate per la classificazione per i partecipanti LTB o HTB. Le trasfusioni per malattie intercorrenti (sanguinamento, procedura chirurgica, infezione, ecc.) non sono considerate.

  5. Refrattarietà o intolleranza a un precedente trattamento con ESA (sospeso ≥4 settimane prima dell'arruolamento), o improbabilità di rispondere al trattamento con ESA, definita come segue:

    a. Refrattarietà a un precedente trattamento con ESA: documentazione di mancata risposta o di una risposta non più mantenuta con un precedente regime contenente ESA, come agente singolo o in combinazione (es. con fattore stimolante le colonie di granulociti [G-CSF]); il regime ESA deve essere stato: i. EPO umana ricombinante ≥40.000 UI/settimana per ≥8 dosi o equivalente; o ii. Darbepoetina alfa ≥500 mcg ogni 3 settimane per ≥4 dosi o equivalente. b. Intolleranza a un precedente trattamento con ESA: documentazione dell'interruzione di un precedente regime contenente ESA, come agente singolo o in combinazione (es. con G-CSF), in qualsiasi momento dopo l'introduzione a causa di intolleranza o di un evento avverso. c. Improbabilità di rispondere al trattamento con ESA: bassa probabilità di risposta all'ESA basata su un livello di EPO sierica endogena >200 U/L.

    Nota: a causa degli impatti previsti della trasfusione sui livelli di EPO, i campioni di sangue raccolti entro 14 giorni da una trasfusione di globuli rossi o entro 7 giorni da una trasfusione di piastrine non possono essere utilizzati per valutare il livello di EPO sierica per l'idoneità.

  6. Meno del 5% di blasti in un aspirato midollare valutabile raccolto durante lo Screening.
  7. Stato di performance ECOG da 0 a 2.
  8. Donne in età fertile (WOCBP), definite come donne sessualmente mature che non hanno subito sterilizzazione chirurgica o che non sono state naturalmente in post-menopausa per almeno 12 mesi consecutivi devono:

    1. Accettare di utilizzare 1 metodo contraccettivo altamente efficace e 1 metodo efficace aggiuntivo (barriera) contemporaneamente, dal momento della firma del consenso informato fino a 60 giorni dopo l'ultima dose del farmaco in studio; o
    2. Accettare di praticare la vera astinenza, quando ciò è in linea con lo stile di vita preferito e abituale del partecipante. (L'astinenza periodica [es. calendario, ovulazione, sintotermale, metodi post-ovulazione], coito interrotto, solo spermicidi e amenorrea da allattamento non sono metodi contraccettivi accettabili.)
  9. I partecipanti maschi devono, anche se chirurgicamente sterilizzati (cioè, status post-vasectomia):

    1. Accettare di praticare un'efficace contraccezione a barriera dal momento della firma del consenso informato fino a 60 giorni dopo l'ultima dose del farmaco in studio; o

Criteri di esclusione:

Anamnesi Medica

  1. MDS con del(5q) o MDS terapia-correlata (secondaria).
  2. Anemia dovuta a qualsiasi altra causa nota (es. talassemia, anemia emolitica, eventi emorragici o carenza di ferro, B12 e/o folati).
  3. Ricezione di trasfusione di globuli rossi per qualsiasi motivo diverso dall'MDS sottostante entro 16 settimane prima dell'arruolamento.
  4. Malattia cardiovascolare clinicamente significativa definita come:

    1. Malattia cardiaca di classe III o IV secondo la New York Heart Association;
    2. Intervallo QT corretto di Fridericia (QTcF) >500 millisecondi durante lo Screening;
    3. Presenza di ipertensione non controllata definita come pressione arteriosa sistolica media ≥160 mm Hg o pressione arteriosa diastolica ≥100 mm Hg durante lo Screening; o
    4. Aritmia non controllata, infarto miocardico o angina instabile entro 6 mesi prima dello Screening.
  5. Frazione di eiezione nota <35%, confermata da un ecocardiogramma locale eseguito durante lo Screening, o da un ecocardiogramma precedentemente eseguito se raccolto entro 6 mesi prima dello Screening.
  6. Ictus, trombosi venosa profonda o embolia polmonare entro 6 mesi prima dello Screening.
  7. Qualsiasi anamnesi nota di leucemia mieloide acuta (AML).
  8. Anamnesi precedente di neoplasie maligne, diverse dall'MDS, a meno che il partecipante non sia stato libero dalla malattia (incluso il completamento di qualsiasi trattamento, inclusa la terapia di mantenimento, per la neoplasia precedente) per ≥5 anni.   9. Anamnesi di trapianto di organi solidi o midollo osseo.
  10. Infezione attiva che richiede antibiotici per via endovenosa entro 28 giorni o antibiotici per via orale entro 14 giorni prima dell'arruolamento.
  11. Anamnesi o infezione nota attiva o cronica con HIV, virus dell'epatite B (HBV) infettivo attivo o virus dell'epatite C (HCV) infettivo attivo. I partecipanti positivi all'antigene di superficie dell'epatite B (HBsAg), all'anticorpo anti-core dell'epatite B (HBcAb) o all'anticorpo di superficie dell'epatite B (HBsAb) possono essere idonei all'arruolamento se la loro carica virale dell'epatite B è al di sotto del limite di rilevamento.   15. Uso precedente di TAK 226, luspatercept, imetelstat o sotatercept.
  16. Uso precedente di agenti ipometilanti (HMA), inibitore dell'isocitrato deidrogenasi, lenalidomide o terapia immunosoppressiva somministrata per il trattamento dell'MDS.
  17. Terapia chelante del ferro iniziata entro 8 settimane prima dell'arruolamento. Sono ammessi partecipanti in dosi stabili di terapia chelante del ferro per ≥8 settimane.
  18. Terapia con vitamina B12 o folati iniziata entro 4 settimane prima dell'arruolamento. Sono ammessi partecipanti in dosi di sostituzione stabili per ≥4 settimane e senza carenza concomitante in corso di vitamina B12 o folati.
  19. Uso di androgeni entro 8 settimane prima dell'arruolamento. Sono ammessi partecipanti in dosi stabili di androgeni per ipogonadismo per ≥8 settimane.
  20. Uso di corticosteroidi ad alte dosi entro 4 settimane prima dell'arruolamento. Sono ammessi partecipanti in dosi croniche stabili di prednisone/prednisolone ≤10 mg/giorno o equivalente di corticosteroidi per ≥4 settimane.
  21. Trattamento con qualsiasi farmaco sperimentale entro 28 giorni prima dello Screening o, se l'emivita del prodotto è nota, entro 5 volte l'emivita prima dello Screening, a seconda di quale sia più lungo.
  22. Partecipazione in corso a un altro studio clinico interventistico. Esclusioni di Laboratorio (durante lo Screening)
  23. Livello di EPO sierica >500 U/L.
