Hovedmålet med studien er å evaluere hvordan TAK-226 forbedrer symptomer på transfusjonsavhengig anemi hos japanske pasienter med myelodysplastiske syndromer med lavere risiko.

Studien består av screeningsperiode (opptil 6 uker), behandlingsperiode, sikkerhetsoppfølgingsperiode (8 uker) og langtidsoppfølgingsperiode (5 år fra første dose av studiemedikamentet eller 3 år etter siste dose, avhengig av hva som er lengst).

Deltakere i denne studien vil få TAK-226 under behandlingsperioden. Deretter vil deltakerne bli overvåket for bivirkninger knyttet til studiebehandlingen under sikkerhetsoppfølgingsperioden og langtidsoppfølgingsperioden. Den omtrentlige deltakelsesvarigheten for en deltaker er opptil omtrent 6 år.

I løpet av studieperioden vil deltakerne besøke studiepoliklinikken/sykehuset flere ganger i henhold til studiens tidsplan. Under behandlingsperioden vil deltakerne komme til poliklinikken/sykehuset omtrent hver andre til fjerde uke.

Inklusjonskriterier:

  1. Deltakere eller deres lovlige representant må være villige og i stand til å signere informert samtykkeerklæring (ICF) og følge protokollkravene.
  2. Japanske voksne mannlige eller kvinnelige deltakere ≥18 år ved tidspunktet for signering av informert samtykke.
  3. Diagnose av myelodysplastisk syndrom (MDS) med eller uten ringsideroblast (RS) (som fastsatt i en vurderbar beinmargsaspirat tatt ved screening for å bekrefte diagnosen) i henhold til WHO 2016-klassifisering som oppfyller IPSS-R-klassifiseringen av svært lav-, lav- eller intermediær risiko MDS.

    Merknad: På grunn av forventede effekter av transfusjon kan ikke Hgb-verdier fra blodprøver tatt innen 14 dager etter en erytrocyttransfusjon og blodplatelettellinger oppnådd innen 7 dager etter en blodplatelettransfusjon brukes til å evaluere IPSS-R for opptakskvalifisering.

  4. Transfusjonsavhengighet vurdert i de 16 ukene umiddelbart før inkludering i to 8-ukers blokker klassifisert som enten:

    Merknad: Kun transfusjoner for den underliggende sykdommen som studeres vil telles med for klassifisering for LTB- eller HTB-deltakere. Transfusjoner for interkurrente sykdommer (blødning, kirurgisk inngrep, infeksjon, etc.) regnes ikke med.

  5. Refraktær eller intolerant til tidligere erytropoese-stimulerende middel (ESA)-behandling (avsluttet ≥4 uker før inkludering), eller usannsynlig å respondere på ESA-behandling, definert som følger:

    a. Eksklusjonskriterier:

Medisinsk historikk

  1. Del(5q) MDS eller terapi-relatert (sekundær) MDS.
  2. Anemi på grunn av andre kjente årsaker (f.eks. thalassemi, hemolytisk anemi, blødningshendelser eller mangel på jern, B12 og/eller folat).
  3. Mottak av erytrocyttransfusjon av andre årsaker enn underliggende MDS innen 16 uker før inkludering.
  4. Klinisk signifikant kardiovaskulær sykdom definert som:

    1. New York Heart Association hjerteklasse III eller IV;
    2. Fridericia korrigert QT (QTcF) intervall >500 millisekunder under screening;
    3. Tilstedeværelse av ukontrollert hypertensjon definert som gjennomsnittlig systolisk blodtrykk ≥160 mm Hg eller diastolisk blodtrykk ≥100 mm Hg under screening; eller
    4. Ukontrollert arytmi, hjerteinfarkt eller ustabil angina pectoris innen 6 måneder før screening.
  5. Kjent ejeksjonsfraksjon <35%, bekreftet av en lokal ekkokardiografi utført under screening, eller en tidligere utført ekkokardiografi hvis tatt innen 6 måneder før screening.
  6. Slag, dyp venetrombose eller lungeemboli innen 6 måneder før screening.
  7. Kjent historikk med akutt myelogen leukemi (AML).
  8. Tidligere historikk med maligniteter, annet enn MDS, med mindre deltakeren har vært fri for sykdommen (inkludert fullføring av enhver behandling, inkludert vedlikeholdsbehandling, for tidligere malignitet) i ≥5 år. Imidlertid er deltakere med historikk eller samtidig diagnose av følgende tilstander tillatt dersom de ikke krever systemisk terapi:

    1. Basal- eller spinocellulært karsinom i huden;
    2. Carcinoma in situ i livmorhalsen;
    3. Carcinoma in situ i brystet; og/eller
    4. Incidentell histologisk funn av prostatakreft (T1a eller T1b ved bruk av tumor, node, metastase [TNM] klinisk stadieringssystem).
  9. Historikk med transplantasjon av fast organ eller beinmarg.
  10. Aktiv infeksjon som krever intravenøse antibiotika innen 28 dager eller orale antibiotika innen 14 dager før inkludering.

  15. Tidligere bruk av TAK 226, luspatercept, imetelstat eller sotatercept.
  16. Tidligere bruk av hypometylerende midler (HMA), isocitratdehydrogenasehemmer, lenalidomid eller immunosuppressiv terapi gitt for behandling av MDS.
  17. Jernkelasjonsterapi startet innen 8 uker før inkludering. Deltakere på stabile doser av jernkelasjonsterapi i ≥8 uker er tillatt.
  18. Vitamin B12- eller folatterapi startet innen 4 uker før inkludering. Deltakere på stabile erstatningsdoser i ≥4 uker og uten pågående samtidig vitamin B12- eller folatmangel er tillatt.
  19. Androgenbruk innen 8 uker før inkludering. Deltakere på stabil androgendosering for hypogonadisme i ≥8 uker er tillatt.
  20. Høy-dose kortikosteroidbruk innen 4 uker før inkludering. Deltakere på stabile kroniske steroiddoser av prednison/prednisolon ≤10 mg/dag eller kortikosteroidekvivalent i ≥4 uker er tillatt.
  21. Behandling med ethvert undersøkelsesmedikament innen 28 dager før screening eller, hvis halveringstiden til produktet er kjent, innen 5 ganger halveringstiden før screening, avhengig av hva som er lengst.
