Badanie TAK-226 w leczeniu niedokrwistości zależnej od transfuzji u japońskich pacjentów z zespołami mielodysplastycznymi o mniejszym ryzyku
Faza 2, wieloośrodkowe, otwarte, jednoramienne badanie oceniające skuteczność i bezpieczeństwo TAK-226 w leczeniu anemii zależnej od transfuzji u japońskich pacjentów z zespołami mielodysplastycznymi o bardzo niskim, niskim lub pośrednim ryzyku
Głównym celem badania jest ocena, w jaki sposób TAK-226 łagodzi objawy niedokrwistości zależnej od transfuzji u japońskich pacjentów z zespołami mielodysplastycznymi niższego ryzyka.
Badanie składa się z okresu przesiewowego (do 6 tygodni), okresu leczenia, okresu obserwacji bezpieczeństwa (8 tygodni) oraz długoterminowego okresu obserwacji (5 lat od pierwszej dawki leku badawczego lub 3 lata po ostatniej dawce, w zależności od tego, który okres jest dłuższy).
Uczestnicy tego badania będą otrzymywać TAK-226 w okresie leczenia. Następnie uczestnicy będą monitorowani pod kątem działań niepożądanych związanych z leczeniem badawczym w okresie obserwacji bezpieczeństwa i długoterminowym okresie obserwacji. Przybliżony czas trwania udziału dla uczestnika wynosi do około 6 lat.
W trakcie trwania badania uczestnicy będą wielokrotnie odwiedzać klinikę/szpital badawczy zgodnie z harmonogramem badania. W okresie leczenia uczestnicy będą przychodzić do kliniki/szpitala mniej więcej co dwa do czterech tygodni.

Kryteria włączenia:
- Uczestnicy lub ich prawnie upoważnieni przedstawiciele muszą być chętni i zdolni do podpisania świadomej zgody (ICF) oraz przestrzegania wymagań protokołu.
- Japoński dorosły uczestnik płci męskiej lub żeńskiej w wieku ≥18 lat w momencie podpisania świadomej zgody.
Rozpoznanie MDS z lub bez RS (ustalone na podstawie ocenialnej aspiracji szpiku kostnego pobranej podczas badania przesiewowego w celu potwierdzenia diagnozy) zgodnie z klasyfikacją WHO 2016, spełniające klasyfikację IPSS-R MDS o bardzo niskim, niskim lub pośrednim ryzyku.
Uwaga: Ze względu na oczekiwany wpływ transfuzji, wartości hemoglobiny (Hgb) z próbek krwi pobranych w ciągu 14 dni po transfuzji krwinek czerwonych oraz liczba płytek krwi uzyskana w ciągu 7 dni po transfuzji płytek nie mogą być używane do oceny IPSS-R w celu kwalifikacji.
Uzależnienie od transfuzji ocenione w ciągu 16 tygodni bezpośrednio poprzedzających rejestrację, w dwóch 8-tygodniowych blokach, sklasyfikowane jako:
a. Niskie zapotrzebowanie na transfuzję (LTB), zdefiniowane jako 4 do 7 jednostek krwinek czerwonych na 16 tygodni; lub b.
Wysokie zapotrzebowanie na transfuzję (HTB), zdefiniowane jako ≥8 jednostek krwinek czerwonych na 16 tygodni; oraz c.
Dla wszystkich uczestników: i.
Do kwalifikacji liczone są tylko zdarzenia transfuzji, gdy Hgb przed transfuzją wynosiła <10 g/dL; ii.
Co najmniej 1 zdarzenie transfuzji w każdym 8-tygodniowym bloku i minimum 2 zdarzenia transfuzji oddzielone o ≥7 dni w ciągu 16-tygodniowego okresu bezpośrednio poprzedzającego rejestrację; oraz iii.
W ciągu 16-tygodniowego okresu bezpośrednio poprzedzającego rejestrację nie może wystąpić żaden kolejny 56-dniowy okres bez transfuzji krwinek czerwonych.Uwaga: Do klasyfikacji uczestników LTB lub HTB liczone są tylko transfuzje związane z badaną chorobą.
Transfuzje z powodu chorób współistniejących (krwawienie, zabieg chirurgiczny, infekcja itp.) nie są brane pod uwagę.Oporność lub nietolerancja na wcześniejsze leczenie ESA (przerwane ≥4 tygodnie przed rejestracją) lub małe prawdopodobieństwo odpowiedzi na leczenie ESA, zdefiniowane następująco:
a. Oporność na wcześniejsze leczenie ESA: dokumentacja braku odpowiedzi lub odpowiedzi, która nie była już utrzymywana przy wcześniejszym schemacie leczenia zawierającym ESA, jako monoterapia lub w skojarzeniu (np. z czynnikiem stymulującym tworzenie kolonii granulocytów [G-CSF]); schemat leczenia ESA musiał być: i.
Rekombinowana ludzka EPO ≥40 000 IU/tydzień przez ≥8 dawek lub odpowiednik; lub ii.
Darbepoetyna alfa ≥500 µg co 3 tygodnie przez ≥4 dawek lub odpowiednik.

Nietolerancja na wcześniejsze leczenie ESA: dokumentacja przerwania wcześniejszego schematu leczenia zawierającego ESA, jako monoterapia lub w skojarzeniu (np. z G-CSF), w dowolnym momencie po wprowadzeniu z powodu nietolerancji lub działania niepożądanego (AE).c. Małe prawdopodobieństwo odpowiedzi na leczenie ESA: niska szansa na odpowiedź na ESA na podstawie endogennego poziomu EPO w surowicy >200 U/L.
Uwaga: Ze względu na oczekiwany wpływ transfuzji na poziomy EPO, próbki krwi pobrane w ciągu 14 dni po transfuzji krwinek czerwonych lub w ciągu 7 dni po transfuzji płytek nie mogą być używane do oceny poziomu EPO w surowicy w celu kwalifikacji.
- Mniej niż 5% blastów w ocenialnej aspiracji szpiku kostnego pobranej podczas badania przesiewowego.
- Stan sprawności wg ECOG od 0 do 2.
Kobiety w wieku rozrodczym (WOCBP), zdefiniowane jako dojrzałe seksualnie kobiety, które nie przeszły sterylizacji chirurgicznej lub nie były naturalnie w stanie pomenopauzalnym przez co najmniej 12 kolejnych miesięcy, muszą:
- Zgodzić się na stosowanie 1 wysoce skutecznej metody antykoncepcji i 1 dodatkowej skutecznej metody (barierowej) jednocześnie, od momentu podpisania świadomej zgody do 60 dni po ostatniej dawce leku badawczego; lub
- Zgodzić się na praktykowanie prawdziwej abstynencji, gdy jest to zgodne z preferowanym i zwykłym stylem życia uczestnika.
(Okresowa abstynencja [np. kalendarzowa, owulacyjna, objawowo-termiczna, metody poowulacyjne], stosunek przerywany, same środki plemnikobójcze oraz brak miesiączki laktacyjny nie są akceptowalnymi metodami antykoncepcji.)
Uczestnicy płci męskiej muszą, nawet jeśli są chirurgicznie wysterylizowani (tj. po wazektomii):
- Zgodzić się na stosowanie skutecznej antykoncepcji barierowej od momentu podpisania świadomej zgody do 60 dni po ostatniej dawce leku badawczego; lub
- Zgodzić się na praktykowanie prawdziwej abstynencji, gdy jest to zgodne z preferowanym i zwykłym stylem życia uczestnika.
(Okresowa abstynencja [np. kalendarzowa, owulacyjna, objawowo-termiczna, metody poowulacyjne], stosunek przerywany, same środki plemnikobójcze oraz brak miesiączki laktacyjny nie są akceptowalnymi metodami antykoncepcji.)
Kryteria wykluczenia:
Wywiad medyczny
- MDS z del(5q) lub MDS związany z leczeniem (wtórny).
- Niedokrwistość z powodu jakiejkolwiek innej znanej przyczyny (np. talasemia, niedokrwistość hemolityczna, zdarzenia krwotoczne lub niedobór żelaza, B12 i/lub kwasu foliowego).
- Otrzymanie transfuzji krwinek czerwonych z jakiegokolwiek innego powodu niż podstawowe MDS w ciągu 16 tygodni przed rejestracją.
Klinicznie istotna choroba sercowo-naczyniowa zdefiniowana jako:
- Choroba serca wg Nowojorskiego Towarzystwa Kardiologicznego (NYHA) w klasie III lub IV;
- Poprawiony odstęp QT metodą Fridericii (QTcF) >500 milisekund podczas badania przesiewowego;
- Obecność niekontrolowanego nadciśnienia tętniczego zdefiniowanego jako średnie ciśnienie skurczowe ≥160 mm Hg lub ciśnienie rozkurczowe ≥100 mm Hg podczas badania przesiewowego; lub
- Niekontrolowana arytmia, zawał mięśnia sercowego lub niestabilna dławica piersiowa w ciągu 6 miesięcy przed badaniem przesiewowym.
- Znana frakcja wyrzutowa <35%, potwierdzona lokalnym badaniem echokardiograficznym wykonanym podczas badania przesiewowego lub wcześniej wykonanym badaniem echokardiograficznym, jeśli zostało przeprowadzone w ciągu 6 miesięcy przed badaniem przesiewowym.
- Udar, zakrzepica żył głębokich lub zatorowość płucna w ciągu 6 miesięcy przed badaniem przesiewowym.
- Jakakolwiek znana historia ostrej białaczki szpikowej (AML).
Wcześniejsza historia nowotworów złośliwych innych niż MDS, chyba że uczestnik jest wolny od choroby (w tym ukończył wszelkie leczenie, w tym leczenie podtrzymujące, z powodu wcześniejszego nowotworu) przez ≥5 lat.
Jednak uczestnicy z historią lub współistniejącym rozpoznaniem następujących schorzeń są dopuszczeni, jeśli nie wymagają leczenia systemowego:- Rak podstawnokomórkowy lub płaskonabłonkowy skóry;
- Rak in situ szyjki macicy;
- Rak in situ piersi; i/lub
- Incydentalne stwierdzenie histologiczne raka prostaty (T1a lub T1b wg klinicznego systemu stopniowania TNM [guz, węzeł, przerzuty]).
- Historia przeszczepienia narządu stałego lub szpiku kostnego.
- Aktywna infekcja wymagająca antybiotyków dożylnych w ciągu 28 dni lub antybiotyków doustnych w ciągu 14 dni przed rejestracją.
- Historia lub znana aktywna lub przewlekła infekcja HIV, aktywna zakaźna wirusem zapalenia wątroby typu B (HBV) lub aktywna zakaźna wirusem zapalenia wątroby typu C (HCV).
Uczestnicy, którzy są pozytywni pod względem antygenu powierzchniowego wirusa zapalenia wątroby typu B (HBsAg), przeciwciał przeciwko rdzeniowi wirusa zapalenia wątroby typu B (HBcAb) lub przeciwciał powierzchniowych wirusa zapalenia wątroby typu B (HBsAb), mogą być uprawnieni do rejestracji, jeśli ich miano wirusa zapalenia wątroby typu B jest poniżej granicy wykrywalności.

Historia alergii/anafilaksji na substancje pomocnicze badanego produktu leczniczego (IMP) (patrz aktualna Charakterystyka Produktu Leczniczego (ChPL) elriterceptu [TAK 226] w celu uzyskania listy substancji pomocniczych) lub białka rekombinowane.
Wywiad dotyczący leczenia
- Wcześniejsze stosowanie TAK 226, luspaterceptu, imetelstatu lub sotaterceptu.
- Wcześniejsze stosowanie środków hipometylujących (HMA), inhibitorów dehydrogenazy izocytrynianowej, lenalidomidu lub terapii immunosupresyjnej stosowanej w leczeniu MDS.
- Terapia chelatująca żelazo rozpoczęta w ciągu 8 tygodni przed rejestracją.
Uczestnicy przyjmujący stabilne dawki terapii chelatującej żelazo przez ≥8 tygodni są dopuszczeni. - Terapia witaminą B12 lub kwasem foliowym rozpoczęta w ciągu 4 tygodni przed rejestracją.
Uczestnicy przyjmujący stabilne dawki zastępcze przez ≥4 tygodnie i bez współistniejącego niedoboru witaminy B12 lub kwasu foliowego są dopuszczeni. - Stosowanie androgenów w ciągu 8 tygodni przed rejestracją.
Uczestnicy przyjmujący stabilne dawki androgenów z powodu hipogonadyzmu przez ≥8 tygodni są dopuszczeni. - Stosowanie wysokich dawek kortykosteroidów w ciągu 4 tygodni przed rejestracją.
Uczestnicy przyjmujący stabilne przewlekłe dawki steroidów (prednizon/prednizolon ≤10 mg/dzień lub odpowiednik kortykosteroidu) przez ≥4 tygodni są dopuszczeni. - Leczenie jakimkolwiek lekiem badawczym w ciągu 28 dni przed badaniem przesiewowym lub, jeśli znany jest okres półtrwania produktu, w ciągu 5 okresów półtrwania przed badaniem przesiewowym, w zależności od tego, który okres jest dłuższy.
- Trwający udział w innym interwencyjnym badaniu klinicznym.
Wykluczenia laboratoryjne (podczas badania przesiewowego) - Poziom EPO w surowicy >500 U/L.
Uwaga: Ze względu na oczekiwany wpływ transfuzji na poziomy EPO, wyniki laboratoryjne z próbek krwi pobranych w ciągu 14 dni po transfuzji krwinek czerwonych nie mogą być używane do oceny poziomu EPO w surowicy w celu kwalifikacji. - Liczba płytek krwi ≥450×10³/µL lub ≤25×10³/µL.
Uwaga: Ze względu na oczekiwany wpływ transfuzji, wyniki laboratoryjne z próbek krwi pobranych w ciągu 7 dni po transfuzji płytek nie mogą być używane do oceny liczby płytek krwi w celu kwalifikacji. - Bezwzględna liczba neutrofili ≤500/µL
- AST lub ALT w surowicy ≥3× górna granica normy (GGN).

An AE is any untoward medical occurrence in a clinical trial participant, temporally associated with the use of the trial intervention, whether or not the occurrence is considered related to the trial intervention.
An AE can therefore be any unfavorable and unintended sign (including an abnormal laboratory finding), symptom, or disease (new or exacerbated) temporally associated with the use of the trial intervention.
A TEAE is defined as an AE that commences on or after the first dose of the study treatment and within 60 days after the last dose of the study treatment, or analysis cutoff date, whichever is earlier.
An SAE is any untoward medical occurrence that, at any dose: results in death, is life threatening, requires in-patient hospitalization or prolongation of existing hospitalization, results in persistent or significant disability/incapacity, is a congenital anomaly/birth defect, is a medically important event.
