L'EFFORT Ultrasound (US) è un sottostudio dello studio principale EFFORT; uno studio clinico multicentrico, pragmatico, guidato da volontari, basato su registro, randomizzato, su 4000 pazienti critici dal punto di vista nutrizionale ad alto rischio nell'unità di terapia intensiva. I pazienti saranno randomizzati a 1 di 2 gruppi di trattamento: una prescrizione di cure abituali (≤1,2 g/kg/die) o una prescrizione più elevata (≥2,2 g/kg/die) di proteine. A parte la quantità di proteine a cui il paziente è randomizzato, il resto dell'assistenza fornita al paziente randomizzato sarà a discrezione dei fornitori di terapia intensiva.
In entrambi i gruppi, gli obiettivi saranno raggiunti attraverso qualsiasi combinazione di nutrizione enterale (alto contenuto proteico nel gruppo alto se disponibile), integratori proteici e nutrizione parenterale o solo aminoacidi (come clinicamente disponibile). L'unica differenza tra i 2 gruppi sono gli obiettivi proteici impostati. Sforzi simili dovrebbero essere usati in entrambi i gruppi per raggiungere almeno l'80% di questi obiettivi. Il resto dell'assistenza fornita ai pazienti idonei sarà a discrezione dei fornitori di terapia intensiva.
I pazienti nel sottostudio statunitense saranno sottoposti alle misure statunitensi al basale (entro 24 ore dalla randomizzazione, 10 giorni dopo la randomizzazione (se ancora in ospedale) e appena prima della dimissione dall'ospedale. Nel caso in cui la dimissione ospedaliera avvenga prima del giorno 10, la misura del giorno 10 non verrà eseguita. Per garantire la standardizzazione e la qualità delle misurazioni, abbiamo creato materiali di formazione di alta qualità e invieremo film statunitensi a livello centrale per astrarre tutte le misurazioni. Nei primi 10 pazienti arruolati nel sottostudio statunitense, i centri partecipanti condurranno una fase di run-in in cui i loro dati inviati saranno valutati per la qualità e l'affidabilità (affidabilità sia intra che inter-valutatore) per garantire che le misure successive siano di alta qualità . I dati nutrizionali e clinici per questi pazienti saranno inclusi nello studio genitore EFFORT, ma le misure statunitensi potrebbero essere omesse se la qualità è scarsa.

Nei pazienti in condizioni critiche con "fattori di rischio" nutrizionali, qual è l'effetto della prescrizione di una dose più alta (≥2,2 grammi/kg/giorno) di somministrazione di proteine/amminoacidi rispetto alla prescrizione di cura abituale (≤1,2 grammi/kg/giorno) sulla massa muscolare e sulla qualità e sulla mortalità a 60 giorni?
L'ipotesi proposta:
Rispetto alla normale cura di proteine/aminoacidi, la somministrazione di una dose maggiore di proteine/aminoacidi (conseguenza di una prescrizione più elevata) a pazienti critici dal punto di vista nutrizionale sarà associata a una maggiore massa muscolare, a una migliore sopravvivenza e un più rapido tasso di recupero.

Criterio di inclusione:
- - ≥18 anni
- Nutrizionalmente "ad alto rischio" (che soddisfa uno dei seguenti criteri)
- BMI basso (≤25) o alto (≥35)
- Malnutrizione da moderata a grave (come definita da valutazioni locali). Documenteremo i mezzi con cui i siti stanno effettuando questa determinazione e cattureremo gli elementi della valutazione (storia della perdita di peso, storia della ridotta assunzione orale, ecc.).
- Fragilità (scala di fragilità clinica 5 o più da proxy)
- Sarcopenia- (punteggio SARC-F di 4 o più da proxy)
- Dal punto di screening, durata prevista della ventilazione meccanica >4 giorni
- - Necessità di ventilazione meccanica con durata totale effettiva o prevista della ventilazione meccanica >48 ore
Criteri di esclusione:
- >96 ore continue di ventilazione meccanica prima dello screening
- Morte attesa o sospensione dei trattamenti di sostentamento vitale entro 7 giorni dallo screening
- Incinta
- Il medico responsabile ritiene che il paziente abbia bisogno di proteine basse o alte
- Il paziente necessita solo di nutrizione parenterale e il sito non ha prodotti per raggiungere il gruppo ad alto dosaggio proteico.

Per i pazienti con BMI >30, verrà utilizzato il peso corporeo ideale basato su un BMI di 25.
Appoggeremo le linee guida per gli obiettivi energetici stabiliti da ASPEN/SCCM, soprattutto per quanto riguarda il paziente obeso.
I pazienti saranno sottoposti alle misure ecografiche al basale (entro 24 ore dalla randomizzazione, giorno 10 giorni dopo la randomizzazione (se ancora in ospedale) e appena prima della dimissione dall'ospedale.
