Thymus Transplantatie Veiligheid-werkzaamheid
Veiligheid en werkzaamheid van thymustransplantatie bij volledige DiGeorge-anomalie, IND#9836
Volledige DiGeorge-anomalie (cDGA) is een aandoening waarbij er geen thymusfunctie is. Zonder thymusfunctie ontwikkelen beenmergstamcellen zich niet tot opgeleide T-cellen, die infecties bestrijden. Zonder succesvolle behandeling moeten patiënten met cDGA in omgekeerde isolatie blijven om infectie en daaropvolgende dood te voorkomen.
Gekweekt thymusweefsel met en zonder immunosuppressie (geneesmiddelen die voor en na implantatie worden gegeven) heeft geresulteerd in de ontwikkeling van een goede T-celfunctie bij proefpersonen met een complete DiGeorge-afwijking.
Deze uitgebreide toegangsstudie zet onderzoek naar de veiligheid en werkzaamheid van gekweekt zwezerikweefsel voort voor de behandeling van complete DiGeorge-afwijkingen. In aanmerking komende deelnemers ontvangen gekweekt thymusweefsel. Immuunfunctietesten worden gedurende één jaar na implantatie voortgezet.

Gedetailleerde beschrijving
Volledige DiGeorge-anomalie (cDGA) is een aangeboren aandoening die wordt gekenmerkt door athymie. Zonder succesvolle behandeling moeten patiënten met cDGA in omgekeerde isolatie blijven om infectie en daaropvolgende dood te voorkomen. Bij patiënten met cDGA heeft implantatie van gekweekt zwezerikweefsel met en zonder immunosuppressie geleid tot diverse T-celontwikkeling en goede T-celfunctie.
Het doel van deze uitgebreide toegangsstudie is om onderzoek naar de veiligheid en werkzaamheid van gekweekt zwezerikweefsel voor de behandeling van athymie bij patiënten met cDGA voort te zetten. Totdat de toediening van gekweekt zwezerikweefsel door de FDA is goedgekeurd als standaardzorg voor cDGA, is deelname aan een onderzoeksstudie de enige manier waarop een patiënt toegang kan krijgen tot deze potentieel levensreddende procedure.
Dit protocol omvat 4 groepen: één voor proefpersonen die geen immunosuppressie nodig hebben; en 3 immunosuppressiegroepen voor proefpersonen met verschillende T-celfunctieniveaus die adequaat moeten worden onderdrukt.
In aanmerking komende proefpersonen ontvangen gekweekt thymusweefsel en kunnen een allograftbiopsie ondergaan.
Protocol gespecificeerde onderzoeken gaan door tot ongeveer een jaar na implantatie. De studiedeelname duurt twee jaar.

Beschrijving
Inclusiecriteria voor implantatie van gekweekt zwezerikweefsel:
- Moet 1 van de volgende hebben: 22q11.2ds of 10p13 hemizygositeit; hypocalciëmie die moet worden vervangen; aangeboren hartafwijkingen; of CHARGE-syndroom of CHD7-mutatie
- Volledige DiGeorge: <50 CD3+ T-cellen/cumm of <50 CD3+ T-cellen/cumm die CD62L+ CD45RA+ zijn, of <5% van de CD3+-cellen zijn CD62L+ CD45RA+
- Atypische DiGeorge-proefpersonen moeten huiduitslag hebben of hebben gehad.
Groep 1
•Typisch cDGA waarvan de T-cellen een fytohemagglutinine (PHA)-respons hebben van < 5.000 tellingen per minuut (cpm) en een < 20-voudige PHA-respons.
Groep 2
•Typisch cDGA waarvan de T-cellen een PHA-respons hebben van >5.000 cpm en <50.000 cpm en >20-voudige PHA-respons.
Groep 3
- Typische cDGA waarvan de T-cellen een PHA-respons hebben van >50.000 cpm.
- Typische cDGA met maternale innesteling
- Atypische cDGA waarvan de T-cellen een PHA-respons hebben van <40.000 cpm bij immunosuppressie of <75.000 cpm op PHA wanneer ze geen immunosuppressie ondergaan.
- Atypische cDGA met groep 3 PHA-respons en maternale innesteling
Groep 4
- Atypische cDGA met PHA-responsen van >75.000 cpm zonder immunosuppressie of PHA-responsen van >40.000 cpm tijdens immunosuppressie.
- Atypische cDGA met maternale innesteling en groep 4 PHA-respons
Uitsluitingscriteria voor implantatie van gekweekt zwezerikweefsel:
- Hartoperatie uitgevoerd minder dan 4 weken voorafgaand aan de verwachte implantatiedatum.
- Hartoperatie verwacht binnen 3 maanden na het voorgestelde tijdstip van implantatie
- Afwijzing door chirurg of anesthesioloog als chirurgische kandidaat
- Gebrek aan voldoende spierweefsel om een transplantatie te accepteren
- HIV-infectie
- Eerdere pogingen tot immuunreconstitutie, zoals beenmergtransplantatie of eerdere thymustransplantatie
- CMV-infectie: Voor Groep 2, 3 en 4 CMV-infectie gedocumenteerd door >500 kopieën/ml in het bloed door PCR op twee opeenvolgende assays.
- Beademingsafhankelijkheid of positieve drukondersteuning: beademingsondersteuning of positieve drukondersteuning, zoals continue positieve luchtwegdruk (CPAP) of Bi-level positieve luchtwegdruk (BiPAP) ondersteuning voor een aandoening die als ernstig of onomkeerbaar wordt beschouwd of die het onderwerp te klinisch onstabiel om de procedures te ondergaan.
