Hypoalgetische effecten van specifieke versus niet-specifieke cervicale manipulatie bij gezonde proefpersonen
Deze studie evalueert het hypoalgetische effect van globale en specifieke cervicale gewrichtsmanipulatie bij gezonde individuen.
Eerst kregen de deelnemers een van de interventies en na 48 uur de andere.

Cervicale gewrichtsmanipulatie (CJM) wordt vaak gebruikt voor pijnbehandeling.
Gewrichtsmanipulatie genereert een reeks prikkels in het centrale zenuwstelsel door de activering van proprioceptoren in het gewrichtskapsel of de spieren, stimuleert de PAG en veroorzaakt pijnremming door activering van niet-opioïde dalende remmende routes.

Deelname Criteria

- zonder pijnklachten in de afgelopen 90 dagen
Uitsluitingscriteria:
- eerdere operaties aan de wervelkolom
- stenose van het wervelkanaal
- wervel fractuur
- spondylolisthesis
- kanker
- acute infecties
- hemorragische aandoeningen
- actieve tuberculose
- diepe veneuze trombose
- osteoporose
- reumatische aandoeningen
- stofwisselingsziekten
- cardiorespiratoire aandoeningen
- roken letsel
- musculoskeletaal letsel
- gebruik van een pacemaker
- vrouwen tijdens de menstruatie
- zwangere vrouw
- gebruik van pijnstillers in de afgelopen 48 uur

Actieve vergelijker: Wereldwijde manipulatie
Manuele therapie uitgevoerd in de cervicale regio op een niet-specifieke manier.

De therapeut nam plaats aan het hoofdeinde van de brancard en ondersteunde met één hand de zijkant van het gezicht van de deelnemer.
Ondersteun met de andere hand het tegenovergestelde vlak van hetzelfde en voer vervolgens contralaterale rotatie van de cervicale wervelkolom uit en een lichte tractie op de weefselbarrière om de manipulatie-impuls van hoge snelheid en korte amplitude in rotatie uit te voeren.
De procedure werd bilateraal uitgevoerd.

Experimenteel: Specifieke manipulatie
manuele therapie specifiek uitgevoerd op het C6-7-segment
|
De therapeut positioneerde zich aan het hoofdeinde van de brancard en hield met één hand het middelste vingerkootje van de tweede vinger lateraal op de articulaire processen van de C6-7 wervels.
Met de contralaterale hand ondersteunde hij het tegenoverliggende gezicht van de deelnemer door een kanteling uit te voeren tot aan het C6-7-segment en contralaterale rotatie tot aan de weefselbarrière om de impuls snel en kort uit te voeren.
De procedure werd bilateraal uitgevoerd.

Tijdsspanne: 1 minuut na het einde van de interventie.

De proefpersonen werden op hun rug op een brancard gelegd met hun ellebogen volledig uitgestrekt en het voorste deel van de onderarm op de brancard.
Proefpersonen kregen de instructie om tijdens pogingen hun ogen gesloten te houden.
De onderzoeker gebruikte een Somedic Type II digitale drukalgometer (Somedic Inc, Hörby, Zweden) op het achterste deel van de onderarm, 10 cm onder de laterale epicondylus van de elleboog in de richting van de ringvinger, waarbij de onderarm in pronatie bleef.
De druk werd vervolgens loodrecht op de huid uitgeoefend met een snelheid van 40 kPa / s met behulp van een platte ronde sonde van 1 cm2 bedekt met 1 mm rubber om huidpijn veroorzaakt door scherpe metalen randen te voorkomen.
