Beschrijving
INSLUITINGSCRITERIA
Patiënten moeten aan ALLE volgende criteria voldoen:
- Bereid en in staat om geïnformeerde toestemming te geven
- Volwassen mannen ≥ 18 jaar
- Geschiedenis van histologisch bevestigd adenocarcinoom van de prostaat zonder bewijs van neuro-endocriene of kleincellige differentiatie. Als histologie niet beschikbaar is, moeten patiënten een gemetastaseerde ziekte hebben die typerend is voor prostaatkanker (d.w.z. waarbij bot- of bekkenlymfeklieren of para-aortale lymfeklieren betrokken zijn) EN een serumconcentratie van PSA die stijgt en >20 ng/ml op het moment van prostaatkanker werd klinisch gediagnosticeerd
- Toestemming voor analyse van archiefmateriaal dat bij de diagnose is verzameld, is verplicht
- Voorafgaande chirurgische orchiectomie of indien op LHRH-agonist/antagonist, testosteron < 1,7 nmol/L bij screeningbezoek (patiënten moeten LHRH-agonist/antagonist-therapie behouden voor de duur van de studiebehandeling indien niet chirurgisch gecastreerd)
- Bewijs van uitgezaaide ziekte op botscan of CT-scan
Bewijs van biochemische of beeldvormende progressie in de setting van chirurgische of medische castratie terwijl abirateron wordt gebruikt. Progressieve ziekte voor deelname aan de studie wordt gedefinieerd door een van de volgende drie criteria volgens PCWG317:
- PSA-progressie: minimaal twee stijgende PSA-waarden vanaf een nulmeting met een interval van een week. Minimale PSA bij screeningbezoek is 1,0 ng/ml
- Progressie van weke delen of viscerale ziekte: een toename van ≥20% in de som van de diameter (korte as voor nodale laesies en lange as voor niet-nodale laesies) vanaf de kleinste som van de diameter sinds de start van de behandeling, of het verschijnen van nieuwe laesies (zie appendix B voor de definitie van meetbare ziekte volgens de criteria van RECIST 1.1).
- Botprogressie: ≥ 2 nieuwe laesies op botscan bevestigd op daaropvolgende botscan met een tussenpoos van ten minste 8 weken (2+2 regel volgens PCWG317)
- ECOG-prestatiestatus 0-2 (zie Bijlage C)
- Voorafgaande behandeling met abirateron, in castratiegevoelige of castratieresistente setting.
- Komt in aanmerking voor behandeling met enzalutamide of docetaxel volgens de standaardzorgrichtlijnen
Adequate orgaanfunctie gedefinieerd als:
- Absoluut aantal neutrofielen ≥ 1,5 x 109/l, aantal bloedplaatjes ≥ 100 x 109/l en hemoglobine ≥ 90 g/l
- Creatinineklaring ≥ 30 ml/min (berekend met de formule van Cockcroft-Gault, zie bijlage D)
- Totaal bilirubine ≤ 1,5 x bovengrens van normaal (ULN) behalve voor patiënten met bekend syndroom van Gilbert (direct bilirubine ≤ 1,5 x ULN)
- Alanineaminotransferase (ALAT) ≤ 5 x ULN
- In staat om het onderzoeksgeneesmiddel door te slikken en te voldoen aan de onderzoeksvereisten, waaronder het verstrekken van perifere bloedmonsters op gespecificeerde tijdstippen voor correlatieve onderzoeken
- Herstel van alle eerdere behandelingsgerelateerde toxiciteit tot graad ≤ 2 (volgens CTCAE 5.0)
UITSLUITINGSCRITERIA
Patiënten mogen NIET aan een van de volgende criteria voldoen:
- Ernstige gelijktijdige ziekte of comorbide ziekte die de proefpersoon ongeschikt zou maken voor inschrijving
- Voorafgaande therapie met enzalutamide of andere experimentele anti-androgenen (bijv. ARN-509, TOK-001)
- Eerdere systemische chemotherapie met docetaxel of cabazitaxel (met uitzondering van: patiënten die werden behandeld met docetaxel voor castratiegevoelige ziekte en geen progressie vertoonden gedurende ten minste 12 maanden na voltooiing van docetaxel)
- Actieve gelijktijdige maligniteit (met uitzondering van niet-melanomateuze huidkanker of andere solide tumoren die curatief zijn behandeld zonder bewijs van ziekte gedurende ≥3 jaar)
- Wide-field radiotherapie of radio-isotopen zoals Strontium-89 of Radium-223 ≤ 28 dagen voorafgaand aan het starten van het onderzoeksgeneesmiddel (limited-field palliatieve radiotherapie tot 5 fracties voorafgaand aan het starten van het onderzoeksgeneesmiddel is toegestaan)
- Hersenmetastasen of actieve epidurale ziekte (behandelde epidurale ziekte is toegestaan)
- Contra-indicatie voor prednisontherapie, waaronder slecht gecontroleerde diabetes mellitus
- Geschiedenis van convulsies of convulsies, of geschiedenis van een cerebrovasculaire gebeurtenis binnen 6 maanden na aanvang van de studie.
- Ongecontroleerde hypertensie Graad ≥3 (d.w.z. systolische bloeddruk ≥160 mmHg of diastolische bloeddruk ≥100 mmHg)
- Maagdarmstoornis die de absorptie beïnvloedt
- Grote operatie binnen 4 weken na aanvang van de studiebehandeling
