Gedetailleerde beschrijving
Dit is een single-center prospectieve studie ter evaluatie van EUS-geleide RFA van PCN's (figuur 1). De studie is van plan patiënten gedurende een periode van 3 jaar in te schrijven. Patiënten met een definitieve diagnose van een PCN en met onbetaalbare risico's voor chirurgie zullen opeenvolgend worden gerekruteerd om EUS-geleide RFA te ondergaan, gevolgd door standaardzorgtoezicht.
De volgende beoordelingen zullen worden voltooid. Radiologische en EUS-onderzoeken voorafgaand aan de rekrutering moeten worden uitgevoerd binnen 3-6 maanden na geplande inschrijving en behandeling. De pre-rekruteringsevaluatie maakt deel uit van de standaardbeoordeling van de standaardzorg (SOC) van PCN's.
- Voorbehandeling/basislijn Dit zijn standaardzorgpraktijken voor het beheer van PCN's die zijn gedefinieerd door de International Consensus Guidelines (ICG) en de PCL-richtlijnen van het American College of Gastroenterology.
Vereisten voorbehandeling:
1. MRI/MRCP (maand -6 tot dag 0). Beeldvorming wordt niet uitgevoerd als deze al beschikbaar is als onderdeel van de praktijkstandaard bij het behandelen van PCL's 2. Pancreasprotocol CT-abdomen (maand -6 tot dag 0) (als een patiënt geen MRI/MRCP kan ondergaan) 3. Lichamelijk onderzoek (inclusief ECOG (Eastern Cooperative Oncology Group) prestatiestatus) 4. Laboratorium: Complete Blood Count (CBC), Basic Metabolic Panel (BMP), Leverfunctietesten (LFT's), Protrombinetijd (PT) Test & International Normalized Ratio (INR) bloedtest, serum CA19 -9 (kankerantigeen 19-9), serumamylase, lipase, Chromogranine A (voor cystic-NET). Laboratoriumtests worden niet uitgevoerd als deze al beschikbaar zijn als onderdeel van de praktijkstandaard bij het behandelen van PCL's en standaardzorg voorafgaand aan routinematige EUS-FNA (fijne naaldaspiratie).
5. EUS met FNA (volgens ICG) met ten minste een van de volgende geavanceerde diagnostiek - EUS-nCLE (naaldgeleide confocale laserendomicroscopie) of cystevloeistofanalyse door NGS (Next-generation sequencing).
6. Experimentele arm: Cystevloeistof zal ook worden verzonden voor flowcytometrie voorbehandeling voor immunologische markers van antineoplastische respons (baseline)
Behandeling (Dag 0):
- Fysiek onderzoek
- Geïnformeerde toestemming
- Zwangerschapstests, indien geïndiceerd, worden niet uitgevoerd als deze al beschikbaar zijn als onderdeel van de praktijkstandaard bij het beheer van PCL's en standaardzorg voorafgaand aan routinematige EUS-FNA.
- Toediening van pre-procedure profylaxe tegen post-EUS-RFA pancreatitis. Dit is de standaard profylaxe die wordt gebruikt om pancreatitis te voorkomen tijdens ERCP-procedures (Endoscopische retrograde cholangiopancreatografie). Hierbij wordt Ringers-lactaat gebruikt als IV-vloeistof naar keuze, per rectale indomethacine toegediend tijdens de procedure, en alleen een helder vloeibaar dieet is toegestaan na de procedure (tot de volgende ochtend).
- Eén dosis IV chinolon-antibioticum (of equivalent als patiënt allergisch is voor chinolon)
- Anesthesie: De standaard anesthesie die vereist is voor elke EUS-procedure zal tijdens de procedure worden toegepast. Anesthesie kan gecontroleerde anesthesiezorg (MAC) of algemeen zijn. Zoals gebruikelijk zullen alle patiënten die voor EUS worden doorverwezen, verpleegkundige triage ondergaan. Indien en wanneer geïndiceerd, worden patiënten voorafgaand aan de procedure doorverwezen naar de OSU Preoperatieve Assessment Clinic (OPAC) voor beoordeling door middel van anesthesie.
- EUS-FNA (indien voldoende sturen voor cystevloeistofanalyse volgens SOC) en EUS-RFA van PCN
- Intramurale opname voor nachtelijke observatie
- Pas de AGREE-classificatie toe voor bijwerkingen, documenteer alleen Graad II en hoger aangezien patiënten ter observatie worden opgenomen
- Als de patiënt na de procedure acute pancreatitis heeft, documenteer dan de ernst op basis van de herziene Atlanta-classificatie
Post-procedure dag # 1:
1. Monitoring van symptomen en bijwerkingen - AGREE-classificatie en herziene Atlanta-classificatie (bijlage 3) bij patiënten met acute pancreatitis 2. Lichamelijk onderzoek 3. Laboratorium: CBC, BMP, LFT's, lipase op post-procedure dag # 1
- Follow-up- en responsbeoordeling Dit zijn standaardzorgpraktijken voor het beheer van PCN's gedefinieerd door International Consensus Guidelines (ICG)3 en/of American College of Gastroenterology Guidelines.21

Beschrijving
Inclusiecriteria:
- Leeftijd >18 jaar.
- Een diagnose van een PCN bevestigd door EUS-FNA inclusief cystevloeistof NGS en/of EUS-nCLE en/of EUS-TTNB (via de naaldbiopsie).
- De PCL meet ten minste 2 cm in diameter op CT of MRI/MRCP of EUS en vertoont zorgwekkende en/of risicovolle kenmerken zoals gedefinieerd door International Consensus Guidelines (2017 herziene Fukuoka-richtlijnen).
De patiënt komt niet in aanmerking voor een chirurgische ingreep na overleg met Hepato-Pancreato-Biliary (HPB)-chirurgie en/of beoordeling in de conferentie van de multidisciplinaire tumorcommissie van de pancreas.
Veelvoorkomende klinische scenario's zijn onder meer -
- Cirrose van de lever (veel voorkomend klinisch scenario)
- Hogere leeftijd (veel voorkomend klinisch scenario)
- Morbide obesitas
- Aanzienlijke cardiorespiratoire comorbiditeit
- Keuze van de patiënt (patiënt kiest voor niet-chirurgische behandeling)
- Andere significante comorbide aandoeningen die chirurgische risico's belemmeren
- Geschatte levensverwachting van minimaal 1 jaar.
- In staat om schriftelijke geïnformeerde toestemming te geven.
- Vrouwen die zwanger kunnen worden, moeten op de dag van de behandeling een negatieve zwangerschapstest (serum/urine) hebben. Zwangerschapstesten zijn de standaard zorgpraktijk in het endoscopielaboratorium voor alle patiënten die endoscopie ondergaan en sedatie voor endoscopie.
- De patiënt geeft de voorkeur aan niet-chirurgische behandeling na overleg met Hepato-Pancreato-Biliary (HPB) chirurgie
- De patiënt is geen chirurgische kandidaat en heeft eerdere pogingen tot ablatie van de PCN door EUS-geleide injectie van chemotherapie gehad (OSU IRB-protocol 2020C0198)
Uitsluitingscriteria:
- Een diagnose van een goedaardige of niet-neoplastische PCL zoals een pseudocyste bevestigd door EUS-FNA inclusief cystenvloeistof NGS en/of EUS-nCLE en/of EUS-TTNB.
- Een diagnose van een maligne PCN bevestigd door bewijs van adenocarcinoom en/of invasief carcinoom en/of metastasen op afstand
- Cysten of NETS (neuro-endocriene tumoren) waarbij bloedvaten of de galboom zijn betrokken of in de nabijheid daarvan, waar de ablatiezone deze structuren waarschijnlijk in gevaar brengt.
- PCN's zoals IPMN's waarbij het hoofdkanaal van de alvleesklier is betrokken, zoals bij gemengde IPMN-laesies.
- Acute pancreatitis in de voorgaande 4 weken voorafgaand aan de datum van EUS-RFA
- Elk bewijs van ernstige of ongecontroleerde systemische ziekten of laboratoriumbevindingen die het volgens de onderzoeker onveilig maken voor de patiënt om aan het onderzoek deel te nemen.
- Elke psychiatrische stoornis die betrouwbare geïnformeerde toestemming onmogelijk maakt.
- Zwangerschap of borstvoeding.
- ECOG-prestatiestatus 4.
- Contra-indicatie voor algehele anesthesie na beoordeling door OSU Preoperatieve Assessment Clinic (OPAC)
- Cardiale implanteerbare elektrische apparaten

Ander: EUS-geleide radiofrequente ablatie van pancreascystische laesie(s)
Eenarmige studie.
EUS-geleide interventie met radiofrequente ablatie van pancreas cystische laesies.
Primaire populatie van belang: vertakkingskanaal IPMN (intraductaal papillair slijmvliesneoplasma) met een hoog risico op chirurgische ingreep.
De ingreep wordt uitgevoerd met behulp van een door de FDA goedgekeurd apparaat (EUS-RFA-sonde).

EUS-RFA wordt uitgevoerd met een 19 gauge RFA-naald (STARMed/TaeWoong Medical USA) die tot 30 watt (W) stroom toedient met Continuance Mode-instelling.
RFA-bevestiging wordt endoscopisch bevestigd via EUS (witte bellen op EUS-beeldvorming) en impedantiebewaking op het VIVA™ Combo-systeem.
Impedantie heeft een maximale waarde tot 800 Ohm.
Sequentiële doses elektrische energie bij 10W-30W worden toegepast om de PCN weg te nemen.
Door de FDA goedgekeurd apparaat (EUS-RFA-sonde).
