O estudo EuroSIDA é um estudo de coorte observacional prospectivo de mais de 23.000 pacientes acompanhados em mais de 100 clínicas em 35 países europeus, Israel e Argentina. O estudo é o maior estudo de coorte pan-europeu e poucos estudos de desenho comparável estão disponíveis em escala global. O estudo EuroSIDA é uma colaboração contínua e os pacientes foram inscritos no estudo por meio de 11 coortes desde 1994.
O principal objetivo do estudo permanece o mesmo de 1994: estudar prospectivamente dados clínicos, terapêuticos, demográficos, virológicos e laboratoriais de pessoas HIV-1 positivas em toda a Europa, a fim de determinar seus resultados virológicos, imunológicos e clínicos a longo prazo.
Historicamente, a EuroSIDA tem sido crucial em relatar as principais mudanças na epidemia de HIV, como as mudanças dramáticas na morbidade e mortalidade quando a terapia anti-retroviral combinada (cART) foi introduzida. À medida que o novo tratamento anti-HCV é introduzido para pacientes co-infectados com HIV/HCV, é importante que a EuroSIDA permaneça na vanguarda da investigação dos benefícios e efeitos adversos do tratamento.

Em termos gerais, o objetivo do estudo EuroSIDA é continuar uma coleta prospectiva de longo prazo de dados clínicos, laboratoriais e terapêuticos, bem como plasma em uma grande coorte de pacientes consecutivos infectados pelo HIV em toda a Europa, a fim de (1) avaliar os fatores associados ao curso clínico, imunológico e virológico da infecção pelo HIV e co-infecções e comorbidades relacionadas ao HIV, e (2) continuar a fornecer e desenvolver um sistema de vigilância para descrever mudanças temporais e diferenças regionais no curso clínico da HIV e co-infecções e comorbidades relacionadas ao HIV na Europa.

Resumo: Existem atualmente mais de 1,5 milhão de pessoas infectadas com HIV na Europa. A epidemia continua a se intensificar na região da Europa Oriental, onde a prevalência do HIV continuará a aumentar nos próximos anos. Existem problemas significativos com a gestão desta crise de saúde pública. A terapia antirretroviral (ART) disponível - embora extremamente eficaz - não erradica o HIV e, portanto, deve ser continuada por toda a vida. Outras limitações são o desenvolvimento de resistência, os efeitos adversos do tratamento e a exigência de adesão estrita. Apesar dessas limitações, o uso generalizado de TARV potente resultou em uma diminuição dramática na mortalidade relacionada ao HIV em toda a Europa. À medida que a incidência de AIDS diminuiu, a importância relativa de comorbidades e coinfecções, como hepatite viral crônica e tuberculose (TB), aumentou. Cerca de um terço de todos os pacientes EuroSIDA estão co-infectados com o vírus da hepatite C (HCV), e a morte relacionada ao fígado é agora a segunda causa mais comum de morte depois da AIDS. Com a introdução de novos antivirais orais de ação direta (DAAs) mais potentes e melhor tolerados contra o VHC, são esperadas grandes mudanças no manejo e no desfecho da co-infecção pelo VHC nos próximos anos. No entanto, devido ao alto custo dos DAAs, o acesso a novos tratamentos pode variar substancialmente na Europa.
Mais de 23.000 pacientes HIV-1 positivos inscritos consecutivamente em mais de 100 centros clínicos em 35 países europeus, Israel e Argentina - um quarto da região leste - estão atualmente inscritos no EuroSIDA. Novas coortes de pacientes são normalmente inscritas a cada 2-3 anos para garantir que todas as regiões da Europa onde a epidemia é prevalente sejam representadas, de modo que o estudo forneça informações oportunas sobre a apresentação clínica e o resultado dos pacientes europeus HIV-1 positivos. Em 2012, 2.500 pacientes adicionais foram incluídos na coorte IX da EuroSIDA. Para estar na vanguarda da investigação dos benefícios e efeitos adversos do novo tratamento de HCV em pacientes coinfectados, a coorte X da EuroSIDA inscrita em 2014 consistia em 4.000 pacientes HIV-1 positivos para anticorpos contra HCV. A próxima coorte a ser inscrita (coorte XI) em 2019 será composta por 1.500 pacientes HIV-1 positivos.
O grupo de estudo EuroSIDA trabalha no estudo EuroSIDA desde 1994 e tem várias realizações notáveis até o momento, incluindo a publicação de mais de 200 artigos em periódicos revisados por pares (incluindo o New England Journal of Medicine e o Lancet, entre outros). O foco da EuroSIDA mudou naturalmente ao longo deste período, o que demonstra uma vontade de ser flexível, dinâmico e focado em questões contemporâneas, e o grupo de estudo está empenhado em continuar trabalhando com tais princípios.
Nos últimos anos, a EuroSIDA, além das publicações científicas, priorizou a capacitação na Europa Oriental com seminários sobre HIV, recebendo médicos da Europa Oriental como alunos da European AIDS Clinical Society (EACS) no escritório do Copenhagen HIV Program e patrocinando 2-3 jovens médicos da Europa Oriental para participar de um curso de estatística em Londres todos os anos.
A maioria dos dados são coletados nas clínicas como parte dos cuidados de rotina. Além disso, o repositório central de plasma será usado para ampliar estudos anteriores da epidemiologia viral do HIV (resistência e subtipos).
Objetivo primário do estudo: Estudar prospectivamente dados clínicos, terapêuticos, demográficos, virológicos e laboratoriais de pessoas HIV-1 positivas em toda a Europa, a fim de determinar o resultado virológico, imunológico e clínico de longo prazo. Os objetivos específicos, divididos em quatro categorias principais, são os seguintes:
- Examinar a eficácia da TARV e os fatores que a limitam
- Para detectar eventos adversos atuais ou emergentes de início tardio entre pacientes em TARV
- Continuar a vigilância do HIV em clínicas em toda a Europa para descrever mudanças temporais e diferenças regionais
- Monitorar a aceitação e o resultado da terapia de HCV e o desenvolvimento de antivirais de ação direta e comparar as diferenças entre as regiões EuroSIDA
Visitas do estudo: Inscrição + acompanhamento Coleta de dados: Para todos os pacientes HIV-1 positivos inscritos e sob acompanhamento, são coletados dados laboratoriais, terapêuticos e clínicos sobre HIV, hepatite viral e eventos clínicos graves não relacionados à AIDS, bem como dados demográficos e possíveis dados sobre a gravidez e o estado de HIV do recém-nascido. Os pacientes são atendidos em suas clínicas conforme necessário e de acordo com o médico local. O EuroSIDA não envolve entrevistas com pacientes ou visitas de estudo, as informações são coletadas das anotações dos pacientes duas vezes por ano até 2017, doravante uma vez por ano.

Para os pacientes incluídos no estudo de 1994-1997, foi solicitado que os pacientes tivessem uma contagem de agrupamento de diferenciação 4 (CD4) < 500 células/µL nos últimos 5 meses antes da inclusão.
Para todos os pacientes com HIV inscritos e sob acompanhamento, os dados laboratoriais, terapêuticos e clínicos são coletados duas vezes por ano.
Dados demográficos, data da gravidez e evidências sorológicas para infecção pelo vírus da hepatite B (HBV) e vírus da hepatite C (HCV) são coletados uma vez por ano.

Para os pacientes incluídos no estudo de 1994-1997, foi solicitado que os pacientes tivessem uma contagem de agrupamento de diferenciação 4 (CD4) < 500 células/µL nos últimos 5 meses antes da inclusão.
Para todos os pacientes com HIV inscritos e sob acompanhamento, os dados laboratoriais, terapêuticos e clínicos são coletados duas vezes por ano.

Para todos os pacientes com HIV inscritos e sob acompanhamento, os dados laboratoriais, terapêuticos e clínicos são coletados duas vezes por ano.

Os pacientes a serem incluídos foram aqueles que compareceram ao ambulatório para uma consulta agendada (ou seja,
consulta feita mais de 2 semanas antes da hora da visita).
Os critérios de elegibilidade de um agrupamento de contagem de diferenciação 4 (CD4) < 500 células/µL nos últimos 5 meses antes da inclusão foram removidos para pacientes que ingressaram no estudo em 1999 e além, uma vez que a intenção do estudo é incluir a maioria dos pacientes elegíveis para terapia antirretroviral.
Para todos os pacientes com HIV inscritos e sob acompanhamento, os dados laboratoriais, terapêuticos e clínicos são coletados duas vezes por ano.

Todos os pacientes devem ser pacientes consecutivos (exceto aqueles já inscritos no EuroSIDA) ou pacientes com consulta agendada (ou seja,
aqueles que marcaram consulta > 2 semanas antes da visita) no ambulatório, independentemente da contagem de células do grupo de diferenciação 4 (CD4) e do estado de ART).
Inscrever pacientes com 16 anos ou mais e positivos para anticorpos contra o vírus da hepatite C (independentemente do status de HCV-RNA, estágio de fibrose e tratamento anterior contra hepatite C).

Os dados clínicos são centrados na coleta de variáveis laboratoriais e na determinação de eventos clínicos significativos predefinidos, bem como das razões para alterar e/ou interromper os regimes de tratamento.
O conceito de estudo de coorte observacional significa que a EuroSIDA não coleta Eventos Adversos, Reações Adversas, Reações Adversas Graves ou Suspeitas de Reações Adversas Graves Inesperadas, conforme definido pelos requisitos de farmacovigilância para titulares de autorização de introdução no mercado.
No entanto, a EuroSIDA frequentemente analisa e relata eventos clínicos específicos de interesse, ocasionalmente relacionados a classes de medicamentos, mas mais regularmente com foco nas diferenças regionais no tratamento e cuidados.
