Vliv perorálního podávání konopí a současného podávání alkoholu na poškození

Tato studie bude hodnotit individuální a interaktivní účinky orálního konopí a alkoholu na subjektivní a behaviorální míry poškození.

Detailní popis
Tato klinická laboratorní studie bude dvojitě zaslepená, placebem kontrolovaná a bude využívat experimentální design v rámci jednotlivých subjektů.
Účastníci absolvují 7 ambulantních sezení s podáváním drog, která se budou skládat z vlastní aplikace orálního konopí (0, 10 nebo 25 mg THC) a alkoholu (buď placebo nebo aktivní látka; BAC 0,05 procenta); účastníci vždy dostanou jak alkoholický nápoj (aktivní nebo placebo), tak dávku konopí (aktivní nebo placebo).
Účastníci také absolvují stav, kdy podávají alkohol (BAC: 0,08 procenta) s konopím s placebem jako pozitivní kontrolu.
Primární výsledky zahrnují výkon v terénních testech střízlivosti, kognitivní a psychomotorické poruchy, subjektivní účinky drog a simulovaný jízdní výkon.
Rovněž budou stanoveny koncentrace THC a metabolitů THC v krvi.

Kritéria pro zařazení:
- Poskytli písemný informovaný souhlas
- Být ve věku od 21 do 50 let
- Být v dobrém celkovém zdravotním stavu na základě fyzikálního vyšetření, anamnézy, vitálních funkcí a screeningu moči a krevních testů
- Nesmí být těhotná nebo kojící (pokud je žena). Všechny ženy musí mít negativní těhotenský test v séru při screeningové návštěvě a negativní těhotenský test v moči při každé studijní návštěvě.
- Mít index tělesné hmotnosti (BMI) v rozmezí 19 až 36 kg/m2
- Krevní tlak při screeningové návštěvě nepřesahuje systolický krevní tlak (SBP) 150 mmHg nebo diastolický krevní tlak (DBP) 90 mmHg
- Nedarovali jste krev v předchozích 30 dnech.
- Nahlaste alespoň 2 dny nadměrného pití za posledních 90 dní (více než 4 nebo 5 nápojů při jedné příležitosti u žen a mužů)
- Uveďte ≥ 5 užití konopí za poslední rok
- Poskytněte negativní test moči na užívání nelegálních drog (kromě THC) a negativní test na alkohol v dechu (0 % BAC) při screeningu a před studijním sezením
- Nahlaste alespoň 1 případ současného požití alkoholu a užití v posledním roce.
Kritéria vyloučení:
- Užívání psychoaktivních drog (kromě konopí, nikotinu, alkoholu nebo kofeinu) za poslední měsíc
- Současné užívání volně prodejných (OTC) léků, doplňků/vitamínů nebo léků na předpis, které podle názoru zkoušejícího nebo zdravotnického personálu ovlivní bezpečnost účastníka
- Historie nebo současný důkaz významného zdravotního stavu
- Důkaz o současném psychiatrickém stavu [(MINI for Diagnostic and Statistical Manual (DSM)-V)]
- Splnit kritéria pro těžkou poruchu užívání alkoholu (MINI pro DSM-V)
- Hodnocení klinického ústavu pro hodnocení alkoholové stupnice (CIWA-Ar) > 9
- V minulosti se léčil s poruchou užívání alkoholu nebo konopí
- Užívání konopí v průměru více než 2krát týdně za poslední 3 měsíce
- Testy jaterních funkcí jsou více než 2x normální
- Zařazení do jiné klinické studie nebo příjem jakéhokoli léku v rámci výzkumu během posledních 30 dnů
- Shipleyho skóre slovní zásoby <18 (odpovídá úrovni čtení 5. třídy).

Acute cognitive and behavioral impairment will be assessed with global impairment score (range 0-100) on the DRUID app (higher scores indicate greater impairment).
Results are mean change from baseline.

Impairment will be assessed using a battery of standard field sobriety tests including: the Horizontal Gaze Nystagmus Test (HGN), the Walk and Turn, the One Leg Stand, and the Modified Romberg Balance.
We will report the cumulative amount of clues observed across these tasks (out of a possible 0-22 clues).
Higher scores indicate greater/worse impairment.

SDLP (measured in cm) was calculated across all drives to get a composite index of lateral control and reported as peak change from baseline.
SDLP is the gold standard of quantifying the magnitude of driving impairment from drugs and alcohol and has excellent predictive validity to actual driving.

Higher scores represent higher magnitude of driving impairment.
