Prospectivo de artritis de la articulación basal (BJAM)
Estudio prospectivo multicéntrico sobre la artritis de la articulación basal del pulgar
El estudio plantea la hipótesis de que existen tratamientos quirúrgicos y no quirúrgicos efectivos para ciertas poblaciones de pacientes con artritis de la articulación basal del pulgar. También existe un régimen de tratamiento quirúrgico no quirúrgico o mínimamente invasivo funcionalmente superior, rentable y de bajo riesgo para aliviar el dolor y retrasar la progresión de la enfermedad en aquellos con enfermedad menos avanzada. Del mismo modo, existe una importante utilidad funcional, para la salud y una ventaja económica en el tratamiento quirúrgico de la artritis de la articulación basal avanzada con uno de los procedimientos popularizados actualmente en la práctica.
La artritis en la base del pulgar es una condición común asociada con una incomodidad considerable para el paciente. Se han descrito muchos tratamientos quirúrgicos y no quirúrgicos, pero se han realizado pocos estudios prospectivos que involucren a múltiples centros que comparen estos tratamientos. Al seguir a los pacientes desde el comienzo de su tratamiento para la artritis y evaluar su progreso en determinados momentos mediante cuestionarios y radiografías, los investigadores creen que los resultados y la satisfacción de los pacientes pueden mejorar. Este estudio incluirá a pacientes que reciben tratamiento quirúrgico y no quirúrgico y no cambiará el estándar de atención de los pacientes. Al establecer medidas de resultado estandarizadas y recopilar datos prospectivamente desde el momento de la presentación inicial a través de todas las intervenciones de tratamiento, será posible comparar directamente los tratamientos durante el cuidado conservador y quirúrgico de la artritis de la articulación basal del pulgar. Este estudio multicéntrico identificará aquellos procedimientos específicos o tratamientos conservadores que tienen el mayor potencial para ser estudiados en posteriores ensayos controlados aleatorios centrados y bien diseñados.

La artritis de la articulación basal del pulgar es una condición común asociada con una morbilidad considerable. Se han descrito muchos tratamientos quirúrgicos y no quirúrgicos, pero se han realizado pocos ensayos multicéntricos prospectivos basados en evidencia que comparen los tratamientos estándar. Esta búsqueda continua de consenso sobre las mejores prácticas clínicas se revisó en un metanálisis completo de los tratamientos quirúrgicos para la artritis de la articulación basal. A través de la recopilación sistemática de datos sobre las características específicas del paciente, las intervenciones de tratamiento y las mediciones de resultados funcionales longitudinales, los investigadores creen que los resultados y la satisfacción del paciente pueden mejorar mediante la aclaración de los factores de riesgo para la progresión de la enfermedad y el momento y la selección de las modalidades de tratamiento, ya sea conservador o quirúrgica, para cualquier paciente en particular. El establecimiento de un registro clínico multicéntrico facilitará en gran medida estos objetivos. Al establecer medidas de resultado estandarizadas y recopilar datos prospectivamente desde el momento de la presentación inicial a través de todas las intervenciones de tratamiento, será posible comparar directamente los tratamientos durante el cuidado conservador y quirúrgico de la artritis de la articulación basal del pulgar. La osteoartritis es la forma más común de artritis y la segunda causa más frecuente de discapacidad a largo plazo en América del Norte. La osteoartritis de la articulación basal del pulgar es una enfermedad debilitante que afecta a un gran segmento de la población adulta, aunque no se han realizado grandes estudios epidemiológicos sobre la prevalencia de la artritis de la articulación basal en los Estados Unidos.
La patogenia de la artrosis sigue siendo multifactorial, pero la mayoría de los investigadores coinciden en que se trata de un proceso degenerativo resultante de factores mecánicos, bioquímicos y genéticos que desestabilizan el equilibrio normal de degradación y síntesis del cartílago articular y el hueso subcondral.
Aunque hay algunos estudios excelentes con diseño prospectivo aleatorizado que muestran buenos resultados con varias técnicas quirúrgicas, aún se desconoce qué procedimientos son los más apropiados para que un paciente en particular logre el resultado funcional óptimo y el éxito a largo plazo con un trauma quirúrgico mínimo. Tampoco se conoce el tipo y la duración óptimos de la inmovilización. También se desconoce el efecto de la hiperextensión de la articulación metacarpofalángica sobre los resultados a largo plazo y el método de tratamiento óptimo para esta articulación adyacente. Este estudio propuesto permitirá la recopilación prospectiva de datos de múltiples instituciones en todo el país e involucrará a expertos reconocidos, todos los cuales tratan la artritis de la articulación basal con diferentes protocolos clínicos.
Millones de personas en los EE. UU. tienen o desarrollarán osteoartritis de la articulación basal, que es la articulación más comúnmente afectada en la extremidad superior que requiere intervención quirúrgica. La demanda de una forma más eficaz y rentable de tratamiento quirúrgico y no quirúrgico de esta enfermedad incapacitante es considerable. La identificación de tratamientos seguros y efectivos entre las numerosas modalidades de tratamiento disponibles probablemente conduciría a mejoras significativas en la calidad de vida de un gran segmento de esta población. Si los estudios futuros muestran que las reconstrucciones menos complicadas, como la trapeciectomía sola, son tan efectivas como las reconstrucciones de ligamentos más complicadas con interposición de tendones, muchos pacientes pueden beneficiarse de esta cirugía menos compleja. Este ensayo aumentaría la conciencia y la confianza de médicos y pacientes en los regímenes de tratamiento efectivos.

Descripción
Criterios de inclusión:
- Pacientes que tienen artritis de la articulación basal sintomática.
- Pacientes que son capaces de dar su consentimiento informado
Criterio de exclusión:
- Pacientes menores de 18 años en el momento de la inscripción
- Pacientes con enfermedad neuromuscular que afecta a la mano operada, no causada por la operación de CMC
- Pacientes con afecciones artríticas inflamatorias conocidas, como artritis reumatoide o psoriásica
- Pacientes con antecedentes o infección actual de la articulación basal de la mano afectada.
- Pacientes con demencia o incapaces de dar su consentimiento informado.
- Pacientes que no pueden cumplir con las pautas del estudio.
- Los pacientes que tienen hiperextensión de la articulación metacarpofalángica NO están excluidos. Estos pacientes serán seguidos y si reciben una capsulodesis u otro procedimiento en el MCPJ al mismo tiempo que su artroplastia de la articulación basal, esto se anotará en sus registros de datos.
